  • 阻塞性睡眠呼吸暂停低通气综合征合并高血压患者血浆同型半胱氨酸的变化

    目的 探讨同型半胱氨酸( HCY) 在OSAHS 合并高血压发病中的作用。方法 检测22例OSAHS 合并高血压患者、20 例OSAHS不伴高血压患者血浆HCY 水平, 并与对照组比较, 将检测结果与睡眠呼吸监测( PSG) 指标进行相关性分析。PSG 监测指标包括呼吸暂停低通气指数( AHI) 、血氧饱和度小于90% 占总睡眠时间百分比( SLT90% ) 、最低血氧饱和度( LSaO2 ) 、平均血氧饱和度( MSaO2 ) 、觉醒指数( AI) 、睡眠呼吸暂停最长时间( LAT) 。结果 OSAHS 不伴高血压患者血浆HCY明显高于对照组[ ( 18. 28 ±6. 80) μmol /L 比( 6. 32 ±3. 95) μmol /L, P lt;0. 01] , OSAHS 合并高血压组与OSAHS 不伴高血压组比较血浆HCY 进一步升高[ ( 23. 83 ±5. 90) μmol /L 比( 18. 28 ±6. 80) μmol /L, P lt;0. 01] 。血浆HCY 水平与OSAHS 不伴高血压组及OSAHS 合并高血压组的AHI、SLT90% 均呈正相关( P 均lt;0.01) , 与LSaO2、MSaO2 均呈负相关( P 均lt;0. 01) 。结论 HCY 参与了OSAHS 的病理生理过程, 在OSAHS 合并高血压患者发病中起一定作用。

  • Research on the Roles of Homocysteine Transsulfuration Pathway, Vitamin B6 and Endogenous Hydrogen Sulfide in Treating Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease

    目的 研究同型半胱氨酸转硫途径、维生素B6及内源性硫化氢在慢性阻塞性肺疾病急性加重期(AECOPD)中的作用。 方法 2010年2月-4月间筛选AECOPD患者16例和健康志愿者(对照组)13例,测定AECOPD患者加重期、缓解期及对照组的肺功能、血清硫化氢(H2S)、丙二醛(MDA)、叶酸、维生素B12、C反应蛋白、白介素6、血浆同型半胱氨酸、胱硫醚、半胱氨酸和维生素B6的浓度。计算半胱氨酸转化率(半胱氨酸浓度/胱硫醚浓度)与胱硫醚转化率(胱硫醚浓度/同型半胱氨酸浓度)参与分析。 结果 ① 加重期血清MDA水平[(7.3 ± 5.1)nmol/L ]比缓解期[(3.0 ± 1.4)nmol/L ]和对照组[(3.0 ± 2.2)nmol/L ]均升高(P<0.01);血清MDA水平与第1秒用力呼气容积/用力肺活量(FEV1/FVC)、第1秒用力呼气容积占预计值百分比(FEV1%预计值)呈负相关。② 加重期血清H2S水平与血浆维生素B6水平较缓解期与对照组降低(P<0.01);缓解期血清H2S水平[(47.2 ±5.1) μmol/L ]高于对照组[(38.8 ± 2.1) μmol/L ],P<0.01;血清H2S水平、血浆维生素B6水平均与FEV1%预计值呈正相关(r=0.651、0.680,P<0.01),均与血清MDA水平呈负相关(r=-0.334、-0.448,P<0.05)。③ 加重期半胱氨酸转化率(3.97 ± 2.41)低于缓解期(5.92 ± 2.18)与对照组(6.14 ± 3.15)差异有统计学意义(P<0.05);而胱硫醚转化率则相反。④ 叶酸与维生素B12水平各组间均无差异。 结论 提高AECOPD患者维生素B6及H2S浓度可能能促使AECOPD患者向稳定状态转归,减轻氧化应激损伤。维生素B6与H2S可能成为AECOPD患者的一个新的治疗点。Objective To study the roles of homocysteine (Hcy) transsulfuration pathway, Vitamin B6 and endogenous hydrogen sulfide in treating patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods Sixteen AECOPD patients and 13 healthy controls (Control group) from February to April 2010 were recruited in this study. Lung function, serum hydrogen sulfide (H2S), malondialdehyde (MDA), folate, vitamin B12, C-reactive protein (CRP), interleukin-6 (IL-6), Hcy, cystathionine, cystein (Cys) and vitamin B6 were all measured for all the patients in the acute exacerbation period and alleviation period and healthy controls. The conversion rate of Cys (expressed as Cys/cystathionine) and the conversion rate of cystathionine (expressed as cystathionine/Hcy) were calculated for analysis. Results Serum MDA level for patients in the acute exacerbation period (AE period) [(7.3 ± 5.1) nmol/L] was significantly higher than that in the alleviation period [(3.0 ± 1.4) nmol/L] and in the healthy controls [(3.0 ± 2.2) nmol/L] (P < 0.01). Serum MDA level was negatively correlated with percentage of FEV1 in predicted FEV1 (FEV1% pred) and FEV1/FVC. Serum H2S level and plasma vitamin B6 level for patients in the AE period were significantly lower than those in the alleviation period and in the healthy controls (P < 0.01), and serum H2S level was significantly higher in the alleviation period [(47.2 ± 5.1) μmol/L] than in the controls [(38.8 ± 2.1) μmol/L] (P < 0.01). Both serum H2S and plasma vitamin B6 levels were correlated positively with FEV1% pred for patients in the AE period and healthy controls (r=0.651, 0.680; P < 0.01), but negatively correlated with serum MDA level (r=-0.334, -0.448; P < 0.05). The conversion rate of Cys for patients in the AE period (3.97 ± 2.41) was significantly lower than that in the alleviation period (5.92 ± 2.18) and the control group (6.14 ± 3.15) (P < 0.05), but the conversion rate of cystathionine was just the opposite (P < 0.05). There were no significant differences in the levels of serum folate and vitamin B12 among the three groups. Conclusion Raising the Vitamin B6 and H2S level may facilitate stabilizing of conditions in patients with AECOPD and reduce oxidative stress. Therefore, it may become a new treatment method for AECOPD.

  • The Concentration of Plasma Homocysteine in Patients with Acute Cerebral Infarction and its Relationship with TOAST Subtypes

    ObjectiveTo explore the concentration of the plasma homocysteine (Hcy) and the relationship with TOAST subtypes in patients with acute cerebral infarction. MethodsA total of 120 patients with acute cerebral infarction (ACI) treated from April 2012 to April 2013 were enrolled into the ACI group.They were classified with Korean TOAST classification as five subtypes:atherothrombosis (AT) type,small artery disease (SAD) type,cardioembolism (CE) type,stroke of other disease (SOD) type,and stroke of undetermined etiology (SUE) type.The plasma Hcy concentrations in each group and in 60 heathy people who were selected into the control group were measured.Furthermore,the relationship between plasma Hcy concentration and their subtypes were analyzed. ResultsThe plasma Hcy level in ACI group was significant higher than that in the control group (P<0.01).The levels of plasma Hcy were much higher in patients with AT,SAD,SOD,and CE than those in the control groups (P<0.01).In different subtypes,AT and SAD subtypes had higher homocysteine concentration than SUD and CE subtypes did.The concentration of Hcy in AT and SAD group had no significant difference. ConclusionACI is related to hyperhomocysteinemia.The plasma Hcy level varies with different TOAST subtypes of ACI,specially elevating in the subtypes of AT and SAD,which may indicate that hyperhomocysteinemia may increase stroke risk through proatherogenic effect and endothelial dysfunction.

  • The relationship between serum homocysteine and post-stroke epolepsy

    Homocysteine is an intermediate product of methionine and cysteine metabolism, and plays a key role in methylation. Epilepsy is one of the common diseases of the nervous system, long-term repeated seizures will not only cause damage to the brain tissue, but also cause cognitive impairment. At present, the clinical treatment for epilepsy is still mainly to control symptoms, the fundamental etiology of epilepsy still needs to be improved, to explore the etiology of seizures, fundamentally control seizures, is still our long-term struggle direction. High homocysteine is associated with many diseases. Epidemiological studies have shown that the serum homocysteine level of 10% ~ 40% of epilepsy patients is higher than that of the normal population. By exploring the relationship between serum Hcy and epilepsy,We expect to provide help for the diagnosis and treatment of clinical epilepsy.

  • Changes of Plasma Homocysteine and Peripheric Arterial Stiffness in Patients with Subclinical Hypothyroidism

    ObjectiveTo observe the changes of plasma homocysteine (Hcy) and brachial ankle pulse wave velocity (baPWV) in patients with subclinical hypothyroidism, and discuss the relationship between subclinical hypothyroidism and arterial stiffness. MethodSeventy-three patients with subclinical hypothyroidism who were not treated before were divided into two groups according to thyroid stimulating hormone (TSH) level between January 2013 and June 2014. There were 35 patients in group A (4 mU/L < TSH < 10 mU/L) and 38 in group B (TSH ≥ 10 mU/L). Another 30 healthy individuals were selected as controls. Hcy and baPWV were determined in all subjects. ResultsCompared with the controls, patients had significantly higher level of TSH, Hcy and baPWV in group A, and had significantly higher TSH, triacylglycerol (TG), low density lipoprotein cholesterol (LDL)-C, Hcy, and baPWV in group B (P<0.05). Compared with group A, TSH, TG, LDL-C, Hcy, and baPWV in group B patiens were significantly higher (P<0.05). Pearson correlation analysis showed that Hcy was positively correlated with TSH (r=0.353, P<0.01) and baPWV was positively correlated with TSH (r=0.416, P<0.01). ConclusionsHcy level and peripheric arterial stiffness increase in patients with subclinical hypothyroidism. Both of them are correlated positively with TSH.

  • 经颅多谱勒对HCY与脑动脉粥样硬化关系的探讨

    摘要:〖HTSS〗对30例有并发症T2DM病患者、20例无发症T2DM病患者及22例健康支援者检测双侧大脑前动脉(ACA)、大脑中动脉(MCA)、大脑后动脉(PCA)和双侧椎动脉(VA)及基底动脉(BA)的平均阻力指数(RI)和搏动指数(PI)。并检测HCY、胆固醇(TC)、甘油三脂(TG)、空腹血糖(FBG)、餐后2小时血糖(PPBG)及糖化血红蛋白(HbA1c),结果显示并发症组TC、TG、FBG、PPBG、HbA1c、HCY及PI、RI明显高于无并发症组和对照组,差异具有统计学意义(Plt;0.05)。且并发症组和单纯T2DM组的TC、TG、FBG、PPBG、HbA1c、HCY及PI、RI均高于对照组,并有统计学意义(Plt;0.05)。反映HCY水平升高与T2DM患者的脑动脉粥样硬化密切相关。

  • The Clinical Significance of Monitoring Homocysteine Levels in Peripheral Blood of Advanced Non-small Cell Lung Cancer Patients during Gemcitabine with Cis-platinum Program of Chemotherapy

    ObjectiveTo observe the alteration of serum homocysteine (Hcy) levels of advanced non-small cell lung cancer (NSCLC) patients during gemcitabine with cis-platinum (GP) program of chemotherapy and to explore the clinical value of monitoring Hcy in evaluating chemotherapy curative effect. MethodsA total of 49 advanced NSCLC patients (including 28 squamous carcinoma and 21 adenocarcinoma) first treated between May 2012 and April 2015 were selected. The Hcy, cytokerantin-19-fragment (CYFRA21-1) and carcinoembryonic antigen (CEA) levels of the morning fasting venous blood were measured before the first and after the second cycle of chemotherapy. Combined the pathological types of NSCLC, statistical analysis was carried out on the test results. ResultsAll of the 49 patients completed two cycles of GP chemotherapy, and the chemotherapy was effective on 31 and ineffective in 18. Before the chemotherapy, the differences in the positive rates of Hcy, CYFRA21-1, and CEA were statistically significant respectively between squamous carcinoma and adenocarcinoma patients (P < 0.05). But when combined the two types, the differences of three indicators's positive rates were not significant (P > 0.05). After two cycles of GP chemotherapy, in the patients with effective chemotherapy, the Hcy, CYFRA21-1 and CEA levels were lower in both squamous carcinoma and adenocarcinoma patients compared with that before the chemotherapy; the difference in the decrease of Hcy levels in both of the two pathological types was significant (P < 0.05), while CEA levels was significant only in adenocarcinoma patients (P < 0.05) and CYFRA21-1 levels was significant only in squamous carcinoma patients (P < 0.05). Among the patients with ineffective chemotherapy, the Hcy, CYFRA21-1 and CEA levels increased compared with those before the chemotherapy; the difference in the increase of Hcy levels were significant in both of the two pathological types (P < 0.05), while CYFRA21-1 levels was significant only in squamous carcinoma patients (P < 0.05) and CEA levels was not significant in both of the two pathological types (P > 0.05). ConclusionThe effect of chemotherapy and the pathogenetic condition can be assessed by monitoring serum Hcy levels of NSCLC patients during the chemotherapy.

  • Correlation between Carotid Atherosclerosis and Homocysteine Levels in Patients with Hypertension: A Cross-sectional Study

    ObjectiveTo study the correlation between neck artery vascular sclerosis and homocysteine levels in hypertensive patients. MethodsA total of 2 132 patients with hypertension participated in the program of "Stroke screening and prevention in Dongying city" were included from Sep. 2012 to Jan. 2013. Questionnaire, physical examination, biochemical blood tests and carotid artery ultrasound were conducted in all included patients. According to the level of homocysteine (Hcy), patients were divided into three groups:716 patients with Hcy level less than 12.3 mm/L were in Group 1, 703 patients with Hcy level 12.3 to 16.5 mm/L were in Group 2, and 713 patients with Hcy level more than 16.5 mm/L were in Group 3. The influence of Hcy levels on carotid atherosclerosis was analyzed. Results① The prevalence rate for carotid plaques in Groups 1, 2, and 3 were 50.28%, 60.03% and 65.36%, respectively. The prevalence rate for carotid plaques in Group 2 was higher than that in Group 1 with a statistical difference (OR=1.485, 95% CI 1.203 to 1.833, P=0.000). The prevalence rate in Group 3 was also higher than that in Group 1 with a statistical difference (OR=1.866, 95% CI 1.508 to 2.308, P=0.000). ② The prevalence rate for carotid plaques was 62.24% in the rural population, and 52.39% in the urban population. The difference between urban and rural populations was statistically significant (OR=1.500, 95% CI 1.259 to 1.788, P=0.000). ③ Using the presence of carotid plaques as a dependent variable and Hcy level as a covariant, logistic regression analysis found that the plaque formation in Group 2 was 1.491 times than in Group 1 and the plaque formation in Group 3 was 1.752 times than in Group 1. After adjusting the risk factors (gender, age, BMI, TG, TC, LDL-C, HDL-C and blood sugar level), the results showed that the plaque formation in Group 2 was 1.388 times than in Group 1, and the plaque formation in Group 3 was 1.452 times than in Group 1. ConclusionThere is a correlation between the increased incidence of carotid plaque and homocysteine levels in patients with hypertension. The incidence of carotid plaque in the rural population with hypertension is higher than that in the urban population. In the population with hypertension, high homocysteine level is an independent risk factor for the formation of carotid plaques.

  • Analysis of the Correlation between Insulin Resistance and Plasma Homocysteine Level in Patients with Cerebral Infarction

    ObjectiveTo analyze insulin resistance in patients with cerebral infarction and its correlation with plasma homocysteine level. MethodsA total of 100 cerebral infarction patients diagnosed between July 2013 and August 2014 and 100 healthy physical examination subjects were included in our research. The plasma homocysteine level was detected; the levels of fasting plasma glucose and fasting insulin were detected at the same time. ResultsPlasma homocysteine level (34.95±14.55) μmol/L in patients with cerebral infarction was significantly higher than that of the control group (8.84±2.27) μmol/L (P<0.05). Insulin resistance index (6.24±3.15) in patients with cerebral infarction was significantly higher than that of the control group (2.19±0.63) (P<0.05). The plasma homocysteine level in the infarction group with insulin resistance was significantly higher than that in the infarction group without insulin resistance (P<0.05). The plasma homocysteine level in the infarction group without insulin resistance was significantly higher than that of the patients with insulin resistance in the control group (P<0.05). In the cerebral infarction group, insulin resistance index was positively correlated with plasma homocysteine level (r=0.600, P<0.01). ConclusionInsulin resistance and plasma homocysteine level play important roles in clinical cerebral infarction. Improving insulin resistance and decreasing plasma homocysteine level may be effective ways to reduce the incidence of cerebral infarction.
