  • Cardiovascular events after acute exacerbation of bronchiectasis

    Objective To investigate the cardiovascular events (CVE) and survival status of patients with bronchiectasis (BE) during follow-up after acute exacerbation. Methods Prospective cohort study was used. Clinical data of 134 BE patients with acute exacerbation who were hospitalized from July 2016 to September 2020 were collected. The patients were followed up after discharge by phone or respiratory clinic every 3 months until November 2022. CVE or death was the endpoint event. Result During the follow-up period, 41 patients developed CVE, while 93 patients did not. Fifty-one patients died during the follow-up period, with a mortality rate of 38.06%. Among them, 41 cases of CVE resulted in 21 deaths, with a mortality rate of 51.22%; 30 cases died in 93 non-CVE patients, with a mortality rate of 32.26%. Logistic regression results showed significant influencing factors for CVE in BE patients were age, hypertension, chronic obstructive pulmonary disease (COPD), and moderate to severe illness. The significant influencing factors for the death of BE patients were age, COPD, moderate and severe illness, and CVE events. The significant influencing factors for the death of CVE patients were age and receiving CVE treatment. The area under ROC curve (AUC) and 95%CI was 0.858 (0.729 - 0.970) for the warning model for CVE in BE patients. The AUC (95%CI) was 0.867 (0.800 - 0.927) for the warning model for death in BE patients. The AUC (95%CI) was 0.811 (0.640 - 0.976) for the warning model for death of CVE patients. Conclusions Population factors and comorbidities are risk factors for CVE in BE patients after acute exacerbation. The appearance of CVE worsens the long-term prognosis of BE patients. The corresponding warning models have high warning effectiveness with AUC>0.8.

  • The Role of Serum Procalcitonin in Antibiotics Use for Acute Exacerbations of Chronic Obstructive Pulmonary Disease

    Objective To evaluate the value of serumprocalcitonin( PCT) guided antibiotic strategy in the treatment of acute exacerbation of chronic obstructive pulmonary disease( AECOPD) .Methods From August 2011 to June 2012, a total of 96 patients hospitalized for AECOPD were randomly assigned into a PCT-guided group( n = 48) and an empirical therapy group( n = 48) . The PCT levels of PCT-guided group were measured by immunochemiluminometric assays before and 5,7, 10 days after treatment. The PCT-guided group was treated with antibiotics according to serum PCT levels, ie. antibiotic treatment was applied when PCT level ≥ 0. 25 μg/L and was discouraged when PCT level lt; 0. 25 μg/L. The empirical therapy group received antibiotics according to physician’s decision. The antibiotics usage rate, length of antibiotic exposure, length of hospitalization, clinical efficacy, hospital mortality, rate of invasive mechanical ventilation and costs of hospitalization were recorded. Results The antibiotics usage rate, length of antibiotic exposure, length of hospitalization, and costs of hospitalization in the PCT-guided group were all lower than those of the empirical therapy group( P lt;0.05) while clinical efficacy, hospital mortality and rate of invasive mechanical ventilation were similar in two groups(Pgt;0.05) . Conclusion PCT guided antibiotic strategy can be used in the treatment of AECOPD, which may reduce the dose of antibiotic drugs to avoid bacterial resistance and lower costs of hospitalization.

  • Prognostic Factors of Hospitalized Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease

    Objective To investigate the prognostic factors related to in-hospital mortality in patients with acute exacerbation of chronic obstructive pulmonary disease ( AECOPD) . Methods A prospective cohort study was carried out in AECOPD patients admitted in three district general hospitals of Jiangyou city, Sichuan province from February 2007 to February 2008. The clinical and epidemiological data at admission and all-cause death in hospital were recorded. The in-hospital mortality rate and potential determinants of mortality of AECOPD were analyzed using Logistic regression method. Results 257 AECOPD inpatients with AECOPD were recruited into the cohort study. The in-hospital mortality rate was 5.84% (15/257) . Univariate analysis showed in-hospital mortality was significantly associated with age, FEV1% pred, arterial oxygen tension ( PaO2 ) , arterial oxygen saturation ( SaO2 ) , pH, and Charlson’s complication index. Multivariate logistic regression model showed that lower arterial oxygen tension ( OR 4.775;95%CI 1.545 ~14.757; P =0.007) and higher Charlson’s complication index ( OR 4. 608; 95% CI 1. 330 ~15. 966; P =0. 016) were significantly associated with in-hospital mortality after adjustment by age. Conclusion For in-patients with AECOPD, PaO2 and Charlson’s complication index are independent risk factors associated with in-hospital mortality.

  • Frailty increases the risk of in-hospital mortality in older patients with acute exacerbation of chronic obstructive pulmonary disease: a real-world study

    ObjectiveTo explore the association between frailty and in-hospital mortality in older patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods Elderly patients who were hospitalized with AECOPD from June 2022 to December 2022 at a large tertiary hospital were selected. The independent prognostic factors including frailty status were determined by multivariate logistic regression analysis. Mediation effect analysis was used to evaluate the mediating relationships between C-reactive protein (CRP) and albumin and in-hospital death. ResultsThe training set included 1 356 patients (aged 86.7±6.6), 25.0% of whom were diagnosed with frailty. The multiple logistic regression analysis showed that frailty, mean arterial pressure, Charlson comorbidity index, neutrophil–lymphocyte ratio, interleukin-6, CRP, albumin, and troponin T were associated with in-hospital mortality. Furthermore, CRP and albumin mediated the associations between frailty and in-hospital mortality. ConclusionFrailty may be an adverse prognostic factor for older patients admitted with an AECOPD. CRP and albumin may be parts of mechanism between frailty and in-hospital death.

  • Gastroesophageal reflux is associated with incresead exacerbation of chronic obstructive pulmonary disease

    Objective To investigate the prevalence and predisposing factors of gastroesophageal reflux(GER)in patients with chronic obstructive pulmonary disease(COPD)and its impacts on the frequency of COPD exacerbations.Methods 50 patients with acute exacerbation of COPD were enrolled in the study.All patients filled a modified version of clinically validated standardized Mayo Clinic GER questionnaire.Pulmonary function tests(PFT),body mass index(BMI),serum albumin,and medications were recorded.The patients were divided in to GER group(13 patients with GER symptoms longer than one year and diagnosed GER by gastroscope) and non-GER group(37 patients without GER symptoms).Results There were no significant differences in the BMI,serum albumin,use of inhaled steroids and theophylline between the two groups.Forced expiratory volume in one second as a percentage of the predicted value(FEV1%pred)(37% vs 38.3%,P=0.608.),residual volume % predicted(123.4 % vs 137.8%,P=0.222),and residual volume to total lung capacity(139.4% vs 141.5%,P=0.798) were not significantly different between the two groups.The number of COPD exacerbations per year was significantly higher in patients with GER compared to patients without GER(4.5 vs 1.3,P=0.006).Conclusions The severity of airflow obstruction and the hyperinflation is unlikely to contribute to the development of GER.Patients with GER experience significantly more exacerbations per year when compared to patients without GER,suggesting more highlight warrant in treatment of GER in COPD patients.

  • 有创-无创序贯机械通气治疗COPD急性加重

    目的 评价有创-无创序贯机械通气治疗COPD 急性加重( AECOPD) 的临床疗效。方法 选择ICU 收治的AECOPD 患者13 例为序贯组, 同样病情患者12 例为对照组。临床出现“肺部感染控制窗”后, 序贯组拔除气管插管, 应用口鼻面罩双水平正压通气直至脱机; 对照组继续有创机械通气, 以压力支持方式脱机。比较两组患者机械通气时间、住院时间、呼吸机相关性肺炎( VAP)患病率和患者转归。结果 与对照组比较, 序贯组有创机械通气时间显著缩短[ ( 4. 33 ±1. 05) d 比( 10. 13 ±2. 06) d, P lt; 0. 001] , ICU 住院时间显著缩短[ ( 8. 79 ±2. 07) d 比( 11. 96 ±2. 11) d, P lt;0. 005] , VAP 发生数显著减少[ 0 比6 例, P lt;0. 01] 。总机械通气时间、总住院时间、再插管率和病死率均无显著性差异( P gt; 0. 05) 。结论 对需要机械通气的AECOPD 患者, 采用有创-无创序贯机械通气治疗, 可以缩短有创机械通气时间和ICU住院时间, 降低VAP 患病率。

  • The clinical phenotype of acute exacerbation of chronic obstructive pulmonary disease by cluster analysis

    ObjectiveTo explore the clinical phenotype of patients with acute exacerbation of chronic obstructive pulmonary disease (COPD) by cluster analysis and provide a basis for individualized treatment.MethodsA total of 515 patients with acute exacerbation of COPD admitted to this department from January 2014 to December 2016 were enrolled. The age, duration, smoking index, number of hospitalizations in the past 1 year, hospitalization days, treatment costs and other information were collected for cluster analysis.ResultsThe patients were divided into three categories of phenotype: " mild-glucocorticoid resistance-antibiotic dependent”," mild-glucocorticoid sensitive”, and " serious complication”. The patients with the first two phenotypes had a milder condition and lower hospitalization costs. There were differences in the time and cumulative dose of glucocorticoids in different pathways, antibiotic use time and usage rate. The third phenotype was the most serious, with the highest cost of hospitalization, and may merge or co-exist with other diseases such as cardiovascular disease and digestive tract disease.ConclusionCluster analysis may identify different phenotypes of acute exacerbation of COPD to provide a reference for clinical individualized treatment.

  • Clinical analysis of two patients with frequent acute exacerbations of chronic obstructive pulmonary disease, both caused by Aspergillus?

    ObjectiveTo investigate the role of Aspergillus in the severe refractory exacerbations of chronic obstructive pulmonary disease (COPD).MethodsThe clinical data of two COPD patients suffering from refractory acute exacerbations were analyzed and the relevant literature were reviewed.ResultsTwo patients were male, aging 72 and 64 years respectively. Both of them had a history of frequent acute exacerbations with severe COPD recently. Meanwhile, they received intravenous use of antibiotics repeatedly, one of them took oral corticosteroids to control wheezing, but failed. Their serum Aspergillus-specific IgG antibody was weakly positive. Besides traditional treatment, they received additional antifungal therapy, and the symptoms alleviated. There was no acute exacerbation in the half a year follow-up period after appropriate therapy.ConclusionsAspergillus colonization, sensitization, infection should be considered in patients with severe COPD. When Aspergillus-associated evidence are acquired, antifungal therapy will be unexpected helpful.

  • Development and validation of a nomogram for predicting the prognosis of acute exacerbation of chronic obstructive pulmonary disease combined with type II respiratory failure

    Objective To develop and validate a nomogram model that can be used to predict the prognosis of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) patients with type II respiratory failure. Methods A retrospective analysis was conducted on the clinical data of 300 hospitalized AECOPD patients in the People’s Hosipital of Leshan from August 2016 to December 2021. Patients were grouped into a training cohort (n=210) and a validation cohort (n=90) in a 7:3 ratio. The variables for the patients in the training cohort were selected using the least absolute shrinkage and selection operator (LASSO), followed by multivariate logistic regression analysis to identify independent risk factors of poor prognosis in AECOPD with type II respiratory failure, and a nomogram model was constructed. Receiver operating characteristic (ROC) curves were plotted for the training and validation cohorts, and the area under ROC curve (AUC) was calculated.The model was validated by conducting the Hosmer-Lemeshow test, drawing calibration curves, and performing decision curve analysis(DCA).ResultsCardiovascular disease, lymphocyte percentage, and red cell distribution width-standard deviation(RDW-SD) were identified as independent risk factors of poor prognosis for AECOPD patients with type II respiratory failure (P<0.05). The AUC values for the training and validation cohorts were 0.742 (95%CI: 0.672-0.812) and 0.793 (95%CI: 0.699-0.888), respectively. The calibration curves of the two cohorts are close to the desirable curves.The Hosmer-Lemeshow test P-values were greater than 0.05, indicating good clinical practicality. The DCA curve indicates that the model has good clinical value. Conclusions The clinical prediction model, based on factors such as cardiovascular disease, lymphocyte percentage, and RDW-SD, showed good predictive value for AECOPD patients complicated by type II respiratory failure.

  • Impact and Association of the COVID-19 pandemic and respiratory infection prevalence on hospitalization for acute exacerbation of chronic obstructive pulmonary disease

    Objective To understand the changing patterns and characteristics of the number of patients hospitalized with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) before, during, and in the post-epidemic period of the COVID-19 pandemic and the Association between acute respiratory infections and hospitalization of patients with AECOPD. Methods A retrospective analysis was conducted to count the patients hospitalized for AECOPD in the Department of Respiratory Medicine of the Third Affiliated Hospital of Chongqing Medical University from July 2017 to June 2024. The pattern of change in the number of AECOPD hospitalizations and the associations with patients with respiratory tract infections in outpatient emergency departments were analyzed. Results During the COVID-19 epidemic, the number of hospitalizations of patients with AECOPD did not increase compared with the pre-epidemic period. Instead, it significantly decreased, especially in the winter and spring peaks (P<0.05). The only exception was a peak AECOPD hospitalization in the summer of 2022. COPD inpatient mortality and non-medical discharge rates tended to increase during the epidemic compared with the pre-epidemic period. Analysis of the curve of change in the number of patients with respiratory infections in our outpatient emergency departments during the same period revealed a downward trend in the number of patients with respiratory infections during the epidemic and an explosive increase in the number of patients with respiratory infections in the post epidemic period, whose average monthly number was more than twice as high as that during the epidemic. Correlation analysis of the number of patients with respiratory infections between AECOPD hospitalizations and outpatient emergency departments showed that there was a good correlation between the two in the pre-epidemic and post-epidemic periods, and the correlation between the two in the post-epidemic period was more significant in particular (r=0.84-0.91, P<0.001).In contrast, there was no significant correlation in 2021 and 2022 during the epidemic (r=0.24 and 0.50, P>0.05 ). The most common respiratory infection pathogens among AECOPD hospitalized patients during the post-epidemic period were influenza virus, COVID-19 virus, and human rhinovirus, respectively. Conclusions The pandemic period of COVID-19 infection did not show an increase in the number of AECOPD hospitalizations but rather a trend towards fewer hospitalizations. Respiratory infections were strongly associated with the number of AECOPD hospitalizations in the pre- and post-pandemic periods, while the correlation between the two was poorer during the pandemic period. Influenza virus was the most important respiratory infection pathogen for AECOPD during the post-epidemic period.
