  • Effects of enteral nutrition support therapy on immune and nutritional status in patients with severe acute pancreatitis

    ObjectiveTo explore clinical effect of enteral nutrition (EN) through nasojejunal tube in severe acute pancreatitis (SAP).MethodsThe clinical and pathological data of 76 patients with SAP who met the inclusion criteria and treated in the Third People’s Hospital of Henan Province from June 2015 to December 2018 were retrospectively collected. According to the nutritional support therapy adopted in the course of treatment, the patients were divided into an EN group (n=44) and a parenteral nutrition group (PN group, n=32), the patients in the EN group were given the EN through the nasojejunal tube and the patients in the PN group were given the routine PN. The APACHE Ⅱ and SOFA scores, nutritional status, and immune function were observed before the treatment and on week 2 after the treatment in the two groups.ResultsThere were no significant differences in the general data of the gender, age, body mass index, etc. between the two groups (P>0.050). ① The score of APACHEⅡ or SOFA score on week 2 after the treatment in both groups was significantly lower than that before the treatment (P<0.050), which in the EN group was significantly lower than that in the PN group on week 2 after the treatment (P<0.050). ② The levels of albumin and prealbumin were increased significantly and the hemoglobin level was decreased (P<0.050) on week 2 after the treatment as compared with those before the treatment in the two groups (P<0.050), which in the EN group were significantly higher than those in the PN group on week 2 after the treatment (P<0.050). ③ The CD4, CD8, and CD4/CD8 and IgG, IgM, and IgA on week 2 after the treatment were significantly lower than those before the treatment in these two groups (P<0.050), which in the EN group were significantly higher than those in the PN group on week 2 after the treatment (P<0.050). ④ The levels of endotoxin, D-lactic acid, diamine oxidase, and serum high mobility group protein B1 on week 2 after the treatment were significantly lower than those before the treatment in the two groups (P<0.050), and which in the EN group were significantly lower than those of the PN group on week 2 after the treatment (P<0.050). ⑤ The complications rate in the EN group was significantly lower than that in the PN group [20.45% (9/44) versus 53.13% (17/32), χ2=8.786, P=0.003].ConclusionEN through nasojejunal tube has a good effect on patients with SAP, which is helpful to improve their immune and nutritional status.

  • Study on The Progress of Relationship Between Cellular Immunological Function and Primary Liver Cancer

    Objective To summarize the role of the relationship between liver cancer and cellular immunological function, and the role of immune therapy in clinical application. Methods To analyze the relationship between liver cancer and cellular immunological function, and the present research situation of immune therapy for liver cancer in clinical application retrospectively via review the related domestic and foreign literatures. Results The cellular immune dysfunction existed in all liver cancer patients. The state of body’s cellular immunological function is closely related with the arising and development of liver cancer, and the lowness of cellular immunological function is an important factor of hepatocellular carcinoma hard to cure or recurrence and metastasis. Immune therapy plays an important role in the treatment of liver cancer by adjusting the body’s cellular immunological function. Conclusions Liver cancer is closely related with the body’s cellular immunological function. Immune therapy is expected to offer a new way for the treatment of liver cancer, which can also be used as an important auxiliary treatment way.

  • Effect of High Intensity Focused Ultrasound on Immunity of Patients with Advanced Primary Liver Cancer

    【Abstract】ObjectiveTo investigate the effect of high intensity focused ultrasound (HIFU) on the immunity of patients with advanced primary liver cancer (PLC). MethodsForty cases of PLC admitted to our institution from Mar. 2003 to Dec. 2003 were included in this study. Patients were divided into 2 groups and received either HIFU or radiofrequency ablation (RFA) treatment randomly. CD3, CD4, CD8, CD4/CD8, NK, IL-2, TNF were chosen to assess the immune status before and after treatment. The results were compared statistically. ResultsThe survival rate after HIFU was 80.0%, 61.1%, 42.9%, 33.3% at 3 months, 6 months, 9 months and 1 year respectively, which was similar to that after RFA treatment. The changes of immunity parameters of CD3, CD4, CD8, CD4/CD8, NK, IL-2 and TNF were not significant after HIFU treatment. In addition, the differences of those parameters between HIFU group and RFA group were insignificant. ConclusionThere are no detrimental effects on immunity in the early period after HIFU treatment.

  • EFFECTS OF CIMETIDINE ON PERIPHERAL BLOOD T CELL SUBSETS AND INTERLEUKIN-2 SECRETIVE CELL AND LYMPHOCYTE TRANSFORMATION TEST IN PATIENTS WITH MALIGNANT TUMOR

    Cimetidine was given intravenously in daily dose of 20mg/kg to 20 patients with malignant tumor from the operative day on. The results show that cimetidine can elevate CD4, CD4/CD8 and decrease CD8. For comparison, blood samples from 40 patients were taken before operation for extracorporal testing. The results show that cimetidine can elevate lymphocyte transformation test and interleukin-2 secretive cell. These indicate that cimetidine can enhance the immune function of the patient and can be used as an adjunctive treatment of neoplasm.

  • Effects of Huaier Granule Combined with Systemic Chemotherapy on Immunologic Function and Prognosis for Advanced Breast Cancer Patients

    ObjectiveTo investigate the effects of Huaier granule combined with systemic chemotherapy on immunologic function and prognosis for advanced breast cancer patients. MethodsNinety-eight cases ofⅣstage breast cancer from March 2006 to March 2009 in this hospital were divided into control group and research group. Only systemic chemotherapy was performed in the control group, while Huaier granule combined with systemic chemotherapy was applied in the research group, and Huaier granule was given on day 1 systemic chemotherapy start. The changes of T lymphocyte subsets and IL-2 level were detected on day 1 before systemic chemotherapy and on month 6 after Huaier granule combined with systemic chemotherapy. The fatality rate and median survival time were also observed between two groups. ResultsCompared with the control group, the changes of T lymphocyte subsets and IL-2 level had no signi-ficant differences on day 1 before systemic chemotherapy between these two groups(P > 0.05). On month 6 after Huaier granule combined with systemic chemotherapy, the CD4+, CD4+/CD8+ T lymphocyte, and IL-2 level were significantly increased, the CD8+ T lymphocyte was significantly decreased in the research group as compared with the control group〔CD4+:(47.35±6.23)% versus(41.33±5.61)%, P < 0.05; CD4+/CD8+: 1.84±0.42 versus 1.47±0.33, P < 0.05; IL-2 level:(1.78±0.45)μg/L versus(1.58±0.30)μg/L, P < 0.05; CD8+:(23.26±3.25)% versus(29.77±4.12)%, P < 0.05〕. The rate of chemotherapy complications and fatality rate within 3 years were significantly decreased in the research group as compared with the control group〔rate of chemotherapy complications: 58.3%(28/48) versus 86.0%(43/50), P < 0.01; fatality rate within 3 years: 62.5%(30/48)versus 82.0%(41/50), P < 0.05〕. The median survival time in the research group was significantly longer than that in the control group(33.5 months versus 24.5 months, P < 0.01). ConclusionsThe preliminary results from this study show that Huaier granule combined with systemic chemotherapy could greatly enhance immune function, reduce side-toxicity of chemotherapy and improve prognosis in advanced breast cancer patients. It provides a beneficial exploration for cancer treatment by integration of traditional and western medicine.

  • Influences of Sentinel Lymph Node Biopsy Following Total Mastectomy on Immunologic Function and Prognosis for Patients with Early Breast Cancer

    ObjectiveTo evaluate the effects of sentinel lymph node biopsy following total mastectomy on immunologic function and prognosis for patients with early breast cancer. MethodsTwo hundred and eleven patients with early breast cancer were entered in this study. In all these cases, the results of sentinel lymph node biopsy were negative. These patients were randomly divided into control group and research group. In 86 cases of control group, the sentinel lymph node biopsy and axillary lymph node dissection following total mastectomy was performed. In 125 cases of research group, the sentinel lymph node biopsy following total mastectomy was performed. The injury of shoulder joint function was analyzed in one year after surgery. The changes of T cell subsets and IL-2 level were detected in the patients respectively on the first day before operation, the second week after operation, and the fourth week after operation. Postoperative fatality rate and postoperative recurrence rate were also observed in two groups. Results①The points of shoulder joint function in the control group and the research group were 72.7±6.5 and 93.5±8.2 respectively, there was an obvious difference (P < 0.05).②The injury degree of shoulder joint function in the research group was significantly lower than that in the control group (P < 0.01).③Compared with the control group, the changes of T cell subsets and the IL-2 level had no significant differences in the research group on day 1 before operation and on week 2 after operation (P > 0.05). On the fourth week after surgery, the CD4+, CD4+/CD8+, and IL-2 level in the research group were obviously higher than those in the control group (P < 0.05). However, the percentage of CD8+ T cell in the research group was significantly lower than that in the control group (P < 0.05).④There were no significant differences for postoperative fatality rate and postoperative recurrence rate between two groups (P > 0.05). ConclusionsSentinel lymph node biopsy for patients with early breast cancer is safe and reliable. With respect to conventional axillary lymph node dissection, it could improve immune function and quality of life after surgery in patients with early breast cancer.

  • Influence on Liver Function and Immune Function of Laparoscopic and Open Cholecystectomy

    目的探讨腹腔镜与开腹胆囊切除术对患者肝功能及免疫功能的影响。 方法根据手术方式将84例胆囊良性病变患者分为腹腔镜胆囊切除术组(LC组,50例)及开腹胆囊切除术组(OC组,34例),比较2组患者手术前后肝功能及免疫功能指标的变化。 结果2组患者肝功能在手术前后不同时间点的差异均无统计学意义(P>0.05);LC组患者术后免疫功能各指标与术前比较无明显变化(P>0.05),OC组CD3+、CD4+及CD4+/CD8+在术后1 d及3 d均较术前明显降低(P<0.05),术后7 d恢复至术前水平(P>0.05)。 结论LC术可引起患者术后肝功能短暂异常,但对免疫功能无明显影响,可作为胆囊切除的首选术式。

  • Comparison of postoperative immune function between da Vinci robot-assisted and video-assisted thoracoscopic surgery in patients with non-small cell lung cancer

    ObjectiveTo compare the effect on postoperative immune function between da Vinci robot-assisted thoracoscopic surgery (RATS) and video-assisted thoracoscopic surgery (VATS) , and to provide clinical support for more effective surgical procedures.MethodsA total of 90 patients undergoing radical resection of pulmonary carcinoma in our hospital from June to November 2019 were included. There were 49 males and 41 females with an average age of 62.67 (37-84) years. Among them, 50 patients underwent da Vinci robot-assisted thoracoscopic surgery (a RATS group) and 40 patients underwent video-assisted thoracoscopic surgery (a VATS group). The perioperative indexes as well as postoperative inflammatory factors and immune level effects between the two groups were compared.ResultsCompared with the VATS, RATS could significantly shorten the operation time and decrease intraoperative blood loss (P<0.05). RATS also effectively reduced the increase of postoperative inflammatory factor level (P<0.05). But there was no significant difference in postoperative immune function between the RATS group and the VATS group (P>0.05).ConclusionRATS is superior to VATS in the treatment of non-small cell lung cancer in perioperative indicators and inflammatory factors.

  • Effect of Marine Peptide on the Nutritional and Immune Function in Malignant Tumor Patients Undergoing Chemotherapy

    【摘要】 目的 探讨海洋肽对恶性肿瘤化学治疗(简称化疗)患者营养状况和免疫功能的影响。 方法 依照纳入排除标准选取2010年3-11月66例恶性肿瘤化疗患者,随机分为试验组和对照组,每组各33例。在正常饮食基础上,试验组和对照组分别服用海洋肽制剂和乳清蛋白制剂21 d,进行肝肾功能、营养状况及免疫指标的测定。 结果 干预前后两组肝肾功及血脂指标差异无统计学意义(Pgt;0.05),且均在正常范围内。试验组干预后体质指数(body mass index,BMI)、上臂围、上臂肌围、总蛋白、白蛋白、球蛋白、前白蛋白(prealbumin,PA)、转铁蛋白较干预前升高有统计学意义(Plt;0.05),而血红蛋白和三头肌皮褶厚度干预前后比较差异无统计学意义(Pgt;0.05);对照组干预前后各指标差异均无统计学意义(Pgt;0.05);试验组BMI、PA的前后差值较对照组高(Plt;0.05),而其他指标差值在两组间差异无统计学意义(Pgt;0.05)。两组在干预前后组内比较及组间免疫指标差值比较,差异均无统计学意义(Pgt;0.05)。 结论 海洋肽作为部分氮源应用于恶性肿瘤化疗患者,对患者的内脏蛋白、人体测量等均有一定的营养改善作用,但对免疫功能的影响不明显,尚待进一步研究。【Abstract】 Objective To investigate the effects of marine peptide on the nutritional status and immune function in malignant tumor patients undergoing chemotherapy. Methods According to inclusive and exclusive criteria, 66 malignant tumor patients undergoing chemotherapy from March 2010 to November 2010 were randomized into study group and control group with 33 patients in each group. The patients in the study group were given marine collagen peptide whey protein while those in the control group were given whey protein for 21 days. Liver and kidney function, nutritional status and immune function were observed before and after intervention. Results Liver, kidney function and blood lipids of all the patients were within normal range, and were not significantly different between the two groups before and after intervention (P>0.05). After intervention, body mass index (BMI), arm circumference, arm muscle circumference, total protein, albumin, globulin, prealbumin (PA), transferring protein of the study group were significantly increased (P<0.05), but hemoglobin and triceps skinfold thickness had no significant changes (P>0.05). There was no significant difference of the above parameters in the control group before and after intervention (P>0.05). The magnitude of change of PA and BMI before and after intervention were higher in the study group than those in the control group (P<0.05), while the magnitude of change of other parameters before and after intervention showed no significant difference between the two groups (P>0.05). The immune function showed no significant change in both groups before and after intervention (P>0.05), and it was also not significantly different between the two groups. Conclusion As part of dietary nitrogen sources, marine peptide can significantly improve nutritional status, including visceral protein and anthropometry in malignant tumor patients undergoing chemotherapy, but it has no significant effect on immune function, which should be further studied in detail.

  • 胸腺肽α1对小肝癌术后红细胞免疫功能的影响

    目的:研究胸腺肽α1对小肝癌术后红细胞免疫功能的影响。方法: 将我科收治的40例小肝癌患者,随机分为两组(n=20),A组为单纯手术切除组,B组为手术切除联合胸腺肽α1治疗(术后日达仙1.6 mg皮下注射,每周2次,共52周)组。分别于术前(T1)、治后1月(T2)、治后3月(T3)、治后6月(T4)采静脉血、离心、收集红细胞,混入等体积Rowe液后液氮保存,采用酵母菌玫瑰花环法测定红细胞C3b受体花环率(RBC-C3b-RR)、红细胞免疫复合物花环率(RBC-ICR)来检测红细胞免疫功能。术后采用超声、增强 CT或MRI等措施随访6月,判断肝癌肝内复发情况。结果: 术前两组红细胞免疫功能差异无统计学意义,术后RBC-ICR含量低于术前,但B组含量低于A组(Plt;0.05);术后RBC-C3bRR高于术前,但B组含量高于A组(Plt;0.05),表明B组红细胞免疫功能强于A组。术后6月A组6例肝癌肝内复发,B组2例肝癌肝内复发(Plt;0.05)。结论: 胸腺肽α1正性调节红细胞免疫黏附补体致敏的癌细胞的功能,增强机体免疫灭活癌细胞的能力,从而降低小肝癌术后复发。
