目的探讨保留幽门的胰十二指肠切除术(PPPD)的手术适应证、捆绑式胰肠吻合术的特点及胃排空障碍的治疗。方法12例均行PPPD,消化道重建按Child法,胰肠吻合均采用捆绑式胰肠吻合。结果除1例术后第21天并发切口裂开、肺栓塞死亡外,其余11例均顺利出院,平均住院时间24 d,随访1年,1例术后8个月死亡,余均健在。结论PPPD是治疗胰头癌及壶腹周围癌的一种具有广阔应用前景的术式,捆绑式胰肠吻合有望作为胰肠吻合的标准术式。
目的 探讨手助腹腔镜胰十二指肠切除术的技术路线。方法 笔者所在科室于2011年10月17日完成1例手助腹腔镜胰十二指肠切除术。术中采用术者双侧站位、左右侧对称戳孔的策略,站立于患者右侧,游离胃网膜左血管和胃左血管,包括清扫No.7、No.8和No.9淋巴结;站立于患者左侧,游离十二指肠环和清扫下腔静脉旁淋巴结。经手助切口直视下完成消化道重建。结果 上腹部正中手术切口长7cm;手术时间为420min;术中出血量约600ml。术后病理报告:送检胃、十二指肠和胰腺标本,十二指肠球部低级别神经内分泌癌,浸润至深肌层,胃及胰腺未受累;两端切缘未见癌细胞,肝十二指肠韧带淋巴结未见癌转移(0/2);慢性胆囊炎。手术后患者生命体征平稳,术后第5天肛门排气,第7天排便。术后发生腹腔积液并感染,经保守治疗治愈。术后28d出院。结论 术者双侧站位、左右对称戳孔是手助腹腔镜胰十二指肠切除术的新模式,安全、可行、微创,值得进一步探索。
目的:探讨胰十二指肠切除术后胰瘘的原因及其预防。方法: 2003年1月至今,对46例行胰十二指肠切除术中采取胰管空肠吻合方式的病例资料进行回顾分析。结果: 46例患者行胰十二指肠切除术后无一例发生胰瘘。结论: 胰十二指肠切除术采取胰管空肠吻合方式可有效预防胰瘘的发生。
目的 介绍保留十二指肠乳头的胰十二指肠切除术。 方法 对2例重型十二指肠合并胰腺损伤患者行保留十二指肠乳头的胰十二指肠切除术。结果 无手术死亡,术后无胰瘘等严重并发症,均痊愈出院。结论 对胆总管完整、十二指肠乳头尚存的重型胰十二指肠损伤患者,急诊手术行保留十二指肠乳头的胰十二指肠切除是值得考虑和应用的安全手术方法。
Objective To explore the clinical value of the improved style of pancreatodeodenectomy. Methods Retrospective analysis the data of 111 cases of pancreatodeodenectomy. Forty-one cases of 111 cases were performed the modified Whipple pancreatic jejunal anastomosis, which reconstruction residual pancreatic duct jejunum into the intestinal mucosa sets of accurate end to side anastomosis type (modified group). Another 70 cases were performed the conventional Whipple pancreatic jejunal anastomosis, which classic lines set into the pancreas jejunum anastomosis (conventional group). The incidence rate of pancreatic fistula after operation were compared in two groups. Results The postoperative recovery in modified group was smooth, and there was no case of pancreatic fistula. Thirteen cases (18.57%) had pancreatic fistula in conventional group. The difference of incidence rate of pancreatic fistula between two groups was statistically significant (P<0.05). The difference in other complications such as gastrointestinal bleeding, delayed gastric emptying, biliary fistula, abdominal infection, lung infection, and wound infection were no statistically significant (P>0.05), and the difference of survival rate was also no statistically significant (P>0.05) in two groups. Conclusions Pancreatic duct jejunum end to side into the mucous membrane of the mucosal anastomosis sets of pancreatodeodenectomy can significantly prevent pancreatic fistula, it is worth to promote the use in clinical work.
目的 研究胰腺-胃吻合术在胰十二指肠切除术消化道重建中的合理性。方法 对2002年3月至2004年10月期间实施胰十二指肠切除和胰腺-胃吻合术重建消化道的24例壶腹周围癌患者的临床资料进行回顾性分析。结果 所有患者均顺利完成胰腺-胃吻合手术,胰瘘的发生率为8.3%(2/24),通过保守治疗均痊愈。结论 胰腺-胃吻合术是胰十二指肠切除术后一种安全的消化道重建方法。
ObjectiveTo investigate value of drainage fluid amylase content and other risk factors in predicting clinically relevant postoperative pancreatic fistula (CR-POPF) after laparoscopic pancreaticoduodenectomy (LPD).MethodsThe clinical data of 166 patients who underwent LPD in this hospital from January 2017 to December 2019 were retrospectively analyzed. The independent risk factors of CR-POPF after LPD were analyzed. And the sensitivity and specificity of drainage fluid amylase content obtained on postoperative day 1 and 3 (Abbreviated as DFA1 and DFA3, respectively) in predicting CR-POPF by receiver operating characteristic (ROC) curve.ResultsA total of 166 patients underwent LPD were collected in this study. The CR-POPF occurred in 16 (9.6%) patients and all of them were grade B. The DFA1 (P=0.037), DFA3 (P<0.001), and positive bacterial culture of drainage fluid after operation (P=0.020) were the independent risk factors of CR-POPF after LPD (P<0.05) by the logistic regression multivariate analysis. The area under the ROC curve of the DFA1 and DFA3 in predicting CR-POPF was 0.880 [95%CI (0.812, 0.949)] and 0.912 [95%CI (0.853, 0.971)] respectively, and the corresponding best critical value was 3 925 and 939 U/L, and the sensitivity was 100% and 100%, specificity was 64.0% and 70.7%, respectively.ConclusionFor patients with DFA1>3 925 U/L, DFA3>939 U/L, and positive bacterial culture of drainage fluid after operation, preventive measures should be made to avoid CR-POPF.
