目的 了解我院住院患者抗菌药物的临床应用现状及存在问题,为临床合理使用抗菌药物提供参考。 方法 采用回顾性调查方法,对本院2008年11月-2009年4月的出院病历资料进行统计、分析。 结果 共调查病历1 000份,抗菌药物总使用率58.70%;其中预防用药使用率62.35%,治疗用药使用率37.65%;联合用药的比例为37.31%;不合理用药占19.76%。 结论 抗菌药物使用率较高,且使用存在一些不合理现象。医院应加强监管,对存在的问题应制订相应措施。
Objective To Investigate the disease constitution and hospitalization expense in Luxi township health center (LxC) in Yongxi county of Jiangxi Province in 2010, to make clear about the local burden of diseases and to provide the baseline data for further study. Methods The inpatient records of LxC in 2010 were collected. Based on the primary diagnosis on hospital discharge record, the diseases were standardized and classified according to the International Classification of Disease, 10th Edition (ICD-10). Data including general information of the inpatients, discharge diagnosis, hospitalization expense and usage of essential medicine etc, were reorganized and analyzed by using Microsoft Excel 2003 and SPSS 13.0 software. Results a) The total number of inpatients were 925 in 2010, with male/female ratio of 0.8; b) The disease spectrum included 17 categories, accounting for 81% of the ICD-10; c) The top 5 diseases were in respiratory, digestive, injury, poisoning amp; external causes, circulatory and genitourinary system, totally accounting for 82.27%; d) The top 15 single diseases were upper respiratory infection, fracture, chronic obstructive lung disease (COPD), chronic gastroenteritis, cerebrovascular disease (CVD), calculi in urinary system, rheumatoid arthritis, intervertebral discs diseases, cholecyslithiasis accompanied with cholecystitis, cardiac disease, reproductive organ diseases, injury amp; poisoning, pneumonia, hypertension and peptic ulcer; e) The patients with upper respiratory infection and pneumonia were mostly older than 65 or younger than 5 years old. With the exception of calculi in urinary system and peptic ulcer, all the other 8 chronic diseases were mainly seen in patients over 65 years old; f) Among the 15 single diseases as listed above, the chronic diseases were associated with shorter average hospital stay and low average expense compared with the acute diseases (4.8 d vs. 11.6 d; ?439.1 vs. ?666.9); and g) The hospitalization expense of LxC, although increasing year by year, was still far below that of the national township health centers (?542.3 vs. ?1 004.6). Conclusion a) The top 3 in inpatients systematic diseases of LxC are respiratory system, digestive system, and injury and poisoning; the former 2 diseases attack more often in females, and the acute diseases are mainly infection and fracture; b) Except for rheumatoid arthritis, cholecyslithiasis accompanied cholecystitis, cardiac diseases, reproductive organ diseases and peptic ulcer, all the other 10 of the top 15 single diseases are similar to Yong’an township health center (YaC) in Sichuan Province in 2010; c) The acute diseases mainly focus on respiratory system, and injury and poisoning, and the chronic diseases mainly focus on digestive system, circulatory system, genitourinary system, the musculoskeletal system and connective tissue system; d) The number of patients who suffer from chronic diseases increases significantly when over of 35 years old, especially, often seen in female rather than male. The acute burden diseases is serious in patients less than 15 or more than 45 years old; e) The upper respiratory infection and pneumonia mainly affect the old and children; f) Compared with Xintian township health center (XtC) in Gansu Province, the average hospital stay of fracture patients is longer (43.7 d vs. 9.0 d), the hospitalization expense is higher (?1 948.0 vs. ?1 648.3), and the diseases is burden heavier (8.1% vs. 4.9%); and g) The average hospital stay of patients with acute diseases is longer than YaC and XtC (11.6 d vs. 3.7 d, 6.2 d), but the hospitalization expense is lower than both of them (?666.9 vs. ?850.4, ?906.9).
Objective To analyze the risk factors associated with prolonged length of hospital stay (PLOS) after lobectomy for lung cancer patients. Methods The clinical records of 771 lung cancer patients undergoing lobectomy between May 2012 and June 2016 in the Second Affiliated Hospital of Harbin Medical University were retrospectively analyzed. According to different length of hospital stay (LOS), 771 patients were divided into two groups, a normal LOS group and a PLOS group. In the normal LOS group, there were 551 patients including 234 females and 317 males with a median age of 59 years, whose LOS was shorter than 8.0 days. In the PLOS group, there were 220 patients including 72 females and 148 males with a median age of 60 years, whose LOS was no less than 8.0 days. Then, we analyzed the independent risk factors of PLOS by multivariate logistic regression analysis. Results Univariate analysis showed that risk factors for PLOS included male, arrhythmias and atrioventricular block, smaller FVC%, unilateral pneumonectomy, operation and anesthesia duration, intraoperative blood loss and number of lymph node dissection in the operation (P<0.05). Multivariate logistic regression analysis showed that the age ≥75 years (OR=4.100, 95%CI 1.677 to 10.026), unilateral pneumonectomy (OR=2.563, 95%CI 1.473 to 4.460), FVC% < 89.05% (OR=1.500, 95%CI 1.020 to 2.206), numbers of lymph node dissection≥ 13.5 (OR=1.826, 95%CI 1.262 to 2.642), operation duration≥126.5 min (OR=1.858, 95%CI 1.200 to 2.876) and arrhythmia (OR=2.944, 95%CI 1.380 to 6.284) were independent risk factors of PLOS (all P<0.05). Conclusion LOS is influenced by age, surgical type, FVC%, numbers of lymph node dissection, arrhythmia and operation duration. Careful assessment and appropriate management of risk factors are helpful to improve postoperative recovery after lobectomy for lung cancer patients.
ObjectiveTo analyze the association between the cultural level and hospitalization management process and length of hospitalization of the colorectal patients served by West China Hospital of Sichuan University as a regional center in the current version of the Database from Colorectal Cancer (DACCA). MethodAccording to the established screening criteria, eligible colorectal cancer patients were collected from the updated version of DACCA on June 29, 2022. The analyzed data items included gender, age, BMI, blood type, marriage, waiting time before admission, preoperative hospitalization time, postoperative hospitalization time, total hospitalization time, and management process, and patients were divided into illiterate group, primary education group, medium education group, and higher education group according to their educational level, then compared the hospitalization management process and length of hospitalization of each group. ResultsA total of 4 765 eligible data were screened, with secondary education being the most prevalent (2 792, 58.6%), followed by primary (1 337, 28.1%) and higher education (417, 8.7%), and illiteracy being the least prevalent (219, 4.6%). In the classification of management processes, “regular” account for the majority (4 219, 88.5%), followed by “enhanced”(274, 5.8%), “individual” was third (231, 4.8%), and “rapid” was the least (41, 0.9%). There was no statistically significant difference in the comparison of waiting time before admission, preoperative hospitalisation time and postoperative hospitalisation time among patients with different literacy levels (P=0.371, P=0.095, P=0.352), but there was a statistically significant difference in total hospitalisation time (P=0.021), with a significant difference in total hospitalisation length between illiterate patients and patients with medium education (P=0.041). There was no statistically significant difference in the comparison of inpatient management processes of patients in different literacy groups (χ2=15.2, P=0.085). ConclusionsAnalysis of the DACCA data revealed a statistically significant difference in total hospitalisation time between patients with illiteracy and those with medium education. However, the choice of hospitalisation management process was similar for patients with different literacy levels, which needs to be further analysed for the reasons.
Objective To investigate drug usage and costs of inpatients with asthma in Karamay Central Hospital in 2014 and to provide baseline for evidence-based pharmacy study of single disease in respiratory system. Methods The information of drug use and expenditure of asthma inpatients were collected from the hospital information system (HIS). We analyzed the data including frequency, proportion and cumulative proportion by Excel 2007 software. Results A total of 150 asthma inpatients were included, the average age was 56.25±18.83 years old. Three kinds of the most commonly used drugs were corticosteroids, antibiotics and antiasthmatic drugs. Budesonide suspension for inhalation, moxifloxacin needle, doxofylline needle accounted for 32.84%, 31.11% and 45.31% in these three categories of drugs, respectively. Conclusion The mainly drugs for treatment of asthma inpatients in Karamay Central Hospital in 2014 are corticosteroids, antibiotics and antiasthmatic drugs. The frequency of systemic corticosteroids is too high, and the frequency of antimicrobial use is irrational, which needs further specification.
Objective To understand the effect and influencing factors of humanistic care on improving the experience of inpatients. Methods Patients were collected from a third grade class A women’s and children’s hospital in June 2015 and June 2016, and their satisfaction was investigated by a third party. The service items of Inpatients Satisfaction Item Score Table in 2015 were analyzed. Appropriate intervention measures were taken to low-score items, such as humanistic knowledge training to all medical staff, improvement health guidelines, implementation of recycling process, carrying out high quality nursing interventions, and so on. The patients satisfaction survey results in 2016 were compared with those of 2015. Results In 2016, the total satisfaction rate (89.94%), and the average score of items ranked the top three (94.64±0.14), including the level of medical technology, medical ethics and the overall evaluation of doctor’s professional ehtics, medical communication and service attitude, were higher than those of 2015 (85.25, 90.86±1.53). The average score of items ranked the last three (89.25±9.21), including hospital ward, hospital environment (clean, quiet and safe), hospital meals and room service, and hospital food quality, was higher than that of 2015 (78.64±2.40). However, compared with the same period in the last year, the rank of hospital environment fell by two places. Conclusions Hardware conditions like physical environment have an important impact on the experience of hospital patients. However, humanistic care is the key factor to improve the patients’ inpatient experience and satisfaction.
