  • Risk factors of persistent cough after pneumonectomy: A systematic review and meta-analysis

    ObjectiveTo systematically evaluate the risk factors for persistent cough after lung resection, providing a theoretical basis for preventing persistent postoperative cough. MethodsThe Cochrane Library, Web of Science, EMbase, PubMed, Chinese Biomedical Literature Database, Wanfang, CNKI, and VIP databases were searched for studies related to risk factors for persistent cough after lung resection. The search period was from database inception to March 30, 2023. Two researchers independently screened the literature, extracted data, and performed quality assessment. RevMan 5.3 software was used for meta-analysis. ResultsA total of 17 articles with 3 698 patients were included. Meta-analysis results showed that females [OR=3.10, 95%CI (1.99, 4.81), P<0.001], age [OR=1.72, 95%CI (1.33, 2.21), P<0.001], right-sided lung surgery [OR=2.36, 95%CI (1.80, 3.10), P<0.001], lobectomy [OR=3.40, 95%CI (2.47, 4.68), P<0.001], upper lobectomy [OR=8.19, 95%CI (3.87, 17.36), P<0.001], lymph node dissection [OR=3.59, 95%CI (2.72, 4.72), P<0.001], bronchial stump closure method [OR=5.19, 95%CI (1.79, 16.07), P=0.002], and postoperative gastric acid reflux [OR=6.24, 95%CI (3.27, 11.91), P<0.001] were risk factors for persistent cough after lung resection, while smoking history was a protective factor against postoperative cough [OR=0.59, 95%CI (0.45, 0.77), P<0.001]. In addition, the quality of life score of patients with postoperative cough decreased compared with that before surgery [MD=1.50, 95%CI (0.14, 2.86), P=0.03]. ConclusionCurrent evidence suggests that females, age, right-sided lung surgery, lobectomy, upper lobectomy, lymph node dissection, bronchial stump closure method (stapler closure), and postoperative gastric acid reflux are independent risk factors for persistent postoperative cough in lung resection patients, while smoking history may be a protective factor against postoperative cough. This provides evidence-based information for clinical medical staff on how to prevent and reduce persistent postoperative cough in patients and improve their quality of life in the future.

  • Clinical value of routine chest X-ray after thoracoscopic lung resection: A retrospective cohort study

    ObjectiveTo explore the necessity of routine X-ray examination after lung surgery based on patient symptom burden. MethodsA retrospective study was conducted on the patients who underwent thoracoscopic lung resection at the Department of Thoracic Surgery of Guangdong Provincial People's Hospital from March 2020 to April 2023. The routine chest X-ray examination results and symptom burden of postoperative patients were analyzed. Symptom burden was evaluated using the Perioperative Symptom Assessment Lung inventory. Results A total of 2 101 patients were collected, including 915 males and 1 100 femals, with a median age of 56 years. Among patients who underwent routine postoperative chest X-ray, only 1.0% patients accepted intervention. Among patients who had chest X-ray after chest tube removal, only 0.5% of them needed intervention. Among patients who had chest X-ray one month after discharge, only 1.3% of them required intervention. The intervention group had significantly worse shortness of breath (3 points vs. 2 points, P=0.015), pain (2 points vs. 1 point, P=0.039), and disturbed sleep (3 points vs. 2 points, P=0.036) compared with the normal group. Conclusion Very few routine postoperative chest X-ray examinations change patients’ management, and patients who need extra-intervention tended to have more severe symptom burden after surgery.

    目的 为行肺、食管等开胸术时保留肋骨,以保持胸廓的完整性.方法 采用"Z"字形断肋的方法行肺手术10例,食管手术4例.结果 全组病例术后恢复良好,切口疼痛明显减轻.胸部X线片示:各肋骨排列完整,断肋对合好.均痊愈出院.结论 该术式开胸时可快捷进胸、损伤小、出血少、切口暴露好;关胸时断肋对合严密,不易松脱;操作简单、方便、易掌握.

  • 电视胸腔镜肺切除的手术方法

    目的 为了评价电视胸腔镜肺切除术在肺部肿瘤治疗中的可行性,而对其手术方法和治疗原则进行探讨. 方法 回顾性分析1992年10月至2000年3月,127例胸腔镜肺切除术患者的临床资料,其中肺楔形切除术71例,肺叶切除术50例,全肺切除术6例. 结果 全组无手术死亡及严重并发症, 手术时间、引流时间、住院时间均明显缩短.全组平均胸腔引流时间2.4天,平均住院天数10.6天. 结论 胸腔镜肺切除术是安全可行的,只要严格掌握手术适应证,运用合理的手术方法和技巧,一般可以达到与常规开胸手术同样的效果.

  • Coopdech支气管封堵器用于全肺切除术后对侧胸腔内手术三例

    目的探讨Coopdech支气管封堵器用于全肺切除术后对侧胸腔内手术麻醉的可行性及疗效。 方法回顾性分析上海市肺科医院2012年1~12月3例全肺切除术后患者需要进行对侧胸腔内手术时,在纤维支气管镜引导下置入Coopdech支气管封堵器隔离目标肺叶,进行选择性肺叶通气麻醉。比较3例患者封堵前后生命体征和血气分析。 结果3例患者均顺利完成手术,术中调整呼吸参数(封堵后,减少潮气量、增加呼吸频率)维持脉搏血氧饱和度(SpO2)大于96%、动脉血氧分压(PaO2)大于75 mm Hg、动脉血二氧化碳分压(PaCO2)35~45 mm Hg、pH值7.32~7.40,术中目标肺叶萎陷良好,给外科医生提供了良好的手术视野,术毕患者清醒拔管回ICU。 结论Coopdech支气管封堵器可以用于全肺切除术后对侧胸腔内手术的麻醉。

  • Clinical analysis of the feasibility and safety of single utility port robot-assisted lung resection

    ObjectiveTo investigate the feasibility and safety of single utility port Da Vinci robot-assisted lung resection via anterior approach.MethodsThe clinical data of 21 patients who underwent single utility port Da Vinci robot-assisted lung resection from February to March 2021 were retrospectively analyzed. There were 10 males and 11 females, with a median age of 50 (34-66) years. The operation time, blood loss, postoperative hospitalization time, postoperative complications and other indicators were analyzed.ResultsAll patients completed the operation successfully with no transition to thoracotomy or perioperative death. Overall surgery time was 103 (70-200) min, Docking time was 5 (3-10) min, operation time was 81 (65-190) min. The blood loss was 45 (20-300) mL. All patients had malignant tumors, the number of dissected lymph node station was 3 (1-6), and the number of lymph nodes was 5 (2-16). The postoperative indwelling time was 3 (2-5) d. The postoperative hospitalization time was 5 (3-7) d. The pain score for the first 3 days after surgery was 3±1 points.ConclusionSingle utility port robot-assisted lung resection via anterior approach is safe, less traumatic, more convenient and effective, which can be gradually promoted and applied to clinical trials.

  • Progress of Thoracoscopic Pulmonary Segmentectomy for Early-Stage Non-small Cell Lung Cancer

    Abstract: The principles of 2010 National Comprehensive Cancer Network(NCCN) clinical practice guidelines in non-small cell lung cancer address that anatomic pulmonary resection is preferred for the majority of patients with non-small cell lung cancer and video-assisted thoracic surgery (VATS) is a reasonable and acceptable approach for patients with no anatomic or surgical contraindications. By reviewing the literatures on general treatment, pulmonary segmentectomy, pulmonary function reserve, and the anatomic issue of early stage non-small cell lung cancer surgery, the feasibility and reliability of thoracoscopic pulmonary segmentectomy are showed.

  • Progress on three-dimensional visualization technology in precise lung resection

    Three-dimensional (3D) visualization technology can well characterize lung nodules, accurately locate lung nodules, accurately identify lung anatomical structures, shorten operation time, reduce intraoperative and postoperative complications, and make thoracoscopic precise lung resection safer and more efficient. However, the mastery of 3D reconstruction technology in some hospitals still needs to be improved. Due to the time and economic cost of 3D printing, the development of this technology is restricted. With the application and improvement of 3D visualization technology in more centers in the future, the development of precise lung resection will be more extensive. This article reviewed the progress on 3D visualization technology in thoracoscopic precise lung resection.

  • Upper left lung cancer with congenital complete left pericardial defect: A case report

    A 54-year-old asymptomatic man underwent a video-assisted thoracoscopic left pneumonectomy for squamous-cell carcinoma. During the surgery, a complete left pericardial defect was unexpectedly discovered, but no special intervention was made. The preoperative chest CT was reciewed, which showed the heart extended unusually to the left, but the left pericardial defect was not evident. The operation time was 204 min and the patient was discharged from hospital upon recovery 9 days after the surgery. The pathological result indicated moderately differentiated squamous-cell carcinoma (T2N1M0, stage ⅡB), and metastasis was found in the parabronchial lymph nodes (3/5). The patient did not receive chemotherapy after the surgery, and there was no signs of recurrence 6 months after the surgery. Complete pericardial defects usually do not endanger the lives of patients, and if the patient is asymptomatic, pneumonectomy is feasible.

  • Influence of Perioperative Nursing Intervention on Pulmonary Tuberculosis Patients Undergoing Lung Resection

    ObjectiveTo observe the impact of perioperative nursing intervention on pulmonary tuberculosis patients undergoing lung resection. MethodThirty-six pulmonary tuberculosis patients going to undergo lung resection hospitalized between January and December 2013 were randomly divided into intervention group and control group with 18 in each. Patients in the intervention group received routine perioperative care plus nursing intervention including preoperative, environmental, dieting and pain intervention. Then, complications, hospitalization time and costs, as well as patients' satisfaction with the nursing work were compared between the two groups. ResultsAfter nursing intervention, complications, hospitalization time, hospital costs of patients in the intervention group were significantly less than the control group (P<0.05), and patients' satisfaction was significantly higher (P<0.05). ConclusionsPerioperative nursing intervention on lung resection treatment and rehabilitation of patients play an active and effective role, which can reduce complications, shorten hospitalization time, reduce hospital costs and improve patients' satisfaction.
