  • Clinical Evidence for Palliative Medicine

    Objective We searched the Cochrane Library(Issue 3, 2005 )to identity evidence related to palliative treatment. We found that Opioids are effective for the palliation of breathlessness in terminal illness. Oral Morphine and hydromorphone is effective for cancer pain. Radiotherapy and Bisphosphonates can relief pain secondary to bone metastases. Corticosteroids can resolve malignant bowel obstruction in advanced gynaecological and gastrointestinal cancer.

  • Effects of Pulmonary Rehabilitation on Patients with Stable COPD of Different Severity

    ObjectiveTo investigate the effect of pulmonary rehabilitation on pulmonary function,perception of dyspnea and quality of life in stable COPD patients of different severity. Methods300 patients with COPD in stable stage were divided into a moderate COPD group (n=120),a severe COPD group (n=100) and a very severe COPD group (n=80). Each group was randomly subdivided into a control group and a treatment group. The treatment groups received pulmonary rehabilitation for 6 months in addition to usual care,and the control groups received usual care without pulmonary rehabilitation. Pulmonary function(FEV1),6 minute walking distance (6MWD),modified medical research council (mMRC) scale,and acute exacerbation frequency of COPD were compared before and after intervention and among groups. ResultsAfter pulmonary rehabilitation for 6 months,the quality of life score and 6MWD were significantly improved in the treatment groups with moderate,severe,very severe COPD,and the increscent of 6MWD was greatest in the severe COPD patients. The mMRC of the patients with very severe COPD improved significantly after pulmonary rehabilitation(P<0.05). Lung function before and after the intervention in three groups all showed no significant difference (P>0.05). The acute exacerbation frequency of the severe COPD patients was significantly reduced by pulmonary rehabilitation (P<0.05), while there was no significant change in the moderate and very severe groups (P>0.05). ConclusionPulmonary rehabilitation can improve exercise tolerance and quality of life of COPD patients with different severity,reduce acute exacerbation frequency in severe COPD,reduce the dyspnea degree in very severe COPD. Pulmonary rehabilitation is a cost-effective treatment for stable COPD.

  • Influence of Evidence-based Nursing Intervention on the Treatment Effect and Quality of Life in Patients with Peritoneal Dialysis

    ObjectiveTo investigate the influence of evidence-based nursing intervention on the treatment effect and quality of life in patients with peritoneal dialysis. MethodSixty-four patients who underwent peritoneal dialysis between January 2013 and December 2014 in our hospital were randomly divided into control group (n=32) and intervention group (n=32) . The control group received routine nursing and the intervention group received evidencebased nursing intervention. Urea clearance index (KT/V), serum albumin and hemoglobin levels were detected for both groups of patients before and after nursing intervention. Generic Quality of Life Inventory (GQOLI)-74 scale was used to assess patients' quality of life. ResultsAfter nursing intervention, KT/V, serum albumin and hemoglobin levels in the intervention group were significantly higher than those in the control group (P<0.05) . The scores of such items as physical function, psychological function, social function and overall evaluation in the GQOLI-74 scale in the intervention group were significantly higher than those in the control group (P<0.05) . ConclusionsEvidence-based nursing intervention can effectively improve the treatment effect and quality of life in patients with peritoneal dialysis, which is worthy of clinical popularization and application.

  • 静脉环磷酰胺冲击治疗狼疮肾炎患者生存质量调查分析

    目的:了解和分析静脉环磷酰胺冲击治疗狼疮肾炎患者的生存质量状况及主要影响因素。方法:使用中文版健康状况调查问卷简表-36量表对58例接受冲击治疗的患者进行生存质量调查。结果:狼疮肾炎冲击治疗患者量表各维度得分仍显著低于一般人群(Plt;0.01),但与普通SLE患者人群比较,量表PF、BP、GH、VT、SF等5个维度及综合评分均有显著提高(Plt;0.05);狼疮肾炎患者生存质量,特别是躯体功能,仅与环磷酰胺冲击治疗次数有明显的正相关rp=0.3593,Plt;0.01),并使患者对自身健康状况评价有明显改善(HT与冲击次数呈负相关,rs =-0.318,P=0.020),而与疾病的活动度及其它因素无明显的相关性。结论:坚持正规环磷酰胺冲击治疗,控制狼疮活动度以防止、减少对肾脏的损伤是提高狼疮肾炎患者生存质量的关键。

  • Establishment of multidisciplinary diagnosis and treatment model for breast cancer based on doctor-patient shared decision-making: a prospective randomized controlled study

    ObjectiveTo evaluate the effects of multi-disciplinary diagnosis and treatment model based on doctor-patient shared decision making on treatment outcomes, quality of life and postoperative complications of breast cancer patients. MethodsA total of 100 breast cancer patients were included in this study through a prospective randomized controlled design, and were randomly divided into control group and intervention group, with 50 patients in each group. The control group received traditional treatment mode, while the intervention group implemented a multidisciplinary treatment mode based on doctor-patient sharing decision making. The results of treatment, quality of life and postoperative complication rate were compared between the two groups. ResultsThe completion rate of adjuvant radiotherapy and chemotherapy in the intervention group was 94.0%, which was higher than that in the control group (80.0%), and the difference was statistically significant (P=0.037). The satisfaction rate of postoperative breast appearance in the intervention group was 90.0%, which was higher than that in the control group (60.0%), with statistical significance (P<0.001). There was no significant difference in grade Ⅲ/Ⅳ toxicity between the two groups (P>0.05). After treatment, the scores of patients’ quality of life in the intervention group were higher than those in the control group, and the difference was statistically significant (P<0.05). The incidence of postoperative complications in the intervention group was 6.0%, which was lower than that in the control group (22.0%), and the difference was statistically (P=0.021). ConclusionsThe application of multidisciplinary diagnosis and treatment model based on doctor-patient sharing decision-making in the treatment of breast cancer patients has significantly improved the treatment effect and quality of life, and effectively reduced the rate of postoperative complications. This model provides a new approach to the treatment of breast cancer that is more personalized, comprehensive and efficient.

  • Effect of internal limiting membrane peeling and transplantation on vision-related quality of life in refractory macular hole

    ObjectiveTo observe the effect of internal limiting membrane peeling and transplantation on vision-related quality of life in refractory macular hole.MethodsA retrospective clinical study. Thirty patients (30 eyes) with refractory macular hole diagnosed in Ophthalmology Department of The First Affiliated Hospital of Nanjing Medical University from January to December 2016 were included in this study. There were 13 males (13 eyes) and 17 females (17 eyes), with the mean age of 57.3±6.9 years. There were 15 patients (15 eyes) with large macular diameter, 12 patients (12 eyes) with high myopia macular hole, and 3 patients (3 eyes) with secondary traumatic macular hole. The BCVA examination was performed using the Snellen visual acuity chart, which was converted into logMAR visual acuity. OCT was performed to measure the macular retinal thickness (CRT), base diameter and minimum diameter of macular hole. Then, the macular hole index (MHI) was calculated. The logMAR BCVA was 1.52±0.30, MHI was 0.51±0.19. The Chinese version of visual-related quality of life scale -25 (CVRQoL-25) was used to evaluate the vision-related quality of life of patients. The CVRQoL-25 score was 57.60±7.13. All patients underwent 23G vitrectomy combined with inner limited membrane peeling and autologous ILM transplantation. The follow-up was at least 3 months after surgery. The changes of BCVA, MHI, CRT and CVRQoL-25 score before and after surgery were comparatively analyzed. Paired t test was performed to compare the measurement data before and after surgery, and Spearman rank correlation analysis was used for the correlation analysis among the parameters.ResultsAt 3 months after surgery, the hole closure was detected in 28 eyes (93.3%), not detected in 2 eyes (6.7%). The logMAR BCVA was 1.16±0.33, CRT was 161.00±15.26, and CVRQoL-25 scores was 70.83±9.77. Compared with before surgery, the BCVA (t=4.386, P=0.000) and CVRQoL-25 score (t=-5.991, P=0.000) after surgery were improved. Spearman rank correlation analysis showed that CVRQoL-25 score was negatively correlated with preoperative and postoperative logMAR BCVA (r=−0.536, −0.796; P=0.002, 0.000); positively correlated with preoperative MHI (r=0.421, P=0.020) and postoperative CRT (r=0.589, P=0.001).ConclusionInternal limiting membrane peeling and transplantation for refractory macular hole can significantly improve the vision-related quality of life and visual acuity, while achieved a high hole closure rate (93.3%).

  • Health-related quality of life in breast cancer survivors

    ObjectiveTo understand the possible influential factors and the effectiveness of interventions for quality of life in breast cancer survivors. MethodThe latest literatures about studies on quality of life in breast cancer survivors were reviewed. ResultsAlthough the cure rate and survival rate of patients with breast cancer had improved, breast cancer as a major stress event, the disease itself and treatment related adverse reactions might cause the fatigue, activity decline, anxiety, depression, and other negative emotions of patients, which seriously affected the quality of life of patients and survival prognosis. The patients’ age, socio-economic background, tumor characteristics, and treatment methods were the factors affecting their quality of life. To a certain extent, meditation, music therapy, and proper physical exercise could improve the quality of life of patients with breast cancer. ConclusionBreast cancer itself and related treatment methods may affect not only the affected organs, but also multiple dimensions of patients’ quality of life, so attention should be paid to improve quality of life in breast cancer survivors, providing with individualized interventions.

  • 对住院肺结核患者生存质量调查及影响因素分析

    目的:对住院肺结核患者的生存质量进行调查并分析其影响因素。方法:采用WHOQOL-BREF生存质量量表对80例住院肺结核患者的生存质量进行评估。结果:①结核患者的生存质量得分低于正常组(Plt;0.05)。②gt;40岁年龄组及学历低的结核患者在生理领域得分低 (Plt;0.01);经济困难患者在环境领域得分较低(Plt;0.05);藏族结核患者在社会关系领域、环境领域及总分得分明显低于汉族患者,差异有统计学意义(P均lt;0.01)。③多元回归分析发现,年龄、民族、职业、学历、体质指数及病程是影响肺结核患者生存质量的主要因素。结论:关心、重视患者,关注中老年、低学历、复治及藏族结核患者,以提高患者的生存质量。

  • The major factors which affect quality of life of patients with diabetic retinopathy

    Objective To study the major factors which affect quality of life of Chinese patients with diabetic retinopathy,and provide basis raising the patients condition of health and quality of life. Methods Quality of life of 179 patients with different stages of diabetic retinopathy and 30 healthy persons was measuerd by the medical outcomes study 36-item short form health survey(SF-36)and the brief scale of quality of life of WHO(WHOQOL-BREF),then the affectting factors were analysed by step regression. Results The major factors which affect quality of life of patients with diabetic retinopathy included:bilateral visual acuity(BVA),different category of retinopathy,course of disease,economic condition,personality,medical care and age. Conclusion The study suggests that bringing about the transformation from the biologic medical model to bio-psycho-social medical model is important to improve quality of life of the patients with diabetic retionpathy.     ObjectiveTo more comprehensive understanding the survival situation of donors after liver transplantation, which can be applied to clinical diagnosis and treatment. MethodsThe related literatures in recent years of living donor liver transplantation (LDLT) postoperative complications, quality of life, and liver regeneration were reviewed, and the donors postoperative survival situation were investigated. ResultsLDLT has become an option, It is safe and feasible for healthy adults to donate partial liver for LDLT. ConclusionsDonor postoperative survival situation is very important, and it affect the development of LDLT.To improve donors postoperative survival situation, we still need more efforts.
