  • 优质护理服务对恶性血液病患者焦虑抑郁的影响

    目的 讨论优质护理服务对恶性血液病患者焦虑及抑郁的影响。 方法 2011年10月-2012年1月对58例恶性血液病患者从人文关怀角度施以关心和心理干预,并加大健康教育宣传力度,提供全程化、无缝隙护理。并采用Zung焦虑自评量表(SAS)和抑郁自评量表(SDS)对入、出院时患者焦虑及抑郁情况进行问卷调查。 结果 患者入院时SAS、SDS得分显著高于我国常模值(P<0.01),出院患者焦虑及抑郁得到改善和缓解,与入院相比SAS、SDS得分差异有统计学意义(P<0.01)。 结论 恶性血液病患者存在焦虑抑郁情绪,通过优质护理服务可改善其焦虑抑郁状况。

  • Research progress of Bruton’s tyrosine kinase inhibitors in the treatment of Waldenström macroglobulinemia

    Waldenström macroglobulinemia (WM) is a rare indolent lymphoplasmacytic lymphoma. Bruton’s tyrosine kinase (BTK) plays a key role in the signaling pathway of WM, which has changed the way of treatment of WM. As a first-generation BTK inhibitor, ibrutinib is an early-stage treatment and a salvage treatment that can control toxicity characteristics. However, in order to overcome the resistance of the first-generation BTK inhibitors, reduce the adverse reactions caused by off-target effects, and improve the efficacy, the research and development of new BTK inhibitors has become a hot topic. This article discusses the clinical studies of the first generation and new BTK inhibitors for WM, aiming to provide a certain basis for the more rational application of BTK inhibitors in WM.

  • BILATERAL THREE-TOE TRANSPLANTATION WITH DORSALIS PEDIS FLAP AND FIRST WEB SPACE FLAP FOR DAMAGE INJURY IN THE HANDS

    Objective To discuss the surgical method and effect of repair of damage injury in the hands. Methods Of the 29 cases, 22 were males, 7 were females. Their ages ranged from 15 to 31.The size of defect areas ranged from 18 cm×8 cm to 22 cm×10 cm .Along with dorsalis pedis flap, lateral hemi-pulp flap was takenfrom great toe and first web space flap. The second toe was taken from one footto reconstruct the thumb, second and third toe from another foot was used to reconstruct two fingers. The dorsalis pedis flaps were used to cover palm and dorsum of hand. The lateral hemi-pulp flaps from great toe and first web space flapswere used to reconstruct first web space of hand.Results With the 58 combined flaps, 29 thumbs and 58 fingers were reconstructed. Followups was done for 1 to 8 years.All the thumbsand fingers of 29 hands were reconstructed. Their shape and function were well recovered. Conclusion This new surgical method is effective inpreserving the function of injured hand. The function of the injured hands can be preserved by this surgical method, therefore this method is optimal.

  • Frequency Distributions of Hormone Receptors, Human Epidermal Growth Factor Receptor-2 and Molecular Subtypes in Patients with Breast Cancer

    目的 研究乳腺癌患者雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(HER-2)表达情况及免疫组织化学分子亚型分布。 方法 对2003年1月-2008年9月四川大学华西医院病理科3 365例乳腺癌病理报告存档资料进行激素受体(ER/PR)、HER-2表达情况及免疫组织化学分子亚型分布进行分析。 结果 3 365例确诊的乳腺癌患者中,ER阳性1 824例(54.2%),PR阳性1 841例(54.7%),HER-2过表达284例(8.4%)。相关分析显示ER与PR表达呈正相关(P<0.001),HER-2与ER、PR表达均呈负相关(P<0.001)。免疫组织化学分子亚型结果显示luminal A型最常见,为1 993例(59.2%);basal-like型为623例(18.5%);HER-2过表达型为169例(5.0%);luminal B型最少,为115例(3.4%);未分类的为465例(13.8%)。 结论 乳腺癌患者激素受体及HER-2表达有特殊性,激素受体阳性率>50%,HER-2阳性率在不同研究中显示出不同的结果,尚需进一步研究;ER、PR与HER-2具有良好的相关性;免疫组织化学分子亚型中luminal A型最常见。

  • Interferon regulatory factor 4 involvement in the pathogenesis and development of B cell or plasma cell tumors: an update of research progresses

    Interferon regulatory factor 4 (IRF4) is one of the transcription factors in the interferon regulatory factor family. In the normal physiological process, IRF4 protein is a key factor regulating B cell development, such as early B cell development, pre-B cell switch recombination, mature B cell somatic hypermutation, and also a key factor regulating plasma cell differentiation. In addition, in recent years, it has been reported that Irf4 gene abnormalities or abnormal protein expression is closely involved in the occurrence and development of a variety of B cell or plasma cell tumors. This article reviews the physiological role of IRF4 in the differentiation and maturation of B cell or plasma cells, how IRF4 participates in the occurrence and development of B cell or plasma cell tumors, and its potential therapeutic target for B cell or plasma cell tumors.

  • Predictive value of liver function and free triiodothyronine levels for hemorrhagic transformation after intravenous thrombolysis in patients with acute ischemic stroke

    Objective To analyze the influencing factors of hemorrhagic transformation (HT) after intravenous thrombolysis with recombinant tissue plasminogen activator (rt-PA) in patients with acute ischemic stroke (AIS). Methods AIS patients hospitalized in the Department of Neurology of the First Affiliated Hospital of Zhengzhou University between June 2017 and June 2020 and receiving rt-PA intravenous thrombolysis were selected. Patients were divided into two groups according to whether they had HT, HT group and non-HT group. General data such as patient’s age, sex, past history, score of National Institute of Health Stroke Scale (NIHSS) before thrombolysis, and related biochemical examination indicators were collected, to analyze the difference between the patients with HT or not, and analyze the related factors affecting the HT of AIS patients after intravenous thrombolysis. Results A total of patients 323 were included. Among them, 46 cases (14.2%) had HT, and 277 cases (85.8%) had no-HT. Except for serum free triiodothyronine (FT3), atrial fibrillation, hypertension, cerebral infarction area, NIHSS score before thrombolysis, uric acid, blood glucose before thrombolysis, white blood cell count, albumin level, alanine aminotransferase, aspartate aminotransferase / alanine aminotransferase and C-reactive protein (P<0.05), there was no significant difference in other indexes between the two groups (P>0.05). Logistic regression analysis showed that NIHSS score≥13 before thrombolysis, aspartate aminotransferase / alanine aminotransferase, blood glucose before thrombolysis≥12.74 mmol/L, low FT3 level, massive cerebral infarction, and atrial fibrillation were independent risk factors for HT after thrombolysis in AIS. Conclusions FT3 and aspartate aminotransferase / alanine aminotransferase levels may be good biomarkers for predicting HT after intravenous thrombolysis. For patients with reduced albumin and uric acid levels, supplementation of exogenous uric acid and albumin may help reduce the risk of HT after AIS thrombolysis.

  • Clinical efficacy of bronchoscope in the treatment of transbronchial broncholithiasis

    ObjectiveTo evaluate the efficacy and safety of bronchoscope in the treatment of transbronchial broncholiths.MethodsThe clinical data, treatment methods, clinical efficacy and complications of 18 patients who were diagnosed with transbronchial broncholiths were etrospectively reviewed in the Respiratory Department of the Second Hospital of Lanzhou University during January 2010 to December 2017.ResultsEighteen patients were diagnosed by bronchoscopy with a total of 22 pieces of stone for transbronchial broncholiths. An elective surgical procedure was performed in 2 cases. Six cases were treated with removing broncholiths once by foreign body forceps under bronchoscope. Repeated treatments were performed in 10 patients. Two cases underwent massive hemorrhage in the process of one-time broncholiths removal by foreign body forceps under bronchoscope, and one case suffered from bronchial fistula after the broncholiths removal, all of the above 3 patients underwent surgical treatment. All patients were followed up for 5 months without any long-term complications.ConclusionsRemoval of broncholithiasis by bronchoscopic forceps can effectively treat transbronchial broncholiths with low complication and high safety. If the broncholiths cause severe distal lung tissue injury, massive hemoptysis, bronchial fistula, or the diagnosis is unclear, surgical treatment is required. When the transbronchial broncholith is difficult to remove completely at once, batch could be chosen. First, to remove one portion which causes the obstruction of airway; and then, to remove the left part of the transbronchial broncholith by stages.

  • Summary of best evidence for prevention and management of intracranial infections related to lumbar drainage

    Objective To retrieve, evaluate, and summarize evidence on the prevention and management of intracranial infections related to lumbar drainage (LD), in order to provide scientific references for clinical practice and decision-making. Methods The literature on the prevention and management of LD-related intracranial infections in LD-related websites and databases was systematically searched, with a search period from the establishment of databases to June 30, 2024. The included literature was evaluated for quality and integrated into evidence. Results A total of 9 articles were included, including 3 guidelines, 1 evidence summary, 1 expert consensus, 1 systematic review, and 3 original studies. A total of 30 pieces of evidence were formed, covering six aspects: risk management, catheter placement, catheter maintenance, extubation, diagnosis and treatment of intracranial infections, education and training. ConclusionsThe prevention and management of LD-related intracranial infections involve multiple pieces of evidence, and medical staff should selectively apply the best evidence based on patient and clinical conditions to reduce the incidence of intracranial infections and improve medical quality.
