目的:分析局部枸橼酸抗凝在地震挤压伤患者CRRT治疗中的效果和安全性,并比较不同实施方式的差异。方法:回顾性的分析汶川地震后我院收治的因挤压伤而接受CRRT治疗的患者中局部枸橼酸抗凝的情况。共计39例患者因挤压伤和多器官功能障碍而接受局部枸橼酸抗凝CRRT治疗。按照不同的实施方式分为A组(23例)以同步输入方式进行,B组(16例)以预充式进行。治疗过程中监测患者的肝肾功能、电解质、凝血指标、血常规等。分析不同治疗组局部枸橼酸抗凝治疗的效果和安全性。结果:39例患者死亡4例,其余35例均康复。存活患者治疗后SCr和BUN指标均较治疗前明显降低,差异有统计学意义,Scr:A组(503±215)μmol/L对(149±129.7)μmol/L,B组(577±227)μmol/L对(180.6±146.5)μmol/L,Plt;0.05;BUN:A组(26.6±10.4) mmol/L对(9.3±6.9)mmol/L,B组(30.5±8.1)mmol/L对(10.9±5.72)mmol/L,Plt;0.05。两组滤器后ACT值均较外周血ACT值明显延长且差异有统计学意义,A组:(161±31) s 对 (122±25)s,B组:(157±33)s 对 (125±31) s,Plt;0.05。A组滤器和管路寿命(47.6±11.2)h与B组(41.3±14.5)h相近,A组略长于B组,但两组差异无统计学意义(Pgt;0.05)。其他凝血指标差异无统计学意义。在治疗过程中未发生严重电解质和酸碱平衡紊乱。结论:局部枸橼酸抗凝适于地震挤压伤患者CRRT治疗,这种抗凝方式抗凝效果确切,而且不会加重患者的凝血功能紊乱,不会增加患者活动性出血的风险。同步式局部枸橼酸抗凝在延长滤器和管路方法可能优于预充方式
目的 探讨心脏机械瓣膜置换术后患者出院指导的综合方法,评价其效果。 方法 将2010年9月-2011年1月242例心脏机械瓣膜置换术患者按入院先后顺序随机分成试验组和对照组。试验组患者采用综合出院指导方案,对照组采用常规宣教方法,在6个月后对两组患者进行调查,评价综合指导方案的效果,并进行统计学分析。 结果 试验组患者均未出现不良并发症,对照组有1例出院后未遵医嘱服药及定期复查,死于血管栓塞;有2例出现血管栓塞,2例牙龈出血,经及时治疗后好转。 结论 对心脏机械瓣膜置换术患者出院时,发放健康教育资料及定期随访指导的综合出院指导方案,可以降低术后并发症,提高患者认知水平和满意度,帮助患者提高依从性,提高生存质量。
Objective To evaluate the efficacy and safety of anticoagulant agents, such as aspirin and heparin, in women with a history of at least two spontaneous miscarriages or one later intrauterine fetal death without apparent causes other than inherited thrombophilias. Methods We searched the Cochrane Pregnancy and Childbirth Group trials register (March 2004), the Central Register of Controlled Trials (The Cochrane Library, Issue 1, 2004), MEDLINE (January 1966 to March 2004), and EMBASE (1980 to March 2004). We scanned bibliographies of all located articles for any unidentified articles. Randomised and quasi-randomised controlled trials that assessed the effect of anticoagulant treatment on the live-birth rate in women with a history of at least two spontaneous miscarriages or one later intrauterine fetal death without apparent causes other than inherited thrombophilias were eligible. Interventions included aspirin, unfractionated heparin, and low molecular weight heparin for the prevention of birth loss. One treatment could be compared with another or with placebo. Two authors assessed the trials for inclusion in the review and extracted the data. Data were entered into the Review Manager software and double checked. Results Two studies (242 participants) were included in the review and for both of them data were extracted for the subgroups of women fulfiling the inclusion criteria of the review. In one study, 54 pregnant women with recurrent spontaneous abortion without detectable anticardiolipin antibodies were randomised to low-dose aspirin or placebo. Similar live-birth rates were observed with aspirin and placebo [relative risk (RR) 1.00, 95% confidence interval (CI) 0.78 to 1.29]. In another study, a subgroup of 20 women who had had a previous fetal loss after the 20th week and had a thrombophilic defect were randomised to enoxaparin or aspirin. Enoxaparin treatment resulted in an increased live-birth rate, as compared to low-dose aspirin, RR 10.00, 95% CI 1.56 to 64.20). Conclusions The evidence on the efficacy and safety of thromboprophylaxis with aspirin and heparin in women with a history of at least two spontaneous miscarriages or one later intrauterine fetal death without apparent causes other than inherited thrombophilias is too limited to recommend the use of anticoagulants in this setting. Large, randomised, placebo-controlled trials are urgently needed.
Objective To investigate the risk factors and the prevention and cure methods of ischemic stroke during low intensity anticoagulation therapy after mechanical heart valve replacement. Methods From March 2004 to July 2008,twentythree patients with ischemic stroke after mechanical heart valve replacement had been researched(ischemic stroke group). One hundred and twenty patients who had undergone mechanical heart valve replacement were randomly chosen in the same period as control group. Gender, age, the dose of warfarin , anticoagulation intensity(INR), INR review interval, left atrial diameter and heart rhythm were compared between the two groups, and the risk factors of ischemic stroke were analyzed by logistic regression analysis. Results (1) Patients in ischemic stroke group all discharged from hospital after treatment, and they were followed up for 1 month-3 years after discharged. All the patients’ neurological complications improved obviously, and no recurrent embolism and severe hemorrhage was found. (2) There was no statistical significance between two groups in gender, age and the dose of warfarin(Pgt;0.05). (3) Nonconditional logistic regression analysis on influence factors showed that atrial fibrillation(P=0.000), left atrial enlargement(P=0.002), low anticoagulation intensity(P=0.012) and longtime INR review interval(P=0.047)were the risk factors of ischemic stroke during low intensity anticoagulation therapy after mechanical heart valve replacement. Conclusions (1)The prognosis of ischemic stroke during low intensity anticoagulation therapy after mechanical heart valve replacement is better than that of intracranial hemorrhage, and the occurrence of ischemic stroke is related to many risk factors. (2)The influences of risk factors should be minimized in order to avoid ischemic stroke. (3) Early low intensity anticoagulation therapy is safe and effective for patients with ischemic stroke after heart valve replacement.
Objective To investigate the effects of antithrombin-Ⅲ ( AT-Ⅲ) on the inflammatory reaction in oleic acid-induced acute lung injury ( ALI) rats. Methods Sixtymale Sprague-Dawley rats were randomly divided into five groups, ie. a normal control group, an ALI group, an AT-Ⅲ treatment group, an AT-Ⅲ +heparin treatment group, and a heparin treatment group ( n =12) . The ALI rats were induced by injecting oleic acid ( 0. 2 mL/kg) intravenously. The lung histology was scored by modified Smithtechnique. The albumin permeability of pulmonary microvascular ( Palb) was measured by single nuclide tracer technique. The extravascular lung water ( EVLW) and wet/dry weight ratio ( W/D) of lung tissues were measured by gravity way. The activity of AT-Ⅲ in plasma was determined by the method of syntheticchromogenic substrate. Tumor necrosis factor α( TNF-α) , interleukin 6 ( IL-6) and von Willebrand factor ( vWF) levels in serum were determined using commercial enzyme-linked immunosorbent assay kits. The expressions of lung tissue extacellular signal-regulated kinases ( ERK) -1 /2, P38 mitogen-activated proteinkinase ( MAPK) and c-jun N-terminal kinases ( JNK) were determined by Western blot. Results The Smith scores, EVLW, Palb , plasma level of vWF, lung tissue levels of phospho-ERK1 /2 and phospho-P38 MAPK expressions in the ALI group were all significantly higher than those in the normal control group ( P lt; 0.05) , while not significant differentwith other three treatment groups. There were not significant differences in the activity of AT-Ⅲ in plasma and phospho-JNK expression among all five groups. The serum levels of TNF-αand IL-6 in the ALI group were significantly higher than those in the normal control group and three treatment groups. Conclusions AT-Ⅲ downregulates the levels of downstreamcytokines TNF-αand IL-6,but can not inhibite the activation of ERK1 /2 and P38 MAPK, and can not relieve endothelial permeability.The study do not demonstrate the lung protective effect of AT-Ⅲ in oleic acid-induced acute lung injury.
Objective To compare the result of different anticoagulation methods after mechanical heart valve replacement in order to optimize the early-stage anticoagulation therapy. Methods Eighty-six patients with mechanical valve replacement were divided into four groups according to different anticoagulation methods, warfarin group 1 : warfarin was given from the 1st day after operation; warfarin group 2: warfarin was given from the 2nd day after operation; dipyridamole group: dipyridamole was given for two days from the 1st day after operation, and usage of warfarin from the 2nd day after operation ; fraxiparine group : fraxiparine was given for two days from the 1st day after operation, and warfarin was given from the 2nd day after operation. Complication and post-operative death were observed. The value of porthrombin time (PT), international normal ratio (INR), and blood clotting factor Ⅱ , Ⅶ etc. were monitored respectively at the day before operation, the day of operation and the 3rd, 5th day after operation. Results All the results of 4 groups have met the anticoagulation requirement on the 5th day after operation (INR 1.5-2. 0), and no hemorrhage, embolism and death occurred. In warfarin group 1, level of blood clotting factor Ⅱ , Ⅶ decreased, and level of PT, INR rose continuously, level of INR was 2.13±1.14 at the 5th day after operation, 1NR of 7 cases more than 2.0 in warfarin group 1 which was significant statistically compare with the other 3 groups(P〈0. 01). Conclusions To adopt warfarin with the maintainable dose from the 2nd day after mechanical valve replacement could simplify anticoagulation method, with no increased risk of post-operative complications, such as hemorrhage and embolism. There is no need for additional anticoagulation methods.
Abstract: Objective To construct an Anticoagulation Therapy Database of Chinese Patients after Heart Valve Replacement in accordance with blood coagulation characteristics of Chinese patients, fill the gap of Chinese clinical research in valvular heart diseases, and provide a scientific and objective basic data and information exchange platform. Methods A national multicentre,prospective and cohort clinical research method was applied to establish an anticoagulation therapy database of Chinese patients after heart valve replacement, using the Internet as a platform. A case report form (CRF), which was in line with the actual situation of Chinese anticoagulation patients after heart valve surgery, was formulated through the discussion of experts from 36 cardiovascular surgery centers in China in the starting meeting of National Science amp; Technology Support Program during the Twelfth Five-year Plan Period.We planned to prospectively include patients receiving warfarin anticoagulation therapy and formal anticoagulation monitoring after heart valve replacement from January 1, 2011 to December 31, 2014. Database was constructed using warehousing technology, which allowed not only data monitoring, query and statistics, but also regular data backup and system updates. Results A network database entitled Anticoagulant Therapy Database of Chinese Patients after Heart Valve Replacement was constructed and linked with the homepage of Chinese Journal of Clinical Thoracic and Cardiovascular Surgery (http://www. zgxxwkzz. com), which constituted a national Internet information platform. From 1 January 2011 to 1 December 2012, 8 452 anticoagulation patients after heart valve replacement from 34 level-3A hospitals in China had been registered in the database. Further follow-up of these patients was being carried out in respective hospitals. Conclusion A large multi-center and open database and network information platform has been constructed. The database variables are in line with clinical characteristics of Chinese anticoagulation patients after heart valve replacement, which provide scientific and objective basic data and support for future clinical research and systemic analysis.
目的探讨广泛门静脉血栓形成(portal vein thrombosis,PVT)的诊治经验。 方法回顾性分析笔者所在医院2004年1月至2012年12月期间收治的7例广泛PVT患者的临床资料。 结果按Yerdel’s分级7例患者属Ⅲ~Ⅳ级;男4例,女3例;年龄28~54岁,中位年龄45岁;起病至就诊时间4~10 d,平均6.9 d。表现为上腹痛3例,全腹痛、腹胀4例,血便2例,休克1例,腰背痛1例,恶心、呕吐3例。查体:有腹膜炎体征3例,左下腹压痛1例,腹水征阳性3例,肠鸣音消失2例,减弱1例。2例行D-二聚体检查均升高。所有患者超声检查均提示门静脉血栓形成、累及肠系膜上静脉。给予抗凝、祛聚、溶栓等基础治疗;1例经肠系膜上动脉导管溶栓,2例手术切除坏死肠管,其中1例同时行脾切除术。1例术后发生肠瘘,经保守治疗治愈;3例患者发生门静脉高压性胃肠病,口服普萘洛尔治疗。 结论早期行血浆D-二聚体及影像学检查,尽早行抗凝治疗,无禁忌时行溶栓或介入治疗以及实时手术治疗,PVT患者可有较好的预后。
Objective To explore the role of thrombus precursor protein(TPP) in the monitoring of anticoagulation in the patients with atrial fibrillation (Af) after mechanical heart valve replacement, and suggest the reasonable anticoagulant range. Methods Ninety patients were divided into Af group (n=45), sinus rhythm group (SR group, n=45), and control group (20 patients with non-valvular heart diseases), according to whether Af exist after mitral valve replacement. TPP concentrations and International Normalized Ratio(INR) in the anticoagulant patients were analyzed. Results In patients after mechanical mitral valve replacement, plasma TPP concentrations in both SR group and Af group were lower than that in control group (Plt;0.05,0.01), their INR value were higher than that in control group (Plt;0.01), and Af group had higher plasma TPP concentrations than that in SR group((Plt;)0.05). It was found that there existed contradictions between INR and plasma TPP concentrations in Af group. There were 28 patients with plasma TPP concentrations below 6 μg/ml and without spontaneous bleeding complications in the group with Af, who might be at the optimal anticoagulant status. Their 95% confidence of INR value was 1.90-2.30 and their plasma TPP concentration was 4.29±0.75μg/ml. Conclusion Patients with Af after mechanical heart valve replacement might have higher risk of thromboembolism, INR between 1.90 - 2.30 and plasma TPP concentration between 2.84-6.00 μg/ml might be the optimal anticoagulant therapeutic range.
