  • Treatment choices of chronic thoracoabdominal aortic dissection aneurysm

    The treatment of chronic thoracoabdominal aortic dissection aneurysm remains a major challenge in aortic surgery. Open surgery is the mainstream treatment at present. New devices for endovascular treatment of chronic thoracoabdominal aortic dissection are gradually applied in clinical practice. The hybrid procedure is a combination of open and endovascular procedures. The appropriate treatment should be selected according to the patient's age, anatomy, genetic aortic disease, and comorbidities.

  • Treatment of type B aortic dissection without an optimal "landing zone": A case control study

    Objective To compare the short and mid-term outcomes of open surgery and hybrid technique for the treatment of complex type B aortic dissection (AD). Methods A total of 45 patients (37 acute AD and 8 chronic AD) with complex type B AD were admitted to Nanjing First Hospital from January 2012 to June 2016, including 37 males and 8 females. All patients were confirmed by computed tomography angiography (CTA), and ultrasonic cardiogram (UCG) to rule out valvular diseases, aortic root and ascending aorta lesion, and pericardial effusion. According to different treatments, patients were divided into two groups: the open surgery group (OS group) with a total of 25 patients (20 males, 5 females, a mean age of 50.16±10.87 years); the hybrid technique group (HT group) with a total 20 patients (18 males, 2 females, mean age of 51.31±8.11 years). The short and mid-term outcomes of open surgery and hybrid technique for the treatment of complex type B AD were compared. Results All the patients were discharged successfully. There was no death, cognitive impairment, cerebral infarction, hemiplegia, paraplegia, coma and other neurological complications in both groups. In the OS group, one patient suffered acute kidney injury and received renal replacement therapy (RRT), whose renal function was returned to normal prior to discharge; one patient was transferred to ICU again owing to pericardial effusion, respiratory failure and lung infection; one patient underwent debridement surgery because of postoprative sternal dehiscence. In the HT group, one patient with recurrent chest pain five days after endovascular aortic repair, whose CTA showed hematoma of aortic arch and ascending aorta caused by reverse tear, underwent Sun’s procedure immediately. All patients received CTA examination three months after operation in outpatient room. In the OS Group, the tear of AD was closed well by stent-graft and no leakage or shunt was detected in CTA. The rate of thrombosis formation in thoracic aortic false lumen was 100.0%. Meanwhile, in the HT Group, there was one patient with type Ⅱ leakage and the rate of thrombosis formation in thoracic aortic false lumen was 94.7%. Conclusion For complex type B AD without optimal "landing zone" in descending aorta, open surgery is recommended as the first choice for experienced team because of its less costs and perfect results; hybrid technique which can achieve quicker recovery with less surgical trauma still has serious complications such as leakage, reverse tear, and so on.

  • 关节镜与开放手术治疗臀肌挛缩症的临床疗效比较

    目的 比较关节镜和开放手术治疗臀肌挛缩症的可行性及安全性。 方法 2002 年2 月- 2009 年5 月,分别采用关节镜松解(关节镜组,27 例)及传统开放手术(开放手术组,25 例)治疗双侧臀肌挛缩症患者52 例。关节镜组:男 21 例,女6 例;年龄11 ~ 24 岁,平均16.3 岁。病程6 ~ 13 年,平均9.1 年。臀肌挛缩程度参照张忠等的分级标准:中度1 例,重度26 例。开放手术组:男19 例,女6 例;年龄12 ~ 26 岁,平均16.9 岁。病程7 ~ 12 年,平均8.5 年。臀肌挛缩程度:中度1 例,重度24 例。两组患者性别、年龄、病程、分度等一般资料比较,差异均无统计学意义(P gt; 0.05),具有可比性。 结果 两组手术时间比较差异无统计学意义(P gt; 0.05),但切口长度及术中出血差异有统计学意义(P lt; 0.05)。关节镜组术后切口血肿发生率及感染发生率均低于开放手术组,差异有统计学意义(P lt; 0.05),但感觉缺失发生率和坐骨神经损伤发生率两组差异均无统计学意义(P gt; 0.05)。两组患者均获随访,随访时间12 ~ 36 个月,平均23.7 个月。术后12 个月,根据蔡斌等疗效评价标准,两组患者步态、交腿试验、并膝下蹲及弹跳感指标比较,差异均无统计学意义(P gt; 0.05)。 结论 与开放手术相比,关节镜治疗臀肌挛缩症具有创伤小、出血少、术后并发症发生率低等优点。

  • TREATMENT OF PES ANSERINUS BURSITIS WITH DEBRIDEMENT UNDER ARTHROSCOPY

    Objective To explore the cl inical effectiveness of debridement in treatment of Pes anserinus bursitis under arthroscopy by comparing the curative effect of three therapies: local block therapy, open operation and debridement under arthroscopy. Methods From January 2000 to January 2007, 90 cases of unilateral Pes anserinus bursitis were treated with debridement under arthroscopy (group A, n=30), local block therapy (group B, n=30) and open operation (group C, n=30),respectively. The group A included 18 males and 12 females, aged (40.0 ± 2.5) years old; the locations were left knee in 16 cases and right knee in 14 cases; 10 cases had injury history, 7 cases had tired history and 13 cases had no obvious inducement; the course of disease was (24.0 ± 3.2) weeks. The group B included 17 males and 13 females, aged (37.0 ± 2.5) years old; the locations were left knee in 15 cases and right knee in 15 cases; 10 cases had injury history, 8 cases had tired history and 12 cases had no obvious inducement; the course of disease was (26.3 ± 3.5) weeks. The group C included 16 males and 14 females, aged (39.8 ± 2.2) years old; the locations were left knee in 18 cases and right knee in 12 cases; 8 cases had injury history, 10 cases had tired history and 12 cases had no obvious inducement; the course of disease was (25.0 ± 3.9) weeks. There was no statistically significant difference in the general data among three groups (P gt; 0.05). Results All patients were followed up 15 months on average (12-24 months). In group C, the inflammatory reation occurred at wound in 9 cases (30%) at 3-10 days after operation and was cured after symptomatic management; other incision healed by first intention; and showing statistically significant differences when compared with other 2 groups (P lt; 0.05). In group C, joint swell ing occurred at 1 week after operation in 1 case, l imitation of joint motion in 2 cases at 10-12 weeks after operation and was recovered after symptomatic management. In group B, 21 cases (70%) had a recurrence at 6-12 months after operation, all patients of other 2 groups had no recurrence; showing statistically significant differences between group B and groups A, C (P lt; 0.05). At last follow-up, the pain remain rates were 3.3% (group A), 0 (group B) and 33.3% (group C), and the compl ication incidence rates were 3.3%, 26.7% and 70.0%, respectively; all showing statistically significant differences among three groups (P lt; 0.05). At last follow-up, there were statistically significant differences in the visual analogue scale (VAS) score, the HSS score, and range of motion (ROM) between preoperation and postoperation in group A (P lt; 0.05); there was no statistically significant difference in the VAS score, HSS score and ROM between preoperation and postoperation in group B (P gt; 0.05); the ROM of postoperation in group C was smaller than that of preoperation (P lt; 0.05). There were statistically significant differences in the VAS score and HSS score between group A and groups B, C (P lt; 0.05), and in ROM among three groups after operation (P lt; 0.05). Conclusion The treatment of Pes anserinus bursitis with debridement under arthroscopy has advantages of easy-to-do, less compl ication, low relapse rate and good functional rehabil itation.

  • PROGRESS IN TREATMENT OF ACUTE Achilles TENDON RUPTURE

    Objective To review the progress in the treatment of acute Achilles tendon rupture. Methods Recent literature about the treatment of acute Achilles tendon rupture was reviewed and analyzed. Results Treatments of acute Achilles tendon rupture include operative and non-operative treatments. Operative treatments include open surgery and percutaneous minimally invasive surgery. Compared with non-operative treatment, operative treatment can effectively reduce the re-rupture incidence, but it had higher complication incidences of wound infection and nerve injury. Although early functional rehabilitation during non-operative treatment could reduce the re-rupture incidence, there is no consistent orthopaedic device and guideline for functional rehabilitation. Conclusion Both operative and non-operative treatments have advantages and disadvantages for the treatment of acute Achilles tendon rupture. No consistent conclusion is arrived regarding functional recovery. Future studies should explore the strategy of early functional rehabilitation during non-operative treatment and its mechanism of promoting tendon healing.

  • Short-term effectiveness of orthopedic robot-assisted resection for osteoid osteoma

    Objective To investigate short-term effectiveness and clinical application advantages of orthopedic robot-assisted resection for osteoid osteoma compared with traditional open surgery. Methods A retrospective analysis was conducted on clinical data of 48 osteoid osteoma patients who met the selection criteria between July 2022 and April 2023. Among them, 23 patients underwent orthopedic robot-assisted resection (robot-assisted surgery group), and 25 patients received traditional open surgery (traditional surgery group). There was no significant difference (P>0.05) in gender, age, disease duration, lesion location and size, and preoperative visual analogue scale (VAS) score, and musculoskeletal tumor society (MSTS) score between the two groups. The surgical time, intraoperative blood loss, intraoperative lesion localization time, initial localization success rate, infection, and recurrence were recorded and compared. VAS scores before surgery and at 24 hours, 1, 3, 6, and 9 months after surgery and MSTS score before surgery and at 3 months after surgery were assessed. Results All patients completed the surgery successfully, with no significant difference in surgical time between the two groups (P>0.05). Compared to the traditional surgery group, the robot-assisted surgery group had less intraoperative blood loss, shorter lesion localization time, and shorter hospitalization time, with significant differences (P<0.05). The initial localization success rate was higher in the robot-assisted surgery group than in the traditional surgery group, but the difference between the two groups was not significant (P>0.05). All patients in both groups were followed up, with the follow-up time of 3-12 months in the robot-assisted surgery group (median, 6 months) and 3-14 months in the traditional surgery group (median, 6 months). The postoperative MSTS scores of both groups improved significantly when compared to those before surgery (P<0.05), but there was no significant difference in the changes in MSTS scores between the two groups (P>0.05). The postoperative VAS scores of both groups showed a gradually decreasing trend over time (P<0.05), but there was no significant difference between the two groups after surgery (P>0.05). During follow-up, except for 1 case of postoperative infection in the traditional surgery group, there was no infections or recurrences in other cases. There was no significant difference in the incidence of postoperative infection between the two groups (P>0.05). Conclusion Orthopedic robot-assisted osteoid osteoma resection achieves similar short-term effectiveness when compared to traditional open surgery, with shorter lesion localization time.

  • Interpretation of the section of congenital heart diseases in Annual Report on Cardiovascular Health and Diseases in China (2019)

    Congenital heart disease (CHD) is a birth defect with the highest incidence in China. In September 2020, Annual Report on Cardiovascular Health and Diseases in China (2019) was published by National Center for Cardiovascular Diseases. The present situation of prevention and treatment of CHD was briefly summarized in this report. In this paper, we presented the main opinions of Annual Report on Cardiovascular Health and Diseases in China (2019), supplied with relevant data, such as epidemiology, surgical treatment and interventional treatment of CHD, to further expand and explain this report.

  • COMPARISON OF SHORT-TERM EFFECTIVENESS BETWEEN MINIMALLY INVASIVE SURGERY- AND OPEN- TRANSFORAMINAL LUMBAR INTERBODY FUSION FOR SINGLE-LEVEL LUMBAR DEGENERATIVE DISEASE

    Objective To compare the short-term effectiveness of minimally invasive surgery-transforaminal lumbar interbody fusion (MIS-TLIF) versus open-TLIF in treatment of single-level lumbar degenerative disease. Methods Between February 2010 and February 2011, 147 patients with single-level lumbar degenerative diseases underwent open-TLIF in 104 cases (open-TLIF group) and MIS-TLIF in 43 cases (MIS-TLIF group), and the clinical data were analyzed retrospectively. There was no significant difference in gender, age, disease type, lesion level, disease duration, preoperative visual analogue scale (VAS), and preoperative Oswestry disability index (ODI) between 2 groups (P gt; 0.05). The operation time, intraoperative radiological exposure time, intra- and post-operative blood loss, postoperative hospitalization time, and postoperative complications were compared between 2 groups. The VAS score and ODI were observed during follow-up. The imaging examination was done to observe the bone graft fusion and the locations of internal fixator and Cage. Results There was no significant difference in operation time between 2 groups (t=0.402, P=0.688); MIS-TLIF group had a decreased intra- and post-operative blood loss, shortened postoperative hospitalization time, and increased intraoperative radiological exposure time, showing significant differences when compared with open-TLIF group (P lt; 0.05). Cerebrospinal fluid leakage (2 cases) and superficial infection of incision (2 cases) occurred after operation in open-TLIF group, with a complication incidence of 3.8% (4/104); dorsal root ganglion stimulation symptom (3 cases) occurred in MIS-TLIF group, with a complication incidence of 7.0% (3/43); there was no significant difference in the complication incidence between 2 groups (χ2=0.657, P=0.417). The patients were followed up 18-26 months (mean, 21 months) in MIS-TLIF group, and 18-28 months (mean, 23 months) in open-TLIF group. The VAS scores and ODI of 2 groups at each time point after operation were significantly improved when compared with those before operation (P lt; 0.05). There was no significant difference in VAS score between 2 groups at discharge and 3 months after operation (P gt; 0.05); VAS score of MIS-TLIF group was significantly lower than that of open-TLIF group at last follow-up (t= — 2.022, P=0.047). At 3 months and last follow-up, no significant difference was found in the ODI between 2 groups (P gt; 0.05). The imaging examination showed good positions of Cage and internal fixator, and bone graft fusion in 2 groups. Conclusion The short-term effectiveness of MIS-TLIF and open-TLIF for single-level degenerative lumbar diseases was similar. MIS-TLIF has the advantages of less invasion and quick recovery, but the long-term effectiveness needs more observation.

  • In Situ open surgical repair for complex renal artery aneurysm: Outcomes and technical considerations

    ObjectiveTo summarize the diagnosis, surgical management, and outcomes of one case of complex unilateral renal artery aneurysm repaired by in situ open surgery. MethodThe clinical data of a patient with complex renal artery aneurysm admitted to the Department of General Surgery, West China Hospital of Sichuan University in December 2021 who underwent in situ open surgery were retrospectively analyzed. ResultsThe patient was a middle-aged (41 years old) female with a left renal artery aneurysm detected on physical examination. The renal artery three-dimensional CT imaging showed that the aneurysm was large in size and complex in anatomical structure; the aneurysm was located at the renal hilum, demonstrating multiple outflow tracts and close proximity to renal parenchyma and the ureter. Surgical management included in situ aneurysm resection combined with renal artery branch reconstruction and great saphenous vein bypass grafting. The operation duration was 5 h and the intraoperative urine output was 250 mL, and the intraoperative blood loss was about 400 mL. Four units of erythrocyte suspension, 200 mL of autologous recycled blood, and 400 mL of plasma were transfused during the operation. The results of the 36-month postoperative follow-up showed that the reconstructed renal arterial branches and the bridging vessel had satisfactory blood flow, and renal function was unaffected. ConclusionsThe results of this case suggest that in complex renal artery aneurysms involving multiple branches, in situ resection of the aneurysm followed by revascularization and main renal artery bypass grafting to restore flow is safe and feasible, and the long-term prognosis is good. However, it should be emphasized that the anatomy of renal artery aneurysms should be evaluated in detail preoperatively to determine the method of in situ revascularization. The results of the study also need to be further validated by larger samples and multicenter studies.

  • Advances in management of aortic arch in type A aortic dissection

    ObjectiveTo review current research on aortic arch management strategies in type A aortic dissection (TAAD) and provide a reference for surgical decision-making. MethodsA comprehensive literature search was conducted on aortic arch management techniques globally. ResultsCurrent research shows three managment for type A aortic dissection surgical startegies:open surgery remains the gold standard but is associated with elevated perioperative mortality and significant postoperative complication rates;endovascular repair,while demonstrating reduced complication incidence, presents substantial technical challenges that hinder widespread clinical adoption;Hybrid procedures exhibit lower acute complication rates, yet long-term outcomes remain uncertain. ConclusionsAs one of the most lethal emergencies of cardiovascular surgery , TAAD requires immediate intervention. While open surgery is the current gold standard, it presents clinical challenges including elevated mortality and morbidity. Future research must focus on reducing operative duration without compromising success rates.
