  • Missed Diagnosis of Sleep Apnea Hypopnea Syndrome: Analysis of 42 Cases and Literature Review

    Objective To analyze the causes of missed diagnosis of sleep apnea hypopnea syndrome ( SAHS) . Methods 42 missed diagnosed cases with SAHS from May 2009 to May 2011 were retrospectively analyzed and related literatures were reviewed. Results The SAHS patients often visited the doctors for complications of SAHS such as hypertension, diabetes mellitus, metabolic syndrome, etc. Clinical misdiagnosis rate was very high. Lack of specific symptoms during the day, complicated morbidities, and insufficient knowledge of SAHS led to the high misdiagnosis rate and the poor treatment effect of patients with SAHS. Conclusion Strengthening the educational propaganda of SAHS, detail medical history collection, and polysomnography monitoring ( PSG) as early as possible can help diagnose SAHS more accurately and reduce missed diagnosis.

  • Current status of metabolic syndrome and its influencing factors among physical examination population aged 60 years old and above in Chengdu

    Objective To analyze the current status of metabolic syndrome (MetS) and its influencing factors among physical examination population aged 60 years old and above in Chengdu, and to provide evidence for the screening and management of MetS in people aged 60 years old and above. Methods Retrospective analysis of the data of people aged 60 years old and above who participated in physical examination in the North District of the Department of Health Management of Sichuan Provincial People’s Hospital between January 2018 and December 2020. According to whether the elderly occurred to have MetS, they were divided into MetS group and non-MetS group, to observe the distribution of MetS in different genders and different age groups, and to analyze the influencing factors of MetS by multivariate logistic regression. Results A total of 10 335 elderly were enrolled, 2 769 cases (26.79%) in MetS group and 7 566 cases (73.21%) in non-MetS group. Except for gender and low-density lipoprotein cholesterol (LDL-C) (P>0.05), there were significant differences in other general data between the two groups (P<0.05). There was a statistically significant difference in the detection rate of MetS among different age groups (χ²=64.332, P<0.001). In MetS group, 951 cases (34.34%) of abdominal obesity, 716 cases (25.86%) of high fasting blood glucose (FBG), 1 938 cases (69.99%) of hypertension, 1 011 cases (36.51%) of high triglycerides (TG) and 303 cases (10.94%) of low high-density lipoprotein cholesterol (HDL-C) were detected. Except hypertension and high TG (P>0.05), there were statistically significant differences in the detection rates of abdominal obesity, high FBG and low HDL-C among different genders in MetS (P<0.05). Except hypertension (P<0.05), there was no significant difference in the detection rates of MetS abdominal obesity, high FBG, low HDL-C and high TG in different age groups (P>0.05). Logistic regression analysis showed that body mass index [odds ratio (OR)=1.055, 95% confidence interval (CI) (1.014, 1.097), P=0.008], systolic blood pressure [OR=1.032, 95%CI (1.027, 1.037), P<0.001], diastolic blood pressure [OR=0.992, 95%CI (0.985, 1.000), P=0.049], FBG [OR=1.853, 95%CI (1.764, 1.947), P<0.001], TG [OR=3.787, 95%CI (3.352, 4.278), P<0.001], hemoglobin [OR=0.991, 95%CI (0.986, 0.996), P<0.001], waist circumference [OR=1.158, 95%CI (1.141, 1.175), P<0.001], HDL-C [OR=0.243, 95%CI (0.170, 0.346), P<0.001], total cholesterol [OR=0.615, 95%CI (0.490, 0.773), P<0.001], LDL-C [OR=1.594, 95%CI (1.303, 1.948), P<0.001], and uric acid [OR=1.001, 95%CI (1.000, 1.002), P=0.042] were related to MetS. Conclusions The prevalence of MetS is high among people aged 60 years old and above who undergo physical examination. Routine screening should be carried out during physical examination to take measures to intervene the controllable risk factors, so as to improve the health level of the region.

  • Effect of intradialytic progressive resistance exercise on hemoglobin and iron metabolism in maintenance hemodialysis patients

    ObjectiveTo investigate the effect of 24-week intradialytic progressive resistance exercise on hemoglobin and iron metabolism in maintenance hemodialysis (MHD) patients.MethodsFrom April to May 2019, 62 MHD patients were enrolled and randomly assigned into exercise group (n=31) and control group (n=31). Both groups of patients received regular routine hemodialysis, on that basis, patients in the exercise group completed intradialytic resistance exercise three times per week for 24 weeks. Each exercise included 8-10 muscle groups (grasping the grip ring with both hands, flexion and extension of the elbows and shoulders on the non-vascular side and lower limbs with sandbag), 3 sets of 15 repetitions with a rest of 1-2 min between 2 sets. Exercise began with a low load, the sandbag weight was gradually increased, and the Borg score was aimed to be 11-13 points after exercise. Hemoglobin, serum ferritin, transferrin saturation, serum creatinine, high-sensitivity C-reactive protein, urea clearance index, recombinant human erythropoietin (rHuEPO) dosage at baseline and after 24 weeks, as well as the cumulative iron supplement dose and hemoglobin variation of the two groups during the study period were evaluated.ResultsThere were 20 patients in the exercise group and 30 ones in the control group who completed the study. After 24 weeks of progressive resistance exercise, the medium (lower quartile, upper quartile) of the amount of rHuEPO in the exercise group decreased from 6 000 (6 000, 9 000) U/week to 6 000 (4 500, 7 125) U/week (Z=−2.599, P=0.009), while that in the control group had no statistically significant difference (Z=−1.340, P=0.180); there was no statistically difference in hemoglobin, hemoglobin coefficient of variation, serum ferritin, transferrin saturation, or 24-week cumulative iron supplementation between the two groups.ConclusionIntradialytic progressive resistance exercise can reduce the amount of rHuEPO in MHD patients, which is benefitial to optimizing the management of hemoglobin.

  • A meta-analysis of the comparison of intervention effects of high-intensity interval training and moderate-intensity continuous training on patients with metabolic syndrome

    Objective To explore the difference of intervention effect between high-intensity interval training (HIIT) and moderate-intensity continuous training (MICT) on patients with metabolic syndrome (MetS). Methods China National Knowledge Infrastructure, WanFang Data, PubMed, Web of Science and EBSCO were searched for randomized controlled trials (RCTs) till May 2022. Two reviewers independently reviewed the literature, extracted data, and assessed the risk of bias of included RCTs. Comprehensive Meta-Analysis software was used for meta-analysis. Result A total of 5 RCTs were included, including 216 patients. The results of meta-analysis showed that: except fasting blood glucose, high-density lipoprotein cholesterol, systolic blood pressure, waist circumference, body mass index and body fat percentage (P>0.005), low-density lipoprotein cholesterol [mean difference (MD)=−7.487 mg/dL, 95% confidence interval (CI) (−12.543, −2.431) mg/dL, P=0.004], total cholesterol [MD=−11.487 mg/dL, 95%CI (−16.523, −6.452) mg/dL, P<0.001], triglycerides [MD=−26.296 mg/dL, 95%CI (−50.557, −2.035) mg/dL, P=0.034] and diastolic blood pressure [MD=−2.770 mm Hg (1 mm Hg=0.133 kPa), 95%CI (−5.131, −0.409) mm Hg, P=0.021] of HIIT were better than MICT. Conclusion In terms of blood glucose indicators and morphological indicators, the effect of HIIT group and MICT group was similar, but the effect of HIIT on blood lipid indicators and blood pressure indicators of patients with MetS was better than MICT.

  • Advances in role of nicotinamide N-methyltransferase in gastric cancer and related mechanisms

    ObjectiveTo systematically review the role and potential clinical value of nicotinamide N-methyltransferase (NNMT) in gastric cancer, aiming to provide new insights for diagnosis, treatment, and prognosis of gastric cancer. MethodsRelevant literature from recent years on the involvement of NNMT in gastric cancer was thoroughly analyzed. The review focuses on the mechanisms by which NNMT influences cell proliferation, invasion, migration, and metabolic reprogramming in gastric cancer. Additionally, the study explores the potential of NNMT as a diagnostic and therapeutic target. ResultsNNMT was significantly overexpressed in gastric cancer tissues and was closely associated with tumor progression. It promoted malignant behaviors through various pathways, including metabolic regulation, enhancement of cancer cell proliferation and invasion, and alteration of the tumor microenvironment. Furthermore, NNMT played a crucial role in modulating host immune responses, which might impact the clinical prognosis of gastric cancer patients. ConclusionsNNMT exhibits significant biological functions in the development and progression of gastric cancer. As a potential biomarker and therapeutic target, it holds promising clinical value in the diagnosis and targeted treatment of gastric cancer, providing new strategies and evidence for precision therapy and prognosis assessment.

  • Expression of inducible nitric oxide (NO) synthase and argininemetabolic relative enzymes in retinal pigment epithelial (RPE) cells and the effect of NO on tight junction of RPE cells

    ObjectiveTo detect the induction of inducible nitric oxide synthase (iNOS) and nitric oxide (NO) production in immunostimulated retinal pigment epithelial (RPE) cells to seek for the supplying of the arginine, a substrate for NOS; as well as the effects of produced NO on the tight junction of RPE-J cells. MethodsRat′s RPE-J cells were treated with interferon-γ(INF-γ), tumor necrosis factor-α(TNF-α) and lipopolysaccharide (LPS), and Northern and Western blotting were applied to analyze the expression of the citrulline-NO cycle enzymes and related enzymes and the effect of dexamethasone and cyclic adenosine monophosphate (camp) on the expression of iNOS. Immunocytochemistry reaction and Western blotting were used to evaluate the effect of produced NO on the tight junctions of RPE-J cells.ResultsiNOS and argininosuccinate synthetase (AS) were highly induced at both mRNA and protection levels in immunostimulated RPE cells while arginiosuccinate lyase (AL) was not induced. NO was produced by cells after stimulation with TNFα, IFNγ and LPS. The induction of iNOS mRNA and the production of NO by these immunostimulated cells was further enhanced by cAMP. NO was produced from citrulline as well as from arginine. And the produced NO impaired the tight junction of RPE-J cells, decreased the production of tight junction related protein ZO-1.ConclusionIn activated RPE-J cells, citrullinearginine recycling is important for NO production, and the produced NO weakened the function of tight junction of RPE-J cells.(Chin J Ocul Fundus Dis, 2005,21:32-36)

  • Effect of tear film stability after vitrectomy combined with silicone oil tamponade

    Objective To observe the effect of vitrectomy (PPV) combined with silicone oil filling on the stability of the tear film. Methods A total of 72 eyes of 36 patients with vitreous hemorrhage and retinal detachment were enrolled in the study with PPV combined with silicone oil filling. The operation and contralateral eyes were set up in the operation group and the control group respectively, each had 36 eyes. The tear film rupture time (BUT), the base tear secretion test or Schirmer Ⅰ test (SⅠT) and corneal fluorescein staining (CFS) were performed at 7, 30, 60, and 90 days after operation. The difference of BUT, SⅠT and CFS at different time points after the operation of the two groups were compared. Results After operation 7, 30 days, SⅠT and CFS increased, BUT staining is shortened in the surgery group, the differences were statistically significant (t=1.78, P<0.05); after operation 60, 90 days, SⅠT, CFS, BUT were same between the surgery group and the contralateral eyes (t=12.39, P>0.05). Conclusion PPV combined with silicone oil filling can affect the stability of the tear film, which can be recovered to the preoperative level at postoperative 60 days.
