  • Efficacy and safety of mechanical thrombectomy in cerebral venous sinus thrombosis: a literature review

    ObjectivesTo review the efficacy and safety of mechanical thrombectomy in patients with cerebral venous sinus thrombosis (CVST).MethodsWe searched The Cochrane Library, PubMed, EBSCO, Web of Science, CBM, CNKI and VIP databases to collect studies on mechanical thrombectomy in CVST patients from inception to April, 2018. Two reviewers independently screened literature, extracted the data and qualitative analysis of the included studies.ResultsA total of 33 studies including 552 patients were included for data analysis. Specifically, 157 (30%) patients had a focal neurological deficit, 145 (28%) patients had a pretreatment intracerebral hemorrhage or infarct, and 152 (29%) patients were stuporous or comatose. Wire was the most commonly used device. Overall, 483 (88%) patients had good outcome, while 35 (6%) patients deceased. Moreover, 333 (65%) patients had complete recanalization, 148 (29%) patients had partial recanalization, and 14 (2.5%) patients had worsen or new intracranial hemorrhage.Conclusions The current evidence suggests that mechanical thrombectomy is reasonably safe in the majority of cases. Due to limited quality and quantity of included studies, more high quality studies are required to verify above studies.

  • RESEARCH OF DEEP VENOUS THROMBOSIS STAGE BY COMPARING LENGTH AND WEIGHT OF RAT INFERIOR VENA CAVA THROMBUS

    ObjectiveTo understand the initiation, maturing, and resolution of thrombus by comparing the length and weight of thrombus at different time in the rat inferior vena cava (IVC) stenosis model. MethodsForty-eight female Sprague Dawley rats, weighing 180-230 g, were selected to prepare the IVC stenosis model by blocking the most of the IVC blood flow. The length and weight of IVC thrombus were measured at different time points, the histological features were observed via HE staining. ResultsBlood clots formed after 2 hours of modeling, the thrombus length and weight showed no significant difference between at 2 hours and 4 hours after modeling (P>0.05). The thrombus length and weight increased significantly at 6 hours, showing significant differences between at 2 and 4 hours and at 6, 8, 12, 24, and 48 hours (P<0.05); and from 6 to 48 hours, there was no significant difference in the thrombus length and weight (P>0.05), indicating that thrombus was stable, or maturing. Blood clots began to become smaller after 3 days when compared with ones at 48 hours (P<0.05), indicating the start of resolution at 3 days. At 7 days, the thrombus length and weight became further smaller when compared with ones at 3 days (P<0.05). The thrombus completely subsided at 21 days, the IVC recanalized. HE staining showed that thrombus formed after 2 hours of modeling; from 6 to 48 hours, the lumen became hyperemia, and the inflammatory cells, especially neutrophils, could be found. The organization of thrombus could be observed at 3 days and 7 days; thrombus gradually vanished at 21 days. ConclusionThe time of thrombus initiation, maturing stage, and resolution stage is 6 hours, 6 to 48 hours, and 3 to 21 days after modeling, respectively.

  • Chronic thromboembolic pulmonary hypertension: Outcomes of surgical effect in patients with unilateral main pulmonary artery occlusion

    ObjectiveTo summarize the clinical characteristics and the long-term results of pulmonary thromboendarterectomy (PTE) in the chronic thromboembolic pulmonary hypertension (CTEPH) patients with unilateral main pulmonary artery occlusion.MethodsWe retrospectively analyzed the clinical data of 15 CTEPH patients with unilateral main pulmonary artery occlusion in Fuwai Hospital between 2004 and 2018. There were 11 males and 4 females aged 34.1±12.0 years at operation.ResultsThe mean circulatory arrest was 31.1±12.1 minutes. The ICU stay was 5 (2-29) d. The hospital stay was 15 (8-29) d. There was no hospital death. There was a decline in systolic pulmonary artery pressures (sPAP, 69.9±27.9 mm Hg to 35.1±9.7 mm Hg, P=0.020) after surgery. On postoperative V/Q scan, only 6 patients (40.0%) had significant improvement in reperfusion (≥75% estimated) of the occluded lung. There was no death during the median observation period of 49 months follow-up, while 2 patients had recurrence of pulmonary embolism.ConclusionCTEPH patients with unilateral main pulmonary artery occlusion represent a challenging cohort. PTE is a curative resolution in both early- and long- term results, although there is a high requirement of perioperative management and a high risk of postoperative complications and rethrombosis.

  • 下肢肿胀患者D2聚体和静脉造影的诊断价值

    【摘要】 目的 探讨D2聚体对下肢静脉血栓形成的诊断价值。 方法 2009年1月-2010年1月,对80例下肢肿胀患者进行D2聚体和静脉造影检查,对检查结果进行回顾性分析。 结果 下肢完全型血栓12例,占15.00%,髂外、股总静脉血栓33例,占41.25%,腓肠肌静脉丛血栓9例,占11.25%。D2聚体与静脉造影的结果无统计学意义(Pgt;0.05)。D2聚体阳性者57例,非血栓性肿胀者中仅4例;D2聚体阴性者23例,非血栓性肿胀者22例。 结论 D2聚体为下肢血栓提供了一个敏感、有效地检查方法,其阴性的价值远远大于阳性价值,用于排除诊断更有意义。

  • Analysis of thrombotic events and mortality in patients with sever pneumonia in intensive care unit

    Objective To explore the thromboembolic events and mortality in patients with different types of severe pneumonia, and to analyze the related high-risk factors. Methods A total of 161 severe pneumonia patients who admitted in intensive care unit from January 2018 to February 2023 were included in the study. The patients were divided into a COVID-19 group (n=88) and a community-acquired pneumonia (CAP) group (n=73) according to the type of pneumonia, and divided into a thrombosis group and a non-thrombosis group according to the occurrence of thrombosis. The patients were followed-up until discharge or in-hospital death, registering the occurrence of thrombotic events. Results During the in-hospital stay, 32.9% of CAP and 36.4% of COVID-19 patients experienced thrombotic events (P>0.05). In CAP group all the events (including 24 paitents) were venous thromboses, while in COVID-19 group 31 patients were venous and 3 were arterial thromboses (2 were cerebral infarction, and 1 with myocardial infarction). There were statistically significant difference in gender, age, venous thromboembolism score (VTE score), activated partial thromboplastin time (APTT), and procalcitonin (PCT) between the TE group and the Non-TE group. Logistic regression analysis showed that thrombotic events was associated with sex, age and APTT; gender (female: OR=2.47, 95%CI 1.13 - 5.39, P<0.05) and age (OR=1.04, 95%CI 1.01 - 1.07, P<0.05) were positively associated with thrombotic events. During the in-hospital follow-up, 44.3% of CAP patients and 42.5% of COVID-19 patients died (P>0.05). Receiver operator characteristic (ROC) curve analysis showed that APACHEⅡ score was more accurate in predicting mortality of severe pneumonia, and the area under the ROC curve (AUC) was 0.77 (95%CI 0.70 - 0.84, sensitivity 74.3%, specificity 68.1%), the AUC of the VTE score was 0.61 (95%CI 0.53 - 0.70, Sensitivity 31.4%, specificity 81.7%); the AUC of the creatinine was 0.64 (95%CI 0.56 - 0.73, sensitivity 72.9%, specificity 51.2%). While the Kappa value for kidney disease was 0.409 (P<0.05) presenting moderate consistency. Conclusions The incidence of thromboembolic events and mortality are high in patients with different types of severe pneumonia. Thrombophilia was associated with sex, age, and APTT. APACHEⅡ score, VTE score, and creatinine value were independent risk factors for predicting death from severe pneumonia.

  • AngioJet mechanical thrombectomy in treatment of acute deep venous thrombosis of lower extremities

    ObjectiveTo evaluate the effect of percutaneous mechanical thrombectomy (PMT) with AngioJet mechanical thrombus aspiration system for the acute deep venous thrombosis (DVT) of lower extremities. MethodsThe clinical data of 72 patients (72 limbs) with acute DVT who underwent PMT with AngioJet system from December 2015 to June 2018 in our hospital were analyzed retrospectively. ResultsOf the 72 cases, 30 cases underwent PMT alone, while 42 cases underwent PMT combined with catheter directed thrombolysis (CDT). Thrombus clearance rate of grade Ⅲ was obtained in 49 cases (68.05%), grade Ⅱ in 20 cases (27.78%), and grade Ⅰ in 3 cases (4.17%). Thirty-five cases were found with May-Thurner syndrome, and 34 cases were treated with stenting while 1 case complicated with iliac bleeding. The rates of PTS were 1.41% (1/71), 3.57% (2/56), 4.55% (2/44), and 20.00% (3/15) at 3-month, 6-month, 1-year, and 2-year after intervention, respectively. The deep vein patency rates were 86.36% (38/44) and 80.00% (12/15) at 1-year and 2-year after intervention, respectively. The iliac stent patency rates were 100% (23/23) and 87.50% (7/8) at 1-year and 2-year after intervention, respectively. ConclusionThe effect of PMT assisted with CDT for the acute DVT of lower extremities is satisfactory, but its long-term efficacy needs to be further observed.

  • Bipolar Radiofrequency Ablation for Left Ventricular Aneurysm-related Ventricular Arrhythmia Associated with Mural Thrombus

    ObjectiveTo investigate the efficacy of bipolar radiofrequency ablation for left ventricular aneurysm-related ventricular arrhythmia associated with mural thrombus. MethodsFifteen patients with left ventricular aneurysm-related frequent premature ventricular contractions associated with mural thrombus were enrolled in Beijing Anzhen Hospital between June 2013 and June 2015. There were 11 male and 4 female patients with their age of 63.5±4.8 years. All patients had a history of myocardial infarction, but no cerebral infarction. All patients received bipolar radiofrequency ablation combined with coronary artery bypass grafting, ventricular aneurysm plasty and thrombectomy. Holter monitoring and echocardiography were measured before discharge and 3 months following the operation. ResultsThere was no death during the operation. Cardiopulmonary bypass time was 92.7±38.3 min. The aortic clamping time was 52.4±17.8 min.The number of bypass grafts was 3.9±0.4. All the patients were discharged 7-10 days postoperatively. None of the patients had low cardiac output syndrome, malignant arrhythmias, perioperative myocardial infarction, or cerebral infarction in this study. Echocardiography conducted before discharge showed that left ventricular end diastolic diameter was decreased (54.87±5.21 cm vs. 60.73±6.24 cm, P=0.013). While there was no significant improvement in ejection fraction (45.20%±3.78% vs. 44.47%±6.12%, P=1.00) compared with those before the surgery. The number of premature ventricular contractions[4 021.00 (2 462.00, 5 496.00)beats vs. 11 097.00 (9 327.00, 13 478.00)beats, P < 0.001] and the percentage of premature ventricular contractions[2.94% (2.12%, 4.87%) vs. 8.11% (7.51%, 10.30%), P < 0.001] in 24 hours revealed by Holter monitoring were all significantly decreased than those before the surgery. At the end of 3-month follow-up, all the patients were angina and dizziness free. Echocardiography documented that there was no statistical difference in left ventricular end diastolic diameter (55.00±4.41 mm vs. 54.87±5.21 mm, P=1.00). But there were significant improvements in ejection fraction (49.93%±4.42% vs. 45.20%±3.78%, P=0.04) in contrast to those before discharge. Holter monitoring revealed that the frequency of premature ventricular contractions[2 043.00 (983.00, 3 297.00)beats vs. 4 021.00 (2 462.00, 5 496.00)beats, P=0.03] were further lessened than those before discharge, and the percentage of premature ventricular contractions[2.62% (1.44%, 3.49%)vs. 8.11% (7.51%, 10.30%), P < 0.001] was significantly decreased than those before the surgery, but no significant difference in contrast to those before discharge. ConclusionThe recoveries of cardiac function benefit from integrated improvements in myocardial ischemia, ventricular geometry, pump function, and myocardial electrophysiology. Bipolar radiofrequency ablation can correct the electrophysiological abnormality, significantly decrease the frequency of premature ventricular contractions, and further improve the heart function.

  • Curative Effect Observation on Treatment of Lower Limb Thrombophlebitis ofSuperficial Phlebitis with Trivex Cutting System

    目的 探讨Trivex旋切术治疗下肢血栓性浅表静脉炎的临床疗效。方法 回顾性分析67例下肢血栓性浅表静脉炎病例的临床资料。患者术前给予丹参治疗、下肢静脉造影等术前准备; 然后行大隐静脉主干内膜剥脱、用Trivex旋切系统剖吸曲张静脉和静脉丛内的血栓; 术后给予低分子肝素抗凝、早期下床活动等处理。结果 全部患者的手术均顺利,术中及术后无肺栓塞发生。术后2例切口Ⅱ期愈合, 65例切口Ⅰ期愈合。所有病例术后3d均出院,住院时间(5±1) d。15例术后患肢有瘀斑者随访2周均消失; 7例出现内踝部麻木感,随访6个月消失; 本组患者均获随访6个月,未见复发者。结论 下肢血栓性静脉炎早期行以Trivex旋切术为主的综合治疗,其临床疗效好、住院时间短、创伤小及美容效果好,并可有效防止病情反复、深静脉血栓及肺栓塞的发生,是治疗下肢血栓性静脉炎的有效手段。
