  • Analysis of the Clinical Factors Related to Bone Metastasis of Cervical Cancer

    目的 探讨宫颈癌骨转移相关因素。 方法 回顾分析2008年6月-2011年8月收治的352例宫颈癌患者的临床资料,其中鳞癌326例,腺癌26例;临床分期Ⅰ期60例、Ⅱ期184例、Ⅲ期90例、Ⅳ期18例。比较不同期别、不同病理类型、不同组织分级患者的骨转移情况。 结果 352例宫颈癌中有18例发现骨转移,转移率为5.1%;转移时间为3~48个月,2例于骨转移后1年内死亡。鳞癌326例,骨转移率为5.2%;腺癌26例,骨转移率为3.8%。Ⅰ、Ⅱ、Ⅲ和Ⅳ期患者的骨转移率分别为0.0%、3.8%、5.6%和33.3%,晚期与早期相比有统计学意义(P<0.05);高、中和低分化患者骨转移率分别为3.1%、3.1%和6.3%,高分化与中分化相比,差异无统计学意义(P>0.05),低分化与高中分化相比差异有统计学意义(P<0.05)。 结论 宫颈癌骨转移与宫颈癌临床分期、病理类型、细胞分级密切相关。在宫颈癌的治疗过程中,做到早发现、早治疗,可提高患者的治疗效果,延长生存时间。

  • HEPATIC LOBECTOMY AND SEGMENTAL RESECTION OF LIVER FOR INTRAHEPATIC BILE DUCT STONE

    Four hundred and eighty two paients suffering from intrahepatic bile duct stone undergoing lobectomy and segmental resection (from 1975 to 1994,9) has reported. 63% of the patient in this group underwent 1-5 operations, including different types of biliary-intestinal anastomosis (21.6%). 482 cases underwent different types of hepatectomy, including left lateral-lobetomy 321 cases (66.6%),left hemihepatectomy 80 cases(16.6%), right hemihepatectomy 19 cases (3.9%), and multiple segmental resections 39 cases (8.1%, including Ⅴ+Ⅷ 11 cases, Ⅵ+Ⅶ 28 cases). Other type hepatectomy combined with guadrate lobectomy 20 cases (4.1%). Postoperative complication rate was 10.2%, including diliary fistula. hemobilia and subdiaphragmatic and resectional surface infectioin, 85% of the patients were followed up with an excellent result of 88%. The authors emphsize that hepatic lobectomy nad segmental resection is the core of treatment and selection of operative methods depends on clinical-patholigic types of the disease.

  • Analysis of the Clinical Pathway and Pathologic Features of 224 Cases of Renal Biopsy

    ObjectiveTo analyze the clinical manifestations and pathological patterns of renal diseases requiring percutaneous renopuncture, evaluate the clinical significance of renal biopsy and the value of clinical pathway for renal biopsy. MethodsWe retrospectively summarized and analyzed the clinical and pathological data, and the clinical pathway implementation of 224 patients who underwent renal biopsy between October 2009 and September 2014. ResultsIn the 224 patients, there were 62 cases of IgA nephropathy (27.68%), 50 cases of minimal change nephropathy (22.32%), 28 cases of lupus nephritis (12.5%), 26 cases of membrane nephropathy (11.6%), 26 cases of mesangial proliferative glomerulonephritis (11.6%), 6 cases of purpura nephritis (2.68%), 4 cases of focal segmental glomerular sclerosis (1.79%), 4 cases of hepatitis B virus-associated membrane nephropathy (1.79%), 4 cases of nodular diabetic glomerulosclerosis (1.79%), 4 cases of acute tubulointerstitial nephropathy (1.79%), 2 cases of hypertensive renal damage (0.89%), 2 cases of membrano-proliferative glomerulonephritis (0.89%), 1 case of lipoprotein kidney disease (0.45%), and 1 case of fibrillary glomerulopathy (0.45%). A total of 220 specimens in the 224 cases were qualified, accounting for 98.21%. Diagnosis of 70 patients in the qualified 220 cases were re-corrected according to their renal pathology reports, accounting for 31.81%. In the 224 cases, there were 16 cases of gross hematuria (7.14%) and 24 of peri-renal hematoma (10.71%) after renal biopsy. Patients who met the requirement of clinical pathway were divided into clinical pathway group and control group randomly. Average hospitalization time of the clinical pathway group was (7.6±1.2) days, and the average cost was (5 860±237) yuan, both lower than the control group [(11.8±2.3) days, (7 658±360) yuan)]. The difference was statistically significant. ConclusionsIgA nephropathy is the most common pathological type of primary glomerular diseases, and minimal change nephropathy the second. Lupus nephritis, membranous nephropathy, mesangial proliferative glomerulonephritis are still the most common types of glomerular diseases. Lupus nephritis becomes the first secondary glomerular disease. Ultrasound guided percutaneous renal biopsy is safe and has high success rate and high clinical application value. The implementation of clinical pathway can shorten the average length of hospital stay and reduce the average hospital cost.

  • Clinical Pathology Research on Perineural Invasion in Hilar Cholangiocarcinoma

    ObjectiveTo summarize the incidence, patterns and laws of perineural invasion, and explore the path and the influencing factors of perineural invasion in hilar cholangiocarcinoma. MethodsA clinicopathologic study was conducted on sections from 52 patients with hilar cholangiocarcinoma to summarize the incidence and patterns of perineural invasion. The relationship of perineural invasion to lymph node metastasis, serum CA19-9, CEA, total bilirubin (TBIL) level, Bismuth-Corllet classification, or tumor penetration depth of bile duct walls was analyzed by association analysis. ResultsThe overall incidence of perineural invasion was 90.38% (47/52). However, the incidences of perineural invasion had no significant differences among various differentiated adenocarcinoma groups (P > 0.05). The incidences of perineural invasion were not correlated with the lymph node metastasis, serum CA19-9, CEA, TBIL level, and Bismuth-Corlette classification (P > 0.05), which was correlated with the tumor penetration depth of bile duct walls (P < 0.01). There were four patterns of perineural invasion, sequenced them according their incidences from high to low as follows: typeⅡ> typeⅢ> typeⅣ> typeⅠ. The pattern of perineural invasion was correlated with the degree of tumor differentiation (χ2=31.04, P < 0.01). ConclusionsThe incidence of perineural invasion is very high in hilar cholangiocarcinoma. The patterns of perineural invasion are similar in the same patient, and a variety of invasion patterns might coexist. While the pattern of perineural invasion is correlated with the degree of tumor differentiation. The incidence of perineural invasion is correlated with the tumor penetration depth of bile duct walls.

  • Epidemiological and pathological distribution characteristics in 23 228 lung cancer patients

    ObjectiveTo understand variations in clinical epidemiology of lung cancer and to help with early accurate diagnosis and treatment of lung cancer.MethodsThis research was based on the Lung Cancer Database of West China Hospital of Sichuan University. Clinicopathological data of inpatients with primary lung cancer from 2011 to 2018 was collected for analysis. Information regarding sex, age, smoking history, surgical resection modalities, TNM stage of tumor, and pathological subtypes was included.ResultsA total of 23 228 inpatients with primary lung cancer were included in this research. There were 1 932, 2 029, 2 162, 2 773, 2 975, 3 318, 3 993, and 4 046 patients in every single year from 2011 to 2018, respectively. The male-to-female ratio of patients had changed significantly from 2.00∶1 in 2011 to 1.34∶1 in 2018 (P<0.001). Statistically significant difference was found in age distribution of patients in different years, while elderly always accounted for the majority of all patients. More nonsmokers were among the group of lung cancer patients, the proportion of whom rose from 41.61% in 2011 to 52.47% in 2018 (P<0.001). The proportion of adenocarcinoma in all subtypes rose from 54.04% in 2011 to 71.73% in 2018, while the proportion of squamous carcinoma dropped from 29.04% to 17.67% simultaneously. From 2011 to 2018, the proportion of stage Ⅰ lung cancer rose from 15.68% to 40.79%. Patients with stage ⅠA1 accounted for 11.82% in 2018. In 2018, 60.78% of stage ⅠA patients had gone through surgical resection modalities, forming a stark contrast with the fact that only 26.48% of stage ⅠA patients adopted surgeries in 2011.ConclusionsFrom 2011 to 2018, the total number of lung cancer patients increased year by year. The proportions of female patients and nonsmokers rose significantly. Adenocarcinoma accounted for the highest proportion of all subtypes, followed by squamous cell carcinoma. There were more patients with early lung cancer and less with advanced lung cancer diagnosed, implying a gradual accumulation of weight on the early stages according to the TNM classification of lung cancer.

  • 203例原发性视网膜色素变性遗传类型及并发症分析

    原发性视网膜色素变性 (RP)为遗传性眼病,其遗传类型分常染色体隐性遗传(AR),常染色体显遗传(AD)和性连锁隐性遗传(XR),但散发病例亦不少见.本文对203例中日RP患者的遗传类型及并发症进行了总结分析.其中中国患者134例,AR占17.91%,AD占21.64%,XR占2.24%.散发者占58.21%.日本患者69例,AR占28.99%,AD占1.45%,XR占4.35%,散发者占65.21%.其发病年龄中位数为17.4岁,XR和AD发病较AR及散发者早.关于RP的并发症以并发白内障(54.68%)、玻璃体混浊(43.84%)最常见,并发屈光不正次之(21.18%).亦可见有并发视网膜脱离者。 (中华眼底病杂志,1993,9:29-30)

  • The effect of epileptic seizure on the empathy ability of patients with epilepsy

    Objective To explore the damage, damage model and influence factors of the empathy ability on patients with epilepsy by the Chinese version of interpersonal reaction index scale (IRI-C). Methods Eighty-eight non-symptomatic epilepsy patients who were from the First Affiliated Hospital of Dalian Medical University and 100 healthy controls were included in the study from March 2015 to January 2016. Patients with serious cognitive impairment and severe anxiety or depression were ruled out through neural psychology background screening (Montreal Cognitive Assessment, Hamilton anxiety and depression scale) due to their influence on empathy scores. Thereafter the empathy ability was compared between epilepsy patients and healthy controls using the IRI-C, and the effect of seizure type on empathy ability was analyzed. According to the epileptic seizure types of generalized tonic-clonic seizure (GTCS), patients were divided into 3 groups: non-GTCS group (simple or complex partial seizure, n=17), pure GTCS group (only primary GTCS attack, n=23) and SGS group (partial onset secondary GTCS,n=48). The control group included 100 healthy participants. In order to ensure the balance of sample size, 30 samples are randomly selected from SGS and control groups respectively for statistical analysis. Then patients were divided in two groups according to whether he/she has complex partial seizure (CPS), and 30 patients in each group. Statistical analysis was performed using SPSS18.0 software package. Results ① The total IRI-C Empathy scores, Cognitive empathy (CE) and two factors of the patients in epilepsy group were lower than those in control group (P<0.05), while there was no statistical difference in Emotional Empathy (EE) and two factors between groups (P>0.05). ②ANOVA showed the points of CE (include two factors) and total scores of empathy mean different between the four groups (P<0.05), and EE (include two factors) scores were no statistically significant differences between the four groups (P>0.05). The CE scores were differences between the GTCS group, pure GTCS group and SGS group. The scores of the perspective-taking was statistically significant differences (P<0.05) between the pure GTCS group and the SGS group. The scores of perspective-taking of SGS group were lower than the other groups (P<0.05) . In other words, the scores of the perspective-taking of SGS group were lower than those of the other three groups, and the CE scores were lower than the GTCS group and the control group. ③ The opinion selection, imagination, CE and total empathy score of CPS group were all worse than that of non-CPS group (P<0.05). Conclusions The empathy ability of epilepsy patientswere impaired in CE, but reserved in EE Epileptic subtype CPS and GTCS were risk factors of the decline in empathy ability in epilepsy patients.

  • Relation Between Histology Type and Prognosis of Gastric Cancer

    ObjectiveTo investigate the relation between histology type and prognosis of gastric cancer.MethodsThe clinical database of 311 patients who underwent surgical resection for gastric cancer in our hospital, between 2000 and 2004, was retrospectively reviewed and analyzed with SPSS 13.0. ResultsLymph node metastasis, tumor invasion depth, and Borrman type of gastric cancer were related with histology type (χ2 test, Plt;0.05). KaplanMeier survival analysis was used to compare the survival rate and showed that there was a significant difference between highly/moderately differentiated cancers and the poorly differentiated (Plt;0.05), while there was no statistical difference between those patients with same invasion depth (Pgt;0.05). ConclusionThe prognosis of gastric cancers with same invasion depth is same regardless of their histology types.

  • Research progress on three-dimensional printed interbody fusion cage

    Spinal fusion is a standard operation for treating moderate and severe intervertebral disc diseases. In recent years, the proportion of three-dimensional printing interbody fusion cage in spinal fusion surgery has gradually increased. In this paper, the research progress of molding technology and materials used in three-dimensional printing interbody fusion cage at present is summarized. Then, according to structure layout, three-dimensional printing interbody fusion cages are classified into five types: solid-porous-solid (SPS) type, solid-porous-frame (SPF) type, frame-porous-frame (FPF) type, whole porous cage (WPC) type and others. The optimization process of three-dimensional printing interbody fusion cage and the advantages and disadvantages of each type are analyzed and summarized in depth. The clinical application of various types of 3D printed interbody fusion cage was introduced and summarized later. Lastly, combined with the latest research progress and achievements, the future research direction of three-dimensional printing interbody fusion cage in molding technology, application materials and coating materials is prospected in order to provide some reference for scholars engaged in interbody fusion cage research and application.

  • Relationship between ABO blood type of patients with colorectal cancer and surgical characteristics and postoperative complications: a real world study based on DACCA

    Objective To analyze the influence of the ABO blood types of colorectal cancer patients served by West China Hospital as a regional center on surgical characteristics and postoperative complications in the current version of Database from Colorectal Cancer (DACCA). Methods The DACCA version was updated on January 5, 2022. The data items included ABO blood type, sex, type of operation, nature of operation and postoperative complications. The operative characteristics and complications at different stages after operation (in hospital, short-term and long-term after operation) of colorectal cancer patients with different blood types (A, B, AB, O) were analyzed. Results According to the DACCA database, we obtained 5 010 analysable data rows, covering 2005–2022. The results of blood types analysis showed that there was no significant difference among different blood types in the overall postoperative complications and the occurrence of complications in hospital, short-term and long-term after operation (P>0.05). Further subgroup analysis showed that only the difference of anastomotic leakage among different blood types was statistically significant (χ2=9.588, P=0.022). There was no significant difference among different blood types in whether the primary focus of colon cancer surgery was removed or not, the degree of radical resection of the primary focus, and whether the anus was preserved or not in rectal cancer surgery (P>0.05), and there was significant difference among different blood types with different degrees of radical resection of primary rectal cancer (χ2=15.773, P=0.001). Conclusions The ABO blood types of patients with colorectal cancer has nothing to do with the occurrence of overall complications in the short and long term after operation, and has no impact on the implementation of different surgical methods. However, the occurrence of a single postoperative anastomotic leakage is related to blood type, and its possible causes need to be further explored.
