  • Application and ethical exploration of ChatGPT in medical clinical practice

    Following the rapid advancement of artificial intelligence technologies, especially the development of large language models like ChatGPT, the field of medical clinical practice is undergoing an unprecedented technological revolution. These advanced technologies, through efficient processing and analysis of large datasets, not only provide medical professionals with auxiliary diagnoses and treatment suggestions but also significantly enhance the quality and efficiency of medical education. This study conducts a comprehensive analysis and review of the applications of large language models in various aspects, including clinical inquiry, history collection, medical literature writing, clinical decision support, optimization of medical portal websites, patient health management, medical education, academic research, and scientific writing. However, the application of these technologies is not without flaws and presents several limitations and ethical challenges. This paper focuses on challenges related to technological errors, academic dishonesty, abuse risks, over-reliance, possibilities of misdiagnosis and treatment errors, and issues of accountability. In conclusion, large language models demonstrate tremendous potential in the integration and advancement of medical practices. Nevertheless, while fully harnessing the benefits brought by ChatGPT, it is essential to acknowledge and address these ethical challenges to ensure that the application of ChatGPT in the medical field is responsible and effective.

  • 18F-FDG PET/CT combined with CT three-dimensional reconstruction in the differentiation of benign and malignant pulmonary nodules: A retrospective cohort study

    Objective To investigate the accuracy of 18F-FDG positron emission tomography/computed tomography (PET/CT) combined with CT three-dimensional reconstruction (CT-3D) in the differential diagnosis of benign and malignant pulmonary nodules. Methods The clinical data of patients who underwent pulmonary nodule surgery in the Department of Thoracic Surgery, Northern Jiangsu People's Hospital from July 2020 to August 2021 were retrospectively analyzed. The preoperative 18F-FDG PET/CT and chest enhanced CT-3D and other imaging data were extracted. The parameters with diagnostic significance were screened by the area under the receiver operating characteristic (ROC) curve (AUC). Three prediction models, including PET/CT prediction model (MOD PET), CT-3D prediction model (MOD CT-3D), and PET/CT combined CT-3D prediction model (MOD combination), were established through binary logistic regression, and the diagnostic performance of the models were validated by ROC curve. Results A total of 125 patients were enrolled, including 57 males and 68 females, with an average age of 61.16±8.57 years. There were 46 patients with benign nodules, and 79 patients with malignant nodules. A total of 2 PET/CT parameters and 5 CT-3D parameters were extracted. Two PET/CT parameters, SUVmax≥1.5 (AUC=0.688) and abnormal uptake of hilar/mediastinal lymph node metabolism (AUC=0.671), were included in the regression model. Among the CT-3D parameters, CT value histogram peaks (AUC=0.694) and CT-3D morphology (AUC=0.652) were included in the regression model. Finally, the AUC of the MOD PET was verified to be 0.738 [95%CI (0.651, 0.824)], the sensitivity was 74.7%, and the specificity was 60.9%; the AUC of the MOD CT-3D was 0.762 [95%CI (0.677, 0.848)], the sensitivity was 51.9%, and the specificity was 87.0%; the AUC of the MOD combination was 0.857 [95%CI (0.789, 0.925)], the sensitivity was 77.2%, the specificity was 82.6%, and the differences were statistically significant (P<0.001). Conclusion 18F-FDG PET/CT combined with CT-3D can improve the diagnostic performance of pulmonary nodules, and its specificity and sensitivity are better than those of single imaging diagnosis method. The combined prediction model is of great significance for the selection of surgical timing and surgical methods for pulmonary nodules, and provides a theoretical basis for the application of artificial intelligence in the pulmonary nodule diagnosis.

  • The application scenarios study on the intervention of cognitive decline in elderly population using metaverse technology

    China is facing the peak of an ageing population, and there is an increase in demand for intelligent healthcare services for the elderly. The metaverse, as a new internet social communication space, has shown infinite potential for application. This paper focuses on the application of the metaverse in medicine in the intervention of cognitive decline in the elderly population. The problems in assessment and intervention of cognitive decline in the elderly group were analyzed. The basic data required to construct the metaverse in medicine was introduced. Moreover, it is demonstrated that the elderly users can conduct self-monitoring, experience immersive self-healing and health-care through the metaverse in medicine technology. Furthermore, we proposed that it is feasible that the metaverse in medicine has obvious advantages in prediction and diagnosis, prevention and rehabilitation, as well as assisting patients with cognitive decline. Risks for its application were pointed out as well. The metaverse in medicine technology solves the problem of non-face-to-face social communication for elderly users, which may help to reconstruct the social medical system and service mode for the elderly population.

  • Preliminary study on prediction model based on CT for pathological complete response of rectal cancer after neoadjuvant chemotherapy

    ObjectiveTo explore the value of a decision tree (DT) model based on CT for predicting pathological complete response (pCR) after neoadjuvant chemotherapy therapy (NACT) in patients with locally advanced rectal cancer (LARC).MethodsThe clinical data and DICOM images of CT examination of 244 patients who underwent radical surgery after the NACT from October 2016 to March 2019 in the Database from Colorectal Cancer (DACCA) in the West China Hospital were retrospectively analyzed. The ITK-SNAP software was used to select the largest level of tumor and sketch the region of interest. By using a random allocation software, 200 patients were allocated into the training set and 44 patients were allocated into the test set. The MATLAB software was used to read the CT images in DICOM format and extract and select radiomics features. Then these reduced-dimensions features were used to construct the prediction model. Finally, the receiver operating characteristic (ROC) curve, area under the ROC curve (AUC), sensitivity, and specificity values were used to evaluate the prediction model.ResultsAccording to the postoperative pathological tumor regression grade (TRG) classification, there were 28 cases in the pCR group (TRG0) and 216 cases in the non-pCR group (TRG1–TRG3). The outcomes of patients with LARC after NACT were highly correlated with 13 radiomics features based on CT (6 grayscale features: mean, variance, deviation, skewness, kurtosis, energy; 3 texture features: contrast, correlation, homogeneity; 4 shape features: perimeter, diameter, area, shape). The AUC value of DT model based on CT was 0.772 [95% CI (0.656, 0.888)] for predicting pCR after the NACT in the patients with LARC. The accuracy of prediction was higher for the non-PCR patients (97.2%), but lower for the pCR patients (57.1%).ConclusionsIn this preliminary study, the DT model based on CT shows a lower prediction efficiency in judging pCR patient with LARC before operation as compared with homogeneity researches, so a more accurate prediction model of pCR patient will be optimized through advancing algorithm, expanding data set, and digging up more radiomics features.

  • Insights and prospectives of ophthalmologic artificial intelligence technology

    For the past few years, artificial intelligence (AI) technology has developed rapidly and has become frontier and hot topics in medical research. While the deep learning algorithm based on artificial neural networks is one of the most representative tool in this field. The advancement of ophthalmology is inseparable from a variety of imaging methods, and the pronounced convenience and high efficiency endow AI technology with promising applications in screening, diagnosis and follow-up of ophthalmic diseases. At present, related research on ophthalmologic AI technology has been carried out in terms of multiple diseases and multimodality. Many valuable results have been reported aiming at several common diseases of ophthalmology. It should be emphasized that ophthalmic AI products are still faced with some problems towards practical application. The regulatory mechanism and evaluation criteria have not yet integrated as a standardized system. There are still a number of aspects to be optimized before large-scale distribution in clinical utility. Briefly, the innovation of ophthalmologic AI technology is attributed to multidisciplinary cooperation, which is of great significance to China's public health undertakings, and will be bound to benefit patients in future clinical practice.
