  • CLINICAL ANALYSIS OF ELECTRICAL STIMULATION THRESHOLD OF NERVE FASCICLE DURING SELECTIVE POSTERIOR RHIZOTOMY

    Abstract This experiment was to study the feasibility from direct observation of muscle contraction of the lower extremity fromelectrical stimulation threshold of nerve fascicle in identifying the Iα intrafusal afferent fibers during selective posterior rhizotomy (SPR) and to investigate the clinical relationship between the muscle spasm and the electrical stimulation of nerve fascicles. The electrical stimulation threshold of all nerve fascicles in 36 cases during SPR were analysed statistically. The results showed that there was a significant difference between the electrical stimulation threshold of the severed nerve fascicles and intact nerve fascicles no matter the nerve root or each posterior nerve rootlet was examined. It was simple and reliable for surgeons to identify correctly the Iα intrafusal afferent fibers intraoperatively from direct observation of the electrical stimulation threshold of nerve fascicle.

  • THE EFFECT OF HIGHLY SELECTIVE VAGOTOMY AND MUCOSAL ANTRECTOMY ON SERUM GASTRIN AND GASTRIC EMPTYING

    Forty-two patients with duodenal ulcer underwent highly selective vagotomy and mucosal antrectomy (HSV+MA) and were followed up for 3 years. Two weeks, 1 year and 3 year after operation, serum gastrim level and gastric emptying capacity were tested. The results show that he postoperative levels of serum gastrin were lower than the preoperative ones, but wih no significant difference (P>0.05). Only a few patients had delayed gastric emptying 2 weeks and 1year after operation,but it returned to normal in 3 years .The authors conclude that HSV+MA is a better operative treatment for duodenal ulcer since it can abolish the factors of postoperative ulcer recurence and perserve the functions of the antrum and the pylorus.

  • Clinical Study of External Dissection and Internal Ligation, Plus Partial Internal Sphincterotomy in the Treatment of Mixed Hemorrhoids

    ObjectiveTo investigate the effect of external dissection and internal ligation, plus partial internal sphincterotomy in the treatment of mixed hemorrhoids. MethodsDuring January 2010 to January 2012, 364 patients with mixed hemorrhoids selected for surgery were divided into two groups based on whether the patients should accept the treatment of partial internal sphincterotomy. We observed the curative effect of the two groups, including anal pain, bleeding, edema, average healing days and the anorectal stenosis after operation. ResultsThere were statistical differences between the two groups in terms of anal pain, complication rate and hospital stay (P<0.01). The curative effect of the group treated with partial internal sphincterotomy was better than that of the other group. ConclustionExternal dissection and internal ligation, plus partial internal sphincterotomy is a better choice in the treatment of mixed hemorrhoids, which can relieve postoperative symptoms, reduce complications and shorten treatment course.

  • Treatment for Chronic Anal Fissure with Inverted-V-Shaped Flap Repair Plus Internal Sphincterotomy (Report of 62 Cases)

    目的 评估倒“V”字皮瓣修复加内括约肌切断术治疗陈旧性肛裂的效果及合理性。方法 回顾性分析2000~2010年期间我院采用肛门后倒“V”字皮瓣修复加内括约肌切断治疗的62例陈旧性肛裂患者的临床资料,并对其效果进行评价。结果 62例患者经过该手术治疗,均痊愈出院。发生切口血肿4例,切口感染1例,暂时性大便渗漏4例。住院时间平均8.6d,术中出血量平均13ml,创口愈合时间平均8d。术后随访6~15个月(平均12个月),均未出现复发,排便情况满意。结论 该术式治愈率高,并发症少,是治疗陈旧性肛裂有效而可行的一种手术方式。

  • Quality of life in 243 patients with primary palmar hyperhidrosis after endoscopic thoracic sympathicotomy

    ObjectiveTo evaluate the quality of life (QOL) in patients with primary palmar hyperhidrosis (PPH) after endoscopic thoracic sympathicotomy (ETS) and analyze the influencing factors. MethodsA total of 243 patients (118 males and 125 females, with an average age of 21.99±6.31 years) with PPH who were successfully treated with ETS (only T3 level thoracic sympathicotomy) in our hospital from January 2017 to January 2018 were enrolled, and the World Health Organization Quality of Life Scale Brief Version (WHOQOL-BREF) was used to assess the QOL scores before and after ETS. By establishing a linear regression model of gender, age, body mass index, compensatory hyperhidrosis (CH) and palm dryness, and the relationship between the changes of the QOL scores and various factors was studied. ResultsThe total QOL score after surgery was higher than that before surgery (63.01±4.58 vs. 48.11±1.95, P<0.05). Compared with the negative group of CH, the QOL score decreased by 4.662 in the postoperative CH patients. For every grade of CH severity increasing, the QOL score decreased by 3.449. Compared with the negative group, the QOL scores decreased by 1.804 and 2.400 respectively for every grade of CH severity increasing in the patients with postoperative chest and back CH. ConclusionETS can not only improve the symptoms of abnormal palmar hyperhidrosis, but also significantly improve the QOL. Severe chest and back CH is an important factor affecting the QOL of patients.

  • The Effect of Highly Selective Vagotomy Plus Mucosal Antrectomy and Highly Selective Vagotomy Alone on Motor Function of Gastric Antrum

    Objective To investigate the effects of highly selective vagotomy plus mucosal antrectomy (HSVMA) and highly selective vagotomy (HSV) alone on the functions of the gastric antrum. MethodsEighteen dogs and 48 patients with chronic duodenal ulcer were tested. The time of gastric emptying, antral myoelectric activity, antral pressure were measured 4-6 months after operation in 18 dogs which were randomized into the control, HSV and HSVMA groups. The gastric evacuation, frequency and amplitude of electrogastrography (EGG) were observed 4-6 months after operation in 48 patients who were randomly divided into HSV (20 patients) and HSVMA (28 patients) group, their preoperative values were considered as control group. ResultsIt was found by barium meal examinations that the shape of the stomach and duodenum was normal and gastric peristalsis was clearly visible in all the animals and patients of two postoperative groups. The beginning and ending time of gastric emptying were (5.0±0.06) min and (4.0±0.4) h respectively in the dogs of HSVMA group, which were similar to those of the dogs of control and HSV group (Pgt;0.05). The frequency of the antral myoelectric action potential was (3.11±0.65 ) cycles/min in the dogs of HSVMA group, the frequency of EGG was (3.25±0.75) cycles/min in the patients of HSVMA group, which were significantly lower than that of the dogs or patients of control and HSV group (P<0.05). Injection of pentagastrin in dogs or taking meal in patients significantly increased the antral pressure or amplitude of EGG. Conclusion The motor function of the reconstructed antrum in the HSVMA group approaches the normal level despite a lower antral myoelectric frequency. It is believed that HSVMA will be a choice for the treatment of duodenal ulcer.

  • A REVIEW ON THE TREATMENT OF SPASTIC CEREBRAL PALSY WITH SELECTIVE POSTERIOR RHIZOTOMY

    OBJECTIVE To inquire the indications, contraindications, and operative methods of the treatment of spastic cerebral palsy with selective posterior rhizotomy (SPR), and to improve the therapeutic results and decrease the incidence rate of complications. METHODS The documents about SPR were extensively consulted, and the mechanisms, indications, contraindications, operative methods, muscular tension changes and complications after operation were reviewed. RESULTS With the methods of SPR, I alpha fibers of afferent nerve were selectively amputated, reflex circle of spinal cord was locked, and the muscular tension was decreased, so myospasm was removed. The results after operation and incidence rate of complications were closely related to the indications. The therapeutic results were better when the percent of spinal nerve rhizotomy was less than 50%. CONCLUSION It is a good method for the treatment of spastic cerebral palsy.

  • Anesthetic Procedure for Left Thoracic Sympathectomy under Thoracoscope in Long QT Syndrome Patients

    ObjectiveTo discuss the anesthetic procedure for left thoracic sympathectomy under thoracoscope for long QT syndrome patients. MethodsWe selected 8 patients with long QT syndrome classified American Society of Anesthesiologists Ⅱ-Ⅲ who were going to undergo left thoracic sympathectomy under thoracoscope between July 2011 and October 2014 as our study subjects. They were given a moderate amount of beta blockers before operation, inducted with 0.1 mg/kg midazolam, 3-6 μg/kg fentanyl, 2-4 mg/kg propofol, 0.3-0.6 mg/kg cis-atracurium, and maintained with propofol 1-4 mg/(kg·h) combined with 0.025-2.000 μg/(kg·min) fentanyl. We recorded the mean arterial pressure (MAP), heart rate (HR), pulse oxygen saturation (SpO2) and airway peak pressure, and end-tidal carbon dioxide before anesthesia induction (T0), at endotracheal intubation (T1), during artificial lung-collapse when surgery initiated (T2), 5 minutes after surgery initiation (T3), 15 minutes after surgery initiation (T4), during artificial lung-collapse at the end of surgery (T5) and during extubation (T6). ResultsWhen compared with T0, T2 got a higher MAP, T3 and T4 had a slower HR (P<0.05), but all were within a normal range. All the patients showed little change in airway peak pressure and end-tidal carbon dioxide during the surgery with no statistically significant difference (P>0.05). ConclusionProper anesthetic procedure for left thoracic sympathectomy under thoracoscope for long QT syndrome patients can reduce the incidence of perioperative malignant arrhythmia.

  • TREATMENT OF ADULT CONGENITAL MUSCULAR TORTICOLLIS BY MULTIPLE STERNOCLEIDOMASTOIDHEAD AMPUTATION

    【Abstract】 Objective To investigate the therapeutic method and effectiveness of multi ple sternocleidomastoid headamputation for adult congenital muscular torticoll is. Methods Between March 2009 and February 2011, 19 patients withcongenital muscular torticoll is were treated with multi ple sternocleidomastoid head amputation. There were 13 males and 6 females, aged 16-32 years (mean, 23.5 years). The X-ray films showed that 12 cases were accompanied with some extent cervical lateral bending and wedge change. Ten patients were with i psilateral facial bradygenesis. Four patients had recieved single sternocleidomastoid head amputation. All of the 19 patients were treated with multi ple sternocleidomastoid head amputation, then plaster support and neck collar were used after operation for 3-6 months. Results The wounds of all the 19 patients healed primarily, without infection or hematoma. Sixteen patients were followed up 5 months to 2 years (mean, 8 months). The head and neck malformations were amel iorated significantly. The effectiveness was assessed 2 weeks later, in 7 patients without cervical vertebral malformation results were excellent; in 12 patients with cervical vertebral malformation, the results were excellent in 1 case, good in 7 cases, and fair in 4 cases. The length between mastoid process and sternoclavicular joints was elongated (1.88 ± 0.30) cm significantly after operation in patients without cervical vertebral malformation (t=6.24, P=0.00), showing no significant difference when compared with normal value (t=1.87, P=0.11); the length was elongated (3.38 ± 0.30) cm significantly (t=11.37, P=0.00) after operation in patients with cervical vertebral malformation, but it was significant shorter than normal value (t=12.19, P=0.00). Conclusion Multi ple sternocleidomastoid head amputation is a safe and effective method for adult congenital muscular torticoll is, which can improve the neck rotation function.

  • Laparoscopic Highly Selective Vagotomy (Hill) for Treating Acute Perforating Duodenal Ulcer

    Objective To explore the effect of laparoscopic highly selective vagotomy (Hill) on the treatment for acute perforating duodenal ulcer. Methods In 19 patients with acute perforating duodenal ulcers, laparoscopic repair of the perforation, laparoscopic freeing of the vagus, cutting off of posterior vagal trunk, and highly selective resection of anterior vagal trunk were performed. Results In all 19 cases the operation was successful. No patient was converted into open highly selective vagotomy. Ulcer symptoms of 17 patients disappeared after operation, and gastroscopy in follow up after 6 months showed that the ulcers had healed. The postoperative ulcer symptoms of 2 patients were markedly relieved and were easily controlled by medication. Conclusion The treatment of acute perforating duodenal ulcer by laparoscopic highly selective vagotomy (Hill) has the advantages of minor trauma, rapid postoperative recovery, and good results, it is a good procedure for the treatment of perforating duodenal ulcer.
