  • TREATMENT OF CICATRICIAL DEFORMITY OF NECK WITH LOCAL SKIN FLAP

    In order to correct the dysfunction of head and neck with scar contracture, since 1980, sixty-two cases were undertaken the operation by using local skin flap to repair the soft tissue defect after scar resection. The skin flaps included pedicled delto-thoracic skin flap in 26 cases, cervico-thoracic skin flaps in 25 cases, cervico-shoulder flaps in 6 cases, pedicled vascularized extralong delto-thoracic skin flap in 4 cases and free parascapular flap in 1 case. Sixty cases had total survival of the flaps and 2 flaps had partial necrosis. After 1 to 10 years follow-up, the appearance and function of neck were excellent. It was suggested that grafting local skin flap was a good method to treat cicatricial deformity of neck especially using the skin flap with pedicle and vascular bundle.

  • PREVENTION AND TREATMENT OF POSTOPERATIVE COMPLICATIONS FOLLOWING SKIN SOFT TISSUE EXPANSION

    Since 1987, One hundred and fifty-four patients suffered from alopecia, neck and facial scar, and nasal defect had been treated with skin soft tissue expansion. The incidence of complication was decreased markedly, compared to previons report which was 11.7%. Two cases of this group were given up this procedure. The lessous learned from these case were as following. Strictly evaluated the case according to the indication, examined the expander carefully, improved the techniques to inbed the expander and infilled the sailine, those of which could obtain satisfactory result.

  • TISSUE FLAPS IN THE REPAIR OF WOUNDS AND CICATRICIAL DEFORMITIES

    Various tissue flaps were used in the repair of 255 cases of the wounds from severe deep burns and cicatricial deformities. The types of flaps used included: 6 kinds of myocutaneous flaps in 54 cases, 10 kinds of axial cutaneous flaps in 50cases, 7 kinds of fasciocutaneous flaps in 44 cases, pedicled subcutaneous tissues flaps in 12 cases, pedicled thin skin flaps in 54 cases, subdermal vascular networks cutaneous flaps in 38 cases, and free skin flaps with arter ialization of vein flap, retrograd island cutaneous flap with great or small saphaneous vein, in each. The survival rate from the transplantation was 99.2 per cent, and the rate of primary healing was 94.5 per cent. According to the time interval between the injury and operation and the conditions of the wounds, the patients were divided into acute, infected and selective cases, and the rate of primary healing was 93.0 per cent, 91.6 per cent and 97.9 per cent, respectively. The selection of the types of flap to be used and the attentions to be taken dueing operation were discussed.

  • SURGICAL TREATMENT OF BURN SCAP CONTRACTURE OF HEAD AND FACE IN CHILDREN

    The authors reported nine patients with burn scar contracture of head and face treated by operation. The varieties of operations ineiuded: (1) excision of the scar and primary closure of the wound; (2) excision of the scar and coverage of the wound with split or full thickness skin grafts; (3) excision of the scar and repaired by pedicled flap, and (4) skin expansion by expander, followed by excision the scar and transfer of the "more available skin flap" to the wound. According to certain characteristics of children, the choice of the time for operation, the indications of each methods, and some problems related to operation ahd been discussed.

  • EFFECTS OF ASIATICOSIDE ON CELL PROLIFERATION AND SMAD SIGNAL PATHWAYOF HYPERTROPHIC SCAR FIBROBLASTS

    Objective To investigate the effects of asiaticoside onthe proliferation and the Smad signal pathway of the hypertrophic scar fibroblasts.Methods The hypertrophic scar fibroblasts were cultured with tissue culture method. The expressions of Smad2 and Smad7 mRNA after asiaticoside treatment were determined by reverse transcriptionpolymerase chain reaction 48 hours later. Thecell cycle, the cell proliferation, the cell apoptosis and the expression of phosphorylated Smad2 and Smad7 with(experimental group) or without(control group) asiaticoside were detected with flow cytometry, immunocytochemistry and Western blot. Results Asiaticoside inhibited the hypertrophic scar fibroblasts from phase S to phase M. The Smad7 content and the expression of Smad7 mRNA were (1.33±1.26)% and (50.80±22.40)% in experimental group, and (9.15±3.36)% and (32.18±17.84)% in control group; there were significant differences between two groups (P<0.05). While the content and the mRNA expression of Smad2 had no significant difference between two groups. Conclusion Asiaticoside inhibits the scar formation through Smad signal pathway.

  • 拇指指蹼瘢痕挛缩的修复

    为了更好地恢复拇指指蹼瘢痕挛缩患者的手功能,1986年~1994年对不同原因所致拇指指蹼瘢痕挛缩64例,根据不同伤情、皮肤及拇收肌挛缩的程度,选择皮瓣转移的方法进行修复。术后用克氏针弯成形弹性装置撑开拇指指蹼或用克氏针穿过第1,2掌骨保持拇外展位。结果表明,术后应用形弹性装置撑开拇指指蹼可以保持拇指指蹼有足够宽度,防止其再次挛缩。认为,修复拇指指蹼时应根据皮肤及拇收肌挛缩的程度,采用恰当的修复方式才能取得较好治疗效果。

  • HE RESEARCH OF ASSEMBLYING ANIMAL MODELS OF KELOID EMPLOYING THE METHOD OF TISSUE ENGINEERING

    Objective To build animal models of keloid by method of tissue engineering and to discuss the feasibility of using it in clinical and lab researches. Methods Fibroblasts(FB) were isolated from keloids and cultured. The seventh and eighth generation of the cultured FBs were inoculated into the copolymers of polylactic acid and polyglycolic PLGA. After being cultured in rotatory cell culture system (RCCS)for 1 week,the FB was transplanted into athymic mice. The specimens were obtained 4 weeks and 8 weeks and examined histologically. Results All mice survived.The collagen patterns of all keloids were pressed in every specimen obtained 8 weeks. Fibrocytes andFB were observed in specimens by electronic microscope. There were abundent rough endoplasmic reticulum (RER) in FB, which indicated that FB’s capability of synthesizing and secreting collagen was preserved and the cellular characteristicwas remained. Conclusion There is a good affinity between PLGAand FB. The composition of PLGA and FB can form keloids in athymic mice,so that it deserves further researching and developing.

  • COMBINED ROTATIONAL FACENECKPOSTAURAL FLAPS TO REPAIR LARGE FACIAL SCARS

    A combined rotational flap was used to repair large scar on the face. The flap was removed from the lateral part of the neck, face and postaural region, between the zygmatic arch and clavicle. The dissection was carried out on the superfic ial of SMAS and platysmus M. Twentysix (12 males and 14 females) were reported. The age ranged from 5 to 28 years. The flap was survived completely in 19 cases. Small area at the margin of the flap was necrotic, which was reducing appeared in the postaural cular region in 6 cases. By reducing the size of the postaural cual component of the flap, necrosis never occured. Among these cases, 11 were followed up for 6 to 14 months. The results were satisfactory. The combined flap was classified as randomized flap because it had no axial and it could be used to cover a large area of skin defect. The color, thickness and quality of the flaps were all close to the normal facial skin. It was considered especially suitable for repair the large wound on the medial twothirds of the cheek.

  • TWO-STAGE OPERATION IN THE TREATMENT OF SEVERE CICATRICIAL ADHESION ON SUBMENTALTHORACIC REGION

    In order to decrease the risk of operation, complete release of scar tissue and reduce the recurrence after operation, from February 1994 to March 1997, seventy-three cases of severe cicatricical adhesion on the submental-thoracic region were treated with release and resection of scar tissue, and delay skin graft. The grafted skin was survived completely after operation. The result from the release of scar tissue of the cervical region was good. The physiological angle of submental-thoracic angle was recovered. It was thought that two-stage operation for submental-thoracic cicatricial adhesion would decrease the risk of operation and be advantageous to the release of contracted soft tissue of anterior cervical region and reduce the recurrence of contracture. The interval between the two stages of the operation was 2 to 3 days, which did not increase the rate of infection of the wound. The shortcomings including increase of pain to patient and prolong the time in hospital.
