  • Efficacy of Glucosamine Hydrochloride in the Treatment of Lumbar Facet Joint Osteoarthritis

    ObjectiveTo analyze the clinical efficacy of glucosamine hydrochloride in the treatment of lumbar facet joint osteoarthritis, in order to provide the most appropriate treatment for lumbar facet joint osteoarthritis. MethodsA total of 120 patients with lumbar facet joint osteoarthritis and low back pain treated between August 2014 and August 2015 were randomly divided into three groups with 40 in each. Group A was treated with glucosamine hydrochloride; group B accepted loxoprofen-sodium; and group C was given glucosamine hydrochloride plus loxoprofen-sodium. The courses of treatment were all 8 weeks in the three groups. Follow-up lasted for 16 weeks. Oswestry disability index (ODI) and visual analogue score (VAS) of the patients were compared before treatment, 8 weeks after treatment, and 8 weeks after withdrawal. ResultsThree patients in group B gave up treatment due to upper gastrointestinal moderate pain after taking the drug. Another 12 patients in group B suffered from upper gastrointestinal mild discomfort, and the symptoms alleviated after accepting symptomatic treatment. There were no drug-related adverse reactions in group A and C. A total of 117 patients completed the 8-week treatment and were all followed up. Before treatment, the ODI scores and VAS scores were not significantly different among the groups (P>0.05). After treatment, the scores changed significantly in all the groups (P<0.05). At week 8 after treatment, the clinical efficacy in group B and C was superior to that in group A, and the differences were statistically significant (P<0.05), but there was no significant difference between group B and C (P>0.05). Eight weeks after withdrawal, the clinical efficacy in group A and C was better than that in group B, and the differences were statistically significant (P<0.05), but there was no significant difference between group A and C (P>0.05). ConclusionGlucosamine hydrochloride is effective for low back pain caused by lumbar facet joint osteoarthritis, which has no non-steroidal anti-inflammatory drug-related complications. It is worthy of clinical application.

  • Risk factors for pulmonary infection in patients after cardiac surgery: A systematic review and meta-analysis

    ObjectiveTo systematically evaluate the risk factors for pulmonary infection after cardiac surgery. MethodsA computer search was performed to collect researches on risk factors for pulmonary infection in patients after cardiac surgery from the databases, including CNKI, Wanfang, VIP, CBM, PubMed, The Cochrane Library, EBSCO, CINAHL, Web of Science, EMbase from the inception to August 2023. Two researchers independently extracted data and assessed the literature according to the inclusion and exclusion criteria, and the quality of the literature was evaluated using the Newcastle-Ottawa Scale (NOS). The meta-analysis was performed using RevMan 5.4 software. ResultsA total of 23 studies covering 24348 patients were selected, including 21 case-control studies and 2 cohort studies. The NOS scores were≥6 points. The results of meta-analysis showed that age (OR=2.16, 95%CI 1.80 to 2.59, P<0.001), smoking history (OR=1.91, 95%CI 1.67 to 2.18, P<0.001), pulmonary disease (OR=1.61, 95%CI 1.40 to 1.85, P<0.001), diabetes mellitus (OR=1.62, 95%CI 1.26 to 2.08, P<0.001), operation time (OR=2.54, 95%CI 1.86 to 3.46, P<0.001), cardiopulmonary bypass (CPB) (OR=3.78, 95%CI 2.11 to 6.77, P<0.001), CPB time (OR=2.30, 95%CI 1.94 to 2.71, P<0.001), blood transfusion (OR=2.55, 95%CI 2.04 to 3.20, P<0.001), postoperative mechanical ventilation time (OR=2.78, 95%CI 2.34 to 3.30, P<0.001), tracheal intubation time (OR=3.93, 95%CI 2.45 to 6.31, P<0.001) and repeated tracheal intubation (OR=8.74, 95%CI 4.17 to 18.30, P<0.001) were independent risk factors for pulmonary infection in patients after cardiac surgery. ConclusionAge, smoking history, pulmonary disease, diabetes mellitus, operation time, CPB, CPB time, blood transfusion, postoperative mechanical ventilation time, tracheal intubation time, and repeated tracheal intubation are risk factors for pulmonary infection in patients after cardiac surgery. It can be used as a reference to strengthen perioperative evaluation and nursing of high-risk patients and reduce the incidence of pulmonary infection.

  • Comparison of screw implantation parameters between two approaches in capitolunate arthrodesis: an imaging analysis based on CT of the normal wrist

    ObjectiveTo compare the parameters of screw implantation in capitolunate arthrodesis between the 2nd and 3rd metacarpal bones and via the distal dorsal capitate bone approach based on CT images of the normal wrist, and provide reference for the selection of surgical approaches and planning of screw insertion trajectory. MethodsCT data of 50 patients who met the criteria between February 2022 and April 2022 were selected as the study objects. There were 30 males and 20 females, and the age ranged from 18 to 69 years (mean, 37 years). The normal wrist alignment was conformed in all CT images. All CT data from the unilateral wrist of the patients was imported into Mimics Medical 20.0 software to construct a three-dimensional plane model, in which a virtual 3.5 mm screw was implanted between the 2nd and 3rd metacarpal bones and via the distal dorsal capitate bone approach. The angle between screw and coronal, sagittal, and transverse planes, the total length of the screw, the length of the screw in the lunate bone and the capitate bone were measured. The ratios of the length of the screw in capitate bone to the length of the long axis of the capitate bone, the length of the screw in lunate bone to the length of the long axis of the lunate bone, and the length of screw in lunate bone to the length of the screw in capitate bone were calculated. Assuming that the result of screw implantation between the 2nd and 3rd metacarpal bones was better than that via the distal dorsal capitate bone approach, the difference in length of the screw in the lunate bone between the two approaches was calculated, and the superiority test was performed. ResultsCompared with the distal dorsal capitate bone approach, the total length of the screw increased, the length of the screw in the capitate bone decreased, and the length of the screw in the lunate bone increased, the angle between the screw and coronal plane decreased, the angles between screw and sagittal, transverse planes increased after the screw was implanted between the 2nd and 3rd metacarpal bones. And the ratio of the length of the screw in capitate bone to the length of the long axis of capitate bone decreased, the ratio of the length of the screw in lunate bone to the length of the long axis of lunate bone increased, and the ratio of the length of the screw in lunate bone to the length of the screw in capitate bone also increased. All the differences were significant (P<0.05). The difference in the length of the screw in the lunate bone between the two approaches was 1.86 mm [95%CI (1.54, 2.18) mm], which was greater than the superiority margin (1.35 mm). The superiority hypothesis was supported. ConclusionCompared with the distal dorsal capitate bone approach, the screws implanted between the the 2nd and 3rd metacarpal bones in the capitolunate arthrodesis are longer in lunate bone and more vertical to the articular surface of the capitolunate joint. Theoretically, the capitolunate joint are more firmly fixed.

  • Analysis of risk factors for pulmonary infection after heart valve replacement and construction of nomogram prediction model

    Objective To develop and validate a nomogram prediction model for pulmonary infection in patients following cardiac valve replacement surgery, providing a reference for early screening of high-risk populations and implementing targeted preventive measures. Methods Clinical data of patients who underwent cardiac valve replacement surgery at the Second Affiliated Hospital of Anhui Medical University from January 2020 to October 2023 were collected. Patients were randomly assigned to a modeling group and a validation group in a 7 : 3 ratio. Based on the occurrence of pulmonary infection post-surgery, patients were divided into a pulmonary infection group and a non-pulmonary infection group. Risk factors for pulmonary infection after cardiac valve replacement were analyzed using least absolute shrinkage and selection operator (LASSO) regression and logistic regression to establish a risk prediction model, which was subsequently validated. Model evaluation was conducted using the area under the receiver operating characteristic curve (AUC), calibration curves, and decision curve analysis. Results A total of 689 patients were included, comprising 354 males and 335 females, with a median age of 58.0 (50.0, 68.0) years. The incidence of pulmonary infection was 16.0% (110/689). Independent risk factors for pulmonary infection following cardiac valve replacement included emergency admission, smoking history, chronic obstructive pulmonary disease, duration of cardiopulmonary bypass, duration of tracheal intubation, and postoperative renal injury. The AUC for the modeling group was 0.911 [95%CI (0.877, 0.946) ], with a Hosmer-Lemeshow χ2-value of 6.577 (P=0.583) in the modeling group. The AUC value was 0.891 [95%CI (0.840, 0.941) ], with a Hosmer-Lemeshow χ2-value of 5.486 (P=0.705) in the validation group. The model demonstrated good discrimination, calibration, and net benefit. Conclusion The established nomogram prediction model has significant predictive value and can be applied to risk assessment and individualized treatment for pulmonary infection in patients following cardiac valve replacement surgery.

  • A prospective study on treatment of Sanders type Ⅱ and Ⅲ calcaneal fractures with interlocking intramedullary nail fixation system

    Objective To explore the effectiveness of interlocking intramedullary nail fixation system for Sanders type Ⅱ and Ⅲ calcaneal fractures by comparing with open surgery. Methods Forty patients (40 feet) with Sanders type Ⅱ and Ⅲ calcaneal fractures, who were admitted between May 2020 and December 2022 and met the selection criteria, were included in the study. They were randomly allocated into control group and minimally invasive group using a random number table method, with 20 cases in each group. The patients were treated with the interlocking intramedullary nail fixation system in the minimally invasive group and with steel plate internal fixation via a lateral L-shaped incision in the control group. There was no significant difference between the two groups (P>0.05) in terms of gender, age, fracture classification and side, cause of injury, time from injury to admission, and preoperative width, length, height, Böhler angle, and Gissane angle of the calcaneus. The operation time, intraoperative blood loss, incision length, hospital stay, fracture healing time, incidence of complications, as well as pre- and post-operative imaging indicators (Böhler angle, Gissane angle, width, height, and length of the calcaneus) and American Orthopaedic Foot and Ankle Society (AOFAS) score of foot were recorded and compared between the two groups. Results The incision length, operation time, and hospital stay of the minimally invasive group were significantly shorter than those of the control group, and the intraoperative blood loss significantly reduced (P<0.05). All patients of the two groups were followed up, with the follow-up time of 8-12 months (mean, 10.2 months) in the minimally invasive group and 8-12 months (mean, 10.4 months) in the control group. No complication occurred in the minimally invasive group after operation. One case of incision epidermal necrosis and 1 case of traumatic arthritis occurred in the control group after operation. However, there was no significant difference in the incidence of complications between the two groups (P>0.05). At last follow-up, the AOFAS score was significantly higher in the minimally invasive group than in the control group (P<0.05). Imaging examination showed that the calcaneal fractures of the two groups healed, and there was no significant difference in healing time between the two groups (P>0.05). Compared with preoperative conditions, the Böhler angle and Gissane angle of the calcaneus in the two groups significantly increased, the width narrowed, and the height and length increased at 3 days after operation and the last follow-up, with significant differences (P<0.05). There was no significant difference between 3 days after operation and last follow-up (P>0.05). There was no significant difference between the two groups at each time point (P>0.05). ConclusionThe interlocking intramedullary nail fixation system in treatment of Sanders type Ⅱ and Ⅲ calcaneal fractures has the advantages of minimal trauma, shortened hospital stay, reliable fracture reduction and fixation, and satisfactory foot function recovery.

  • The impact of prognostic nutritional index on short-term prognosis in critically chronic obstructive pulmonary disease patients

    Objective To investigate the predictive value of the prognostic nutritional index (PNI) for 28-day all-cause mortality in patients with chronic obstructive pulmonary disease (COPD) in intensive care unit (ICU). Methods The relationship between PNI and short-term mortality in COPD patients was analysed using COX proportional hazards and restricted cubic spline (RCS) models. Receiver operating characteristic (ROC) curves were plotted and area under the ROC curve (AUC) was calculated to assess the predictive performance of PNI. The optimal cut-off value for PNI was determined using the Youden index, and the data were divided into a low PNI group and a high PNI group. Kaplan-Meier curves were then constructed and the log-rank test was used to assess differences in survival between the two groups. Results A total of 980 COPD patients were included in the study. Multivariable COX regression analysis showed that PNI was an independent factor influencing short-term mortality in the severe COPD patients (HR=0.972, 95%CI 0.948 - 0.995, P=0.019). RCS curve results showed a non-linear relationship between PNI and short-term mortality in the severe COPD patients (P for non-linear=0.032), with the risk of death gradually decreasing as PNI increased. The ROC curve indicated that PNI had some predictive power, comparable to that of SOFA score [(AUCPNI=0.693) vs. (AUCSOFA=0.672)]. Kaplan-Meier curve analysis showed a significant difference in survival time between the low (≤38.3) PNI group and the high (>38.3) PNI group (P<0.05). Conclusions PNI has a certain predictive role for short-term all-cause mortality in patients with severe COPD. Patients with low PNI at ICU admission have a higher risk of short-term mortality.

  • 急性人工瓣膜功能障碍六例临床分析

    目的 探讨急性人工瓣膜功能障碍的早期诊断及合理治疗措施。 方法 回顾性分析2011年1月至2012年1月武汉亚洲心脏病医院6例急性人工瓣膜功能障碍患者的临床资料,其中男3例,女3例;年龄28~61岁。风湿性心脏瓣膜病5例,退行性心脏瓣膜病1例;行主动脉瓣置换术1例,二尖瓣置换术3例,二尖瓣、主动脉瓣双瓣膜置换术2例。心瓣膜置换术后2个月~8年8个月出现急性人工瓣膜功能障碍,6例患者入院后均在24 h内因心力衰竭表现难以纠正而行气管内插管+有创呼吸机治疗;4例患者于入院后24 h内行急诊外科手术治疗,2例患者放弃手术治疗。 结果 人工瓣膜的主要病理改变为血栓形成、瓣膜赘生物形成、组织水肿、内膜增生和瓣叶活动度降低。经手术治疗的4例患者无围术期死亡,均痊愈出院;未行手术治疗的2例患者均于48 h内死于心力衰竭 。随访4例,随访时间12~23个月,随访期间4例患者均酌情服用利尿剂、血管紧张素转换酶抑制剂、华法林等药物,术后3个月、6个月、1年复查心脏超声心动图提示人工瓣膜功能良好,术后1年患者心功能均恢复至Ⅰ~Ⅱ级。 结论 急诊床旁经胸超声心动图检查对急性人工瓣膜功能障碍的早期诊断有重要的价值;尽早行外科手术治疗是挽救该类患者生命最有效的手段。
