  • Bibliometric analysis of researches about catheter-related infections from 2014 to 2018 in 10 nursing journals of Statistic Source

    ObjectiveTo explore the current status of nursing researches about catheter-related infections in recent 5 years in China, and provide reference for further research.MethodsThe China National Knowledge Infrastructure database and Wangfang database were selected to search for literature about catheter-related infections published in recent 5 years in 10 nursing journals of Statistic Source, with the terms of " catheter-related bloodstream infection” or " ventilator-associated pneumonia” or " catheter-related urinary tract infection”. Statistical analysis was conducted.ResultsA total of 216 papers were included. The number of papers was not increased year by year. In terms of the object of study, the studies on ventilator-associated pneumonia were the most, accounting for 71.3%. Only 36.5% of the studies had accurate diagnostic criteria. In terms of the type of study, experimental studies were the most (109 articles). The content was concentrated on the best practice intervention studies (149 articles). The data collection methods gave priority to active surveillance/screening, including 113 articles.ConclusionsThe present focused attention is not enough to the researches about catheter-related infections in nursing field, and rigorous design is lacking in published studies. There are only a few nursing studies about catheter-related infections with high quality and high level of evidence. The quantity and quality of nursing researches about catheter-related infections still need to be improved. Nurses should pay more attention to the control and prevention of catheter-related infections, and should improve their research capacity.

  • 临床肺部感染评分评估呼吸机相关性肺炎预后的研究

    目的 探讨临床肺部感染评分( CPIS) 对呼吸机相关性肺炎( VAP) 患者早期病情演变及预后评估的价值。方法 将42 例符合纳入标准的VAP 患者根据其预后分为存活组和死亡组, 采用方差分析及独立t 检验, 对两组患者的一般情况, VAP 起病前、起病后1 d 及5 d 的CPIS 分值进行分析比较。结果 42 例患者中, 存活18 例, 死亡24 例, 死亡率57. 14% 。存活组平均年龄明显低于死亡组[ ( 43. 0 ±14. 58) 岁比( 64. 75 ±14. 19) 岁, P lt; 0. 001] 。两组患者的平均机械通气时间均gt;10 d。存活组CPIS 分值在VAP 起病后1 d 较起病前明显升高[ ( 5. 78 ±0. 94) 分比( 3. 0 ±1. 81) 分,P lt;0. 001] , 起病后5 d 则明显下降[ ( 3. 72 ±1. 36) 分, P lt; 0. 001] ; 死亡组CPIS 分值在VAP 起病后1 d较起病前也有明显升高[ ( 6. 41 ±1. 21) 分比( 3. 75 ±1. 67) 分, P lt;0. 001] , 但起病后5 d 仍维持于较高水平[ ( 7. 08 ±1. 10) 分] 。结论 CPIS 评分在评估VAP患者病情及预后上有一定临床价值。

  • Humidification Effect of MR410 Humidification System and MR850 Humidification System on Patients with Invasive Mechanical Ventilation:A Comparative Study

    Objective To compare the humidification effect of the MR410 humidification system and MR850 humidification system in the process of mechanical ventilation. Methods Sixty-nine patients underwent mechanical ventilation were recruited and randomly assigned to a MR850 group and a MR410 group. The temperature and relative humidity at sites where tracheal intubation or incision, the absolute humidity, the sticky degree of sputum in initial three days after admission were measured. Meanwhile the number of ventilator alarms related to sputum clogging and pipeline water, incidence of ventilator associated pneumonia, duration of mechanical ventilation, and mortality were recorded. Results In the MR850 group,the temperature of inhaled gas was ( 36. 97 ±1. 57) ℃, relative humidity was ( 98. 35 ±1. 32) % , absolute humidity was ( 43. 66 ±1. 15) mg H2O/L, which were more closer to the optimal inhaled gas for human body.The MR850 humidification system was superior to the MR410 humidification system with thinner airway secretions, less pipeline water, fewer ventilator alarms, and shorter duration of mechanical ventilation. There was no significant difference in mortality between two groups. Conclusions Compared with MR410 humidification system, MR850 humidification system is more able to provide better artificial airway humidification and better clinical effect.

  • Research on motion impedance cardiography de-noising method based on two-step spectral ensemble empirical mode decomposition and canonical correlation analysis

    Impedance cardiography (ICG) is essential in evaluating cardiac function in patients with cardiovascular diseases. Aiming at the problem that the measurement of ICG signal is easily disturbed by motion artifacts, this paper introduces a de-noising method based on two-step spectral ensemble empirical mode decomposition (EEMD) and canonical correlation analysis (CCA). Firstly, the first spectral EEMD-CCA was performed between ICG and motion signals, and electrocardiogram (ECG) and motion signals, respectively. The component with the strongest correlation coefficient was set to zero to suppress the main motion artifacts. Secondly, the obtained ECG and ICG signals were subjected to a second spectral EEMD-CCA for further denoising. Lastly, the ICG signal is reconstructed using these share components. The experiment was tested on 30 subjects, and the results showed that the quality of the ICG signal is greatly improved after using the proposed denoising method, which could support the subsequent diagnosis and analysis of cardiovascular diseases.

  • A Systematic Review of Selective Oropharyngeal Decontamination for the Prevention of Ventilator-associated Pneumonia

    ObjectiveTo systematically review the effect of selective oropharyngeal decontamination (SOD) on the prevention of ventilator-associated pneumonia (VAP). MethodsWe electronically searched PubMed, EMbase, CBM, Wanfang, CNKI and Cochrane Central Register of Controlled Trials from the date of its establishment to September 2013. We also hand-searched some relevant references of included studies. Two independent reviewers screened the studies for inclusion, extracted data, and assessed trial quality. Meta-analysis was performed using the Cochrane Collaboration's RevMan 5.1 software. ResultsTen randomized controlled trials involving 2 791 patients were included. Results of meta-analysis showed that, compared with control group, SOD could reduce the incidence of VAP[RR=0.53, 95% CI (0.43, 0.65), P<0.000 01]. No statistical differences between the two groups were found for all cause mortality, average duration of receiving mechanical ventilatory assistance or length of stay in intensive care unit. No severe adverse event related to study participation was identified. ConclusionSOD can reduce the occurrence of VAP effectively and safely. Consequently, it may be considered as a good choice in the prevention of VAP.

  • Clinical features of ventilator associated pneumonia caused by Stenotrophomonas maltophilia in intensive care unit

    Objective To investigate the characteristics of ventilator associated pneumonia (VAP)caused by Stenotrophomonas maltophilia(Sm)in ICU。Methods The clinical data of 39 patients with VAP caused by Sm,from Jan 2001 to Dec 2006,were retrospectively investigated.Results In 15 kinds of antibiotics sensitivity test,all cases showed 100% resistance to 12 kinds of antibiotics except sulfamethoxazole/trimethoprim。ticarcillin/clavulanic acid and ciprofloxacin with sensitivity rate of 46.2% , 30.8% and 12.8% .respectively.92.30% of Sm VAP were CO—infected with other microorganisms and 79.5% of VAP were late-onset.The use of broad-spectrum antibiotics.especially carbapenem.and prolonged mechanical ventilation more than 7 days were risk factors for Sm VAP.Morbidity of Sm VAP was 87.2% .Conclusions Sm VAP has an important role in ICU infections with high morbidity and CO-infection rate.It should be alerted to the possibility of Sm VAP in the case of when prolonged ventilation (gt;7 days)or carbapenem is used.

  • Correlation between uric acid level and cardiovascular disease in patients with obstructive sleep apnea hypopnea syndrome

    Objective To explore the relationship between uric acid (UA) level and cardiovascular disease in patients with OSAHS and its clinical significance. Methods The electronic medical record system of the First hospital of Lanzhou University was used to collect 475 subjects who completed polysomnography (PSG) during hospitalization from January 2019 to May 2020. According to the Guidelines for the Diagnosis and Treatment of Obstructive Sleep Apnea Hypopnea Syndrome (Basic Version), the patients were divided into four group: control group [apnea-hypopnea index (AHI) <5 times/h, n=96], mild group (5≤AHI≤15 times/h, n=130), moderate group (15<AHI≤30 times/h, n=112), and severe group (AHI>30 times/h, n=137). The age, gender, body mass index (BMI), smoking history, drinking history, hypertension, diabetes mellitus, cardiovascular disease and biochemical indexes [including triglyceride, total cholesterol, high density lipoprotein cholesterol, low density lipoprotein cholesterol, glucose, UA, blood urea nitrogen (BUN), serum creatinine, lactate dehydrogenase, homocysteine], PSG indexes were observed and compared among the four groups, and the differences were compared by appropriate statistical methods. Binary logistic regression model was used to evaluate the correlation between various risk factors and cardiovascular disease. Results There were statistically significant differences in age, gender, BMI, drinking history, hypertension and cardiovascular disease among the 4 groups (P<0.05). The levels of UA and BUN in mild, moderate and severe groups were higher than those in the control group, with statistical significance (P<0.05). With the increasing of OSAHS severity, the level of UA increased. There was statistical significance in the incidence of cardiovascular disease among the four groups (P<0.05), and the highest incidence of arrhythmia was found among the four groups. And the incidence of cardiovascular disease increases with the increasing of OSAHS severity. Binary Logistic regression analysis showed that the risk factors for cardiovascular disease in OSAHS patients were age, UA and BUN (P<0.05). Conclusions The occurrence of cardiovascular disease in OSAHS patients is positively correlated with the severity of OSAHS. The level of UA can be used as an independent risk factor for cardiovascular disease in OSAHS patients. Therefore, reducing the level of UA may have positive significance for the prevention and control of the prevalence and mortality of cardiovascular disease in OSAHS patients.

  • Subclinical thyroid dysfunction and risk of atrial fibrillation: a meta-analysis

    ObjectiveTo systematically review the relationship between subclinical thyroid dysfunction and the risk of atrial fibrillation.MethodsDatabases including PubMed, EMbase, The Cochrane Library, Web of Science, CNKI, CBM, VIP and WanFang Data were electronically searched to collect cohort studies on associations between subclinical thyroid dysfunction and atrial fibrillation from inception to June 2020. Two reviewers independently screened literature, extracted data, and evaluated risk of bias of included studies. Meta-analysis was then performed using RevMan 5.3 software.ResultsA total of 11 studies involving 620 874 subjects and 19 781 cases were included. Meta-analysis showed that subclinical hypothyroidism was not associated with atrial fibrillation (adjusted RR=1.20, 95%CI 0.92 to 1.57, P=0.18) and subclinical hyperthyroidism could increase the risk of atrial fibrillation (adjusted RR=1.65, 95%CI 1.12 to 2.43, P=0.01). Subgroup analysis showed that for the community population, subclinical hypothyroidism was not associated with atrial fibrillation (adjusted RR=1.03, 95%CI 0.84 to 1.26, P=0.81); for cardiac surgery, subclinical hypothyroidism could increase the risk of atrial fibrillation (adjusted RR=2.80, 95%CI 1.51 to 5.19, P=0.001); subclinical hyperthyroidism could increase the risk of atrial fibrillation among patients with TSH≤0.1 mlU/L (adjusted RR=2.06, 95%CI 1.07 to 3.99, P=0.03) and TSH=0.1~0.44 mlU/L (adjusted RR=1.29, 95%CI 1.01 to 1.64, P=0.04). ConclusionsSubclinical hypothyroidism is not associated with atrial fibrillation and subclinical hyperthyroidism can increase the risk of atrial fibrillation. Due to limited quantity and quality of included studies, more high quality studies are needed to verify above conclusions.

  • Meta-analysis of the Relationship between IFN-γ and Psoriasis

    Objective To assess the relationship between IFN-γ and psoriasis. Methods We searched The Cochrane Library (Issue 3, 2005), MEDLINE (1996 to 2005) and the China Biological Medicine Database (1978 to 2005). The search was conducted in November 2005. The quality of included clinical controlled trials, case studies and cohort studies was evaluated independently by three reviewers. RevMan 4.2.8 software was used.  Results In total, 23 studies were included, involving 612 psoriasis patients and 441 healthy controls. All studies did not provide sufficient detail, on the random sampling and the specificity of the kits used for the analyses. Compared with the controls, the serum or plasma IFN-γ in psoriasis patients showed significantly higher levels (SMD=0.89, 95%CI 0.29 to 1.48; and RR=6.20, 95%CI 1.78 to 21.61). The concentration of IFN-γ in supernatant obtained from cultured cells showed slightly higher levels (SMD=0.99, 95%CI -0.01 to 1.99; and RR=5.54, 95%CI 2.03 to 15.13). Conclusion The evidence currently available shows that the increase of IFN-γ may be relevant to psoriasis. However, these results could be affected by the high risk of selection, confounding and detection bias of included studies. More persuasive evidence, from high quality studies, is needed.

  • 重症监护病房呼吸机相关性事件监测结果与分析

    目的了解重症监护病房(ICU)呼吸机相关性事件(VAE)的发生状况,为制定干预措施提供参考依据。 方法采用前瞻性监测方法,对2013年4月1日-7月31日所有入住ICU且使用机械通气时间>2 d、年龄≥18岁的患者进行VAE监测,并与2012年同期呼吸机和相关性肺炎(VAP)监测情况进行比较。 结果监测到有效病例31例,共发生VAE 6例,发生率为19.35%;患者平均住院时间12 d,平均机械通气时间10 d;预后:存活25例,死亡6例,病死率为19.35%;抗菌药物使用率100%。2013年的VAE监测与2012年同期院内VAP目标性监测VAP千日感染率分别为2.83、29.57例/1 000导管日,差异有统计学意义(P<0.05)。 结论VAE监测及其诊断标准客观量化、简单易行,但是否适用于我国ICU患者VAP的诊断,值得商榷。
