  • Application of Fast-Track Surgery and Laparoscopy in Treatment for Colorectal Cancer

    Objective To explore the variation about the application of fast-track surgery and laparoscopy in treatment for colorectal cancer in recent years. To investigate the probability of combining protocols of the two for treatment for colorectal cancer. Methods The clinical and basic literatures of related researches about colorectal treatment of laparoscopy and fast-track surgery were collected and reviewed. Results Compared with the traditional treatment modalities, both of fast-track surgery and laparoscopy used for the treatment of colorectal cancer have better clinical effects. Conclusions Fast-track surgery and laparoscopic techniques used for the treatment of colorectal cancer are feasible, but the combination of the two should be confirmed by further randomized controlled trials.

  • 胸腔镜微创术后不留置胸腔引流管在快速康复外科中的应用

    目的探讨患者接受胸腔镜微创手术后不留置胸腔引流管的安全性和可行性。 方法回顾性分析2013年1~9月广东省人民医院胸外科20例行胸腔镜微创手术后不留置胸腔引流管患者的临床资料,其中男14例、女6例,年龄36.7(17~68)岁,分析患者术后恢复情况。 结果全组20例患者均顺利完成手术,无围手术期死亡,无二次手术;术后均未出现严重并发症,均不需有创性操作;术后平均住院时间3.5 d,出院后1周复查胸部X线片均无异常。 结论部分患者接受胸腔镜微创手术后不留置胸腔引流管是安全可行的,符合快速康复外科理念。

    目的探讨快速康复外科理念在脊柱微创手术围手术期护理中的应用体会。 方法2013年5月-2014年3月,将快速康复外科理念应用于62例因椎间盘突出症行脊柱微创手术患者的围手术期护理中,具体方法包括术前超前康复训练,超前镇痛,术后液体控制,尽早进食,术后早期活动,出院指导等。 结果快速康复外科理念运用后,患者术后2 h饮水,6 h进食,疼痛视觉模拟评分(2.25±0.07)分,术后第1天即下床活动。 结论将快速康复外科理念应用于脊柱微创手术围手术期护理,能减轻患者痛苦,有利于缩短住院时间,促进患者康复,提高患者满意度。

  • Clinical Application of Fast Track Surgery in Treatment for Cholecystolithiasis Combined with Calculus of Common Bile Duct by Combination of Laparoscope and Duodenoscope

    Objective To evaluate the use of fast track surgery (FTS) in the treatment for cholecystolithiasis combined with calculus of common bile duct (CBD) by combination of laparoscope and duodenoscope. Methods One hundred and twenty patients with cholecystolithiasis combined with calculus of CBD underwent laparoscopic cholecyst-ectomy (LC) and endoscopic retrograde cholangiopancreatography (ERCP) were divided into FTS group (n=55) and conventional group (n=65),which were accepted the perioperative therapy of FTS or conventional therapy,respectively. After operation,the incision pain,nausea and vomiting,infusion time,loss of body weight,out-of-bed time,dieting time,postoperative hospitalization,hospital costs,and complications were compared in two groups. Results Compared with the conventional group,the postoperative infusion time,dieting time,out-of-bed time,and postoperative hospitali-zation were shorter,the incidence rates of pulmonary infection,and urinary systems infection,pancreatitis,nausea and vomiting, and incision pain were lower,the loss of body weight was lower in the FTS group (P<0.05),but the differences of WBC and serum amylase at 24 h after operation were not significant between the FTS group and conventional group(P>0.05). Conclusion The FTS is safe,economic,and effective in the treatment for cholecystolithiasis combined with calculus of CBD by combination of laparoscope and duodenoscope.

  • Application of multi-subjects and multi-modes intervention in the enhanced recovery afterpancreaticoduodenectomy

    Objective The objective of this study is to evaluate the effect of enhanced recovery after surgery (ERAS) in the perioperative period of pancreatoduodenectomy. Methods This article conducted the forward-looking analysis on the information of 227 patients undergoing the pancreatoduodenectomy in West China Hospital from January 2016 to June 2017, and then compared the differences between the patients subjected to ERAS (ERAS group) and thosesubjected to regular measures (control group) with respect to time of setting in sickbed, time of mobilizing out ofsickbed, time of starting drink water, time of resumption of diet, exhaust time, defecation time, the time of nasogastric tube, postoperative hospitalization duration and expenses, postoperative complications, and postoperative pain scores. Results ① Postoperative indexes: by comparison of the ERAS group and the control group, it was found that the ERAS group had shorter (or lower) time of setting in sickbed, time of mobilizing out of sickbed, time of starting drink water, time of resumption of diet, exhaust time, defecation time, the time of nasogastric tube, postoperative hospitalization duration and expenses (P<0.05). ② Postoperative complications: of all postoperative complications, including pancreatic fistula, postoperative hemorrhage, delayed gastric emptying, biliary fistula, abdominal infection, incision complication, lung infection, and heart complication were without statistically significant differences (P>0.05) between the 2 groups.③ Reoperation and readmission: there was no significant difference on the incidences of reoperation and readmission between the 2 groups (P>0.05). ④ Postoperative pain scores: except 22 : 00 of the 6-day after operation, the pain scores in the ERAS group were all lower than those in the control group at 2 h and 8 h after operation, and the time points of 1–6 days after operation (8 : 00, 16 : 00, and 22 : 00), with statistically differences (P<0.05). Conclusion Without increasing the incidence of complications, ERAS may speed up the rehabilitation of patients undergoing the pancreatoduodenectomy and mitigate the pain of patients.

  • 肾脏部分切除术患者快速康复护理与传统护理的对照研究

    目的评估快速康复护理在肾脏部分切除术患者围手术期应用的可行性及安全性。 方法将2010年1月-2013年7月收治的144例肾脏部分切除患者,按住院号尾数的奇偶分为快速康复组(75例)和传统护理组(69例),分别实施快速康复措施与传统护理方法,观察比较两组患者围手术期恢复的相关指标,评价快速康复护理的效果。 结果快速康复组术后恢复6个指标优于传统护理组(P<0.05),平均住院时间短于传统护理组(P<0.05),且两组并发症发生率差异无统计学意义(P>0.05)。 结论运用缩短术前禁食时间、早期下床活动、早期由口进食、早期拔除引流管等快速康复措施是安全有效的,可加速患者术后康复,缩短术后住院时间,值得推广应用。

  • Development and future of the minimally invasive esophagectomy for esophageal cancer

    In this review, development and application of the minimally invasive esophagectomy(MIE) for esophageal cancer are discussed including the types of MIE procedures, short- and long- term outcome after MIE; as well the future of MIE is forecasted. Main procedures of MIE performed currently include esophagectomy via thoracoscopy and laparoscopy and cervical esophagogastrosty, Ivor-Lewis MIE via thoracoscopy and laparoscopy, and hiatal MIE. Ivor-Lewis MIE gradually becomes a standard surgical option for the cancer of distal esophagus or esophagogastric junction while the solution of intrathoracic anastomosis via thoracoscopy has achieved. Several methods of intrathoracic anastomosis are reported such as hand-sewn, circular stapler, side-to-side and triangular anastomosis. MIE could decrease operative blood loss, shorten hospital stay and ICU stay, reduce postoperative especially pulmonary complications, and harvest more lymph nodes compared to open esophagectomy. The long-term survival has been proved similar with that after open esophagectomy for esophageal cancer. MIE has developed rapidly in recent years with some aspects in future prospectively: individual MIE treatment and quality of life, fast track after surgery, and robot-assisted MIE, as well the endoscopic submucosal dissection for esophageal cancer is mentioned.

  • Fast-track Recovery of Cardiopulmonary Function after Complete Video-assisted Thoracoscopic Lobectomy

    Objective To investigate the impact of complete video-assisted thoracoscopic lobectomy and open lobectomy on perioperative heart rate (HR) and blood oxygen saturation (SO2) of lung cancer patients,and explore whether minimally invasive surgery can enhance postoperative recovery of lung cancer patients. Methods A total of 138 lung cancer patients were chosen from 161 consecutive patients with pulmonary diseases who were admitted to West China Hospital of Sichuan University between September 2010 and December 2011. According to different surgical approach,all the 138 lung cancer patients were divided into routine thoracotomy group (thoracotomy group,70 patients including 53 males and 17 females with their average age of 56.1±9.7 years) and complete video-assisted thoracoscopic lobectomy group (VATS group,68 patients including 46 males and 22 females with their average age of 53.4±6.5 years). There was no statistical difference in preoperative clinical characteristics between the 2 groups. Preoperative and postoperative (1st,3rd,7th and 30th day) numeric pain rating scale (NPRS),HR and SO2 were compared between the 2 groups. Results (1) There was no statistical difference in NPRS on the 1st and 3rd postoperative day between the 2 groups (3.83±0.79 vs. 3.93±0.67, 2.88±0.59 vs. 3.03±0.71,P>0.05),but on the 7th and 30th postoperative day,NPRS of the thoracotomy group was signi- ficantly higher than that of VAST group (1.61±0.33 vs. 1.22±0.12,1.58±0.26 vs. 1.19±0.31,P<0.05). (2) Postop- erative sedentary HR of both VATS group and thoracotomy group were significantly higher than preoperative levels [(84.13±17.21) / minute vs. (73.67±10.32)/minute, (86.13 ±19.67) / minute vs. (72.24±14.21) / minute, P<0.05]. Postoperative HR of VATS group decreased to preoperative level on the 3rd postoperative day,while postoperative HR of the thoracotomy group decreased to preoperative level on the 7th postoperative day. (3) There was no statistical difference between preoperative and postoperative (all the time points) sedentary SO2 of both VATS group and thoracotomy group (96.34 %±2.11% vs. 97.12%±2.31%,95.33%±4.13% vs. 94.93% ±4.31%,P>0.05).(4) The changes of HR and SO2 before and after exercise of VATS group were significantly smaller than those of the thoracotomy group on the 3rd postoperative day [(11.11±4.81)/minute vs. (18.23±6.17)/minute,3.1%±1.2% vs. 7.4 %±2.7%,P<0.05] . Conclusion The impact of complete video-assisted thoracoscopic lobectomy on cardiopulmonary function is comparatively smaller,which is helpful for postoperative fast-track recovery of lung cancer patients.

  • System Evaluation about Application of Enhanced Recovery after Surgery in Radical Resection of Gastric Cancer

    ObjectiveTo investigate the safety and feasibility of enhanced recovery after surgery (ERAS) in radical gastrectomy. MethodsThe Cochrane library, PubMed, EMBASE, and Chinese biomedical literature databases were searched to identify randomized controlled trails including patients undergoing radical gastrectomy surgery who were treated by ERAS or traditional methods.The five outcome variables were the time to passage of first flatus and first defecation, the length of postoperative hospital stay, medical cost, and postoperative complications. ResultsFifteen published reports involving 1 533 participants met the inclusion criteria.Compared with traditional methods, ERAS had better outcomes with shorter time to the passage of flatus (WMD=-26.557 hours, 95%CI:-34.097~-19.018, P < 0.05), shorter time to the passage of defecation (WMD=-34.071 hours, 95%CI:-53.449~-14.693, P < 0.05), shorter postoperative hospital stay (WMD=-2.824 days, 95%CI:-3.638~-2.010, P < 0.05), less medical cost (WMD=-0.342 ten thousand yuan, 95%CI:-0.501~-0.184, P < 0.05), and lower postoperative complication rate (RR=0.597, 95%CI: 0.435-0.820, P < 0.05).Especially, ERAS had a significant lower rate of postoperative digestive tract complications (RR=0.492, 95%CI: 0.350-0.693, P < 0.05).However, there were no significant differences between two methods in the incidence of complications including postoperative pulmonary infection, infection or liquefaction of incision, fistula of anastomosis, and postoperative readmission rate (P > 0.05). ConclusionsThe application of ERAS in the perioperated period of radical gastrectomy surgery shortens the time to passage of first flatus and first defecation, the postoperative hospital stay, as well as lowers the medical cost, and the postoperative complication rate.The results suggest that ERAS is safe and effective in perioperated period of radical gastrectomy and it is very promising in clinical application.
