【Abstract】 Objective To explore the prevention and treatment of leg length discrepancy after total hip arthroplasty(THA). Methods There were 87 patients who were treated by THA from January 2004 to December 2006, including 36males and 51 females, with the average age of 60.2 years (ranging from 35 years to 78 years). Among these cases, there were 35 of avascular necrosis of the femoral head, 38 of subcapital femoral neck fracture, 4 of femoral neck tumor, 6 of rheumatoid arthritis and 4 of acetabular dysplasia. In 70 cases, the patients had leg length discrepancy, and the legs shortened from 1 cm to 6 cm. Based on the cl inical measurement and radiographic examination, the surgical protocols were designed, the type of the hip prosthesis was chosen, and the neck length of the femoral prosthesis and the position of osteotomy were estimated. By the proper wearing of the acetabula, the best rotation point was found out. The cut plane of the femoral neck was adjusted according to the results of the radiographic and other examinations. The neck length was readjusted after the insertion of the prosthesis so as to achieve intended leg-length equal ization. The discrepancy of the leg length was measured and evaluated after operation. Results Superficial infestation happened in 2 cases 5 days after the operation and was cured by mero-drainage. Luxation happened in 4 cases 4 weeks after the operation, in which 2 cases were cured by operation while the other 2 were cured by manual reduction. All the patients were followed up for 6 months to 36 months, with the average time of 18.3 months. The Harris scores were 34.81 ± 1.36 preoperatively and 91.50 ± 1.87 postoperatively (P lt; 0.05). In the 17 patients with equal legs before the operation, 1 was lengthened 1.5 cm in the leg, while in the 70 patients with shortened legs before the operation, 66 returned to the same length in their legs, and 4 were lengthened or shortened from 1.6 cm to 2.1 cm. The total rate of equal leg length was 94.25%. Conclusion The preoperative measurement, radiographic templating and intraoperative correction, together with postoperative orthopraxy, are effective in prevention and treatment of leg length discrepancy after THA.
Skin aging principles play important roles in skin disease diagnosis, the evaluation of skin cosmetic effect, forensic identification and age identification in sports competition, etc. This paper proposes a new method to evaluate the skin aging objectively and quantitatively by skin texture area. Firstly, the enlarged skin image was acquired. Then, the skin texture image was segmented by using the iterative threshold method, and the skin ridge image was extracted according to the watershed algorithm. Finally, the skin ridge areas of the skin texture were extracted. The experiment data showed that the average areas of skin ridges, of both men and women, had a good correlation with age (the correlation coefficient r of male was 0.938, and the correlation coefficient r of female was 0.922), and skin texture area and age regression curve showed that the skin texture area increased with age. Therefore, it is effective to evaluate skin aging objectively by the new method presented in this paper.
ObjectiveTo summarize the application of image texture analysis in the diagnosis and treatment of gastric cancer.MethodsReviewed the literatures on the application of image texture analysis related methods in the diagnosis and treatment of gastric cancer, and summarized the value of texture analysis in the diagnosis and treatment of gastric cancer in terms of diagnosis, staging, curative effect evaluation, and prognosis prediction.ResultsImage texture analysis had been widely used in diagnosis, staging, curative effect evaluation, prognosis prediction of gastric cancer, and other related diagnosis and treatment applications.ConclusionsImage texture analysis is an important part of the diagnosis and treatment of gastric cancer, which has a good development prospect.
【Abstract】ObjectiveTo investigate the perioperative management in hepatectomy using hepatic energy metabolisom for enhancing safety of and improving the survival in patients with primary liver cancer (PLC).MethodsTwo thousands and one hundred fortythree patients with PLC were treated in this hospital from January 1990 to January 2004. The perioperative data, operative approach, postoperative treatment, postoperative clinical course and follow up data were retrospectively analyzed. All patients were divided into two groups: the early period group and the late period group(from January 1997 to January 2004) and comparison was taken between two groups. The preoperative redox tolerance index (RTI), intraoperative hepatopetal blood occlusion of half liver, and postoperative arterial ketone body ratio (AKBR) were investigated and evaluated.Results①The proportion of small PLC and resection rate increased, the morbidity of complications and mortality after hepatectomy decreased, also the survival rate prolonged in the late period group. ②When using RTI as an indicator for selection of hepatectomy, the morbidity of complications decreased from 21.1% to 11.0%, the mortality form 1.6% to 0.3%. ③Comparising hepatopetal blood occlusion of total liver (n=476) with half liver (n=523),the postoperative morbidity of complications and mortality were 25.8% to 11.9% and 2.3% to 0.6% respectively. ④Postoperative AKBR measurements was a reliable indicator to assess the energy status of the liver and liver failure.ConclusionRTI is of potential value in predicting preoperative hepatic functional reserve, hepatopetal blood occlusion of half liver could protect the residual liver function, and postoperative AKBR measuremeant is a simple and accurate means of determining the immediate state of metabolic dysfunctioning in liver resection. The authors propose that perioperative treatment is an important factor in decreasing operative complications and mortality rate after liver resection.
Objective To lay a foundation for study of optic narve damage in glaucoma by measuring the number and diameter of the optic nerve fibers and optic disc area in normal individuals. Methods The cross-sections of the optic nerve and the optic discs in 15 normal human eyes were examined with the use of a computerized image analysis system. Results The mean nerve fiber count was 10.08times;105plusmn;1.61times;105. The mean nerve fiber diameter was (0.99plusmn;0.04)mu;m. The nerve fiber count increased significantly with the increasing of cross-section area of the optic nerve, but the nerve fiber count was independent of the optic dise area. Conclusion This study provided anatomic basis for predicting the prognosis of optic nerve damage and further studyv of nerve damage in glaucoma. (Chin J Ocul Fundus Dis,1999,15:16-19)
In order to solve the saturation distortion phenomenon of R component in fingertip video image, this paper proposes an iterative threshold segmentation algorithm, which adaptively generates the region to be detected for the R component, and extracts the human pulse signal by calculating the gray mean value of the region to be detected. The original pulse signal has baseline drift and high frequency noise. Combining with the characteristics of pulse signal, a zero phase digital filter is designed to filter out noise interference. Fingertip video images are collected on different smartphones, and the region to be detected is extracted by the algorithm proposed in this paper. Considering that the fingertip’s pressure will be different during each measurement, this paper makes a comparative analysis of pulse signals extracted under different pressures. In order to verify the accuracy of the algorithm proposed in this paper in heart rate detection, a comparative experiment of heart rate detection was conducted. The results show that the algorithm proposed in this paper can accurately extract human heart rate information and has certain portability, which provides certain theoretical help for further development of physiological monitoring application on smartphone platform.
【Abstract】 Objective To study the effects of ischemic preconditioning (IP) on the activity of nuclear factor-κB (NF-κB) and the expressions of TNF-α and intercellular adhesion molecule-1 (ICAM-1) during early reperfusion following liver transplantation in rats. Methods The models of rat orthotopic liver transplantation were established. The donor livers were stored for 2 hours in Ringers solution at 4 ℃ before transplantation. All rats were randomly divided into sham operation group (SO group), control group and IP group. IP group was achieved by clamping the portal vein and hepatic artery of donor liver for 10 minutes followed by reperfusion for 10 minutes before harvesting. The activity of NF-κB and expressions of TNF-α and ICAM-1 at 1 h, 2 h, 4 h and 6 h after reperfusion were measured. Serum ALT, LDH were also determined. Results The liver function of recipients with IP were significantly improved. Compared with SO group, the graft NF-κB activity increased after transplantation in control group and IP group (P<0.05), while compared with control group that was significantly attenuated at 1 h and 2 h in IP group. Similarly, hepatic levels of TNF-α and ICAM-1 were significantly elevated in control group and were reduced in IP group. Conclusion IP might down-regulated TNF-α and ICAM-1 expression in the grafts after orthotopic liver transplantation through depressed NF-κB activation, and attenuate neutrophil infiltration in the grafts after reperfusion.
