  • Vitreoretinal surgery using the 25G transconjunctival sutureless vitrectomy system under topical anesthesia

    Objective To evaluate the indications, effectiveness and complications of vitreoretinal surgery using the 25G transconjunctival sutureless vitrectomy system (TSV25G) under the topical anesthesia. Methods The clinical and follow-up data of 22 eyes of 22 patients undergone vitreo-retinal surgery using TSV25G under the topical anesthesia were retrospectively analyzed. All of the patients were monocular sickened, including idiopathic macular hole in 10 eyes, idiopathic macular pucker in 6, vitreoretinal traction syndrome in 4, and vitreous hemorrhage associated with branch retinal vein occlusion in 2. Peeling of epiretinal membrane and/or internal limiting membrane, intra ocular laser coagulation, air-fluid exchange and tamponiding of C3F8 were performed according to the condition of diseases. The postoperative follow-up was 1-11 months, with the mean duration of 6.4 months. The effect of analgesia, cooperation with the patients, operative effect and complications in and after the surgery were observed. Results The operations finished successfully in all of the eyes under the topical anesthesia. The operation duration ranged from 20 to 25 minutes with average of 22 minutes. The patients cooperated with the doctor well without any discomfort. Two days after the surgery, edema of the wounded conjunctiva was found, and recovered 7 days later. A light pigment dot on the surface of the sclera could be seen at the first month. The complic ations included transient increasing of intraocular pressure in 2 eyes, feather-like opacity of lens in 5 eyes, vitreous hemorrhage in 1 eye, and air-bleb under conjunctiva in 2 eyes. No other complications related with the cut were fo und. The macular hole closed in 9 eyes with idiopathic macular hole, and the other 1 had the smaller but not closed hole. Idiopathic macular pucker, vitreoretinal traction syndrome, and vitreous hemorrhage associated with branch retinal vein occlusion were cured successfully. Conclusions Vitreoretinal surgery using the TSV25G under the topical anesthesia has many advantages such as simple procedure, short operation time, micro-invasion, less complications and rapid revovery, and mainly serves simple manipulation in some simple diseases such as idiopathic macular hole, vitreo-retinal traction syndrome, and simple hemorrhage.   • The Experience of Application in Tension-Free Herniorrhaphy Under Local Anesthesia in Senile Inguinal Hernia

    ObjectiveTo investigate the application of tension-free herniarepair under local anesthesia in senile inguinal hernia. MethodsClinical data of 163 cases of senile inguinal hernias with herniorrhaphy under local anesthesia in our department from October 2011 to October 2014 were analyzed retrospectively, including epidural anesthesia 90 patients and local anesthesia 73 patients. ResultsAll patients were successfully completed surgery. Hospital charges in local anesthesia were much cheaper than that in epidural anesthesia group (P=0.002). Hospital days in local anesthesia were much shorter than that in epidural anesthesia group (P=0.035). Lung complication in local anesthesia were much less than that in epidural anesthesia group (P=0.015). Other indicators were no significant difference between the two groups (P > 0.05). ConclusionTension-free herniorrhaphy under local anesthesia in elderly patients is safe, reliable, less invasive method with low costs, slight postoperative pain, and worthy of promotion.

    七氟烷是一种新型吸入全身麻醉剂,它血/气分配系数低,使其起效迅速,诱导时间短,麻醉深度易控制,苏醒速度也快;七氟烷气味芳香,对呼吸道无刺激,对血流动力学和自主呼吸影响也较小。七氟烷的这些药理学特点为吸入诱导在技术上和原理上开创了新的方法,使麻醉诱导有了新的选择。临床上对于不能合作的儿童和特殊的成人患者,常采用不同吸入诱导方式,不同的吸入浓度,根据患者和疾病的不同特点,选择不同的诱导方法和不同的复合方式,使患者更加安全舒适地渡过麻醉诱导过程。在此类研究中,全凭七氟烷或联合应用氧化亚氮、丙泊酚和阿片类药物如舒芬太尼、阿芬太尼、芬太尼和瑞芬太尼等,在成人和儿童中成功地完成了气管插管。该文就七氟烷吸入诱导近年来的研究进展作一综述。

  • Clinical Research on Constant Infusion of Propofol for Conscious Sedation under Combined Spinal-epidural Anesthesia

    【摘要】 目的 探讨腰硬联合麻醉复合丙泊酚恒速输注清醒镇静的可行性、理想的药物剂量、术中知晓情况以及麻醉质量和效果。 方法 收集2009年3-12月480例美国麻醉医师协会(ASA)Ⅰ~Ⅲ级拟在腰硬联合麻醉下行下腹部、会阴部、下肢手术的患者480例,随机分为咪达唑仑组(M组)、丙泊酚Ⅰ组(PA组)、丙泊酚Ⅱ组(PB组)、丙泊酚Ⅲ组(PC组),每组各120例。四组患者均于腰2-3或腰3-4行腰硬联合麻醉,蛛网膜下腔注入轻比重0.2 %布比卡因12~15 mg,麻醉平面确切后,M组予以咪达唑仑0.04~0.06 mg/kg,PA组先予以负荷量丙泊酚0.50 mg/kg再以2.00 mg/(kg•h)剂量持续泵注,PB组予以负荷量丙泊酚0.75 mg/kg再以3.00 mg/(kg•h)剂量持续泵注,PC组予以负荷量丙泊酚1.00 mg/kg再以3.75 mg/(kg•h)剂量持续泵注。观察患者给药前(T0)、给药1(T1)、3(T2)、5(T3)、10(T4)、30(T5)、60 min(T6)各时点血流动力学平均动脉血压(MAP)、心率(HR)的变化、脑电双频指数(BIS)值及镇静评分、术中所看到的图片的回忆及不良反应。 结果 各组在给予镇静药后MAP、HR均有所下降,但测量值的变化在正常范围内;在T3时间点,各组BIS值及镇静/警醒OAA/S评分降低,与T0比较,差异有统计学意义(Plt;0.05);与其他3组比较,在T4、T5、T6时点PC组BIS值与OAA/S评分降低,差异有统计学意义(Plt;0.05),PC组的镇静遗忘满意率高于其他3组;各组间未见发生严重的舌后坠、呼吸暂停和血氧饱和度(SpO2)lt;90%。 结论 在下腹部、下肢手术中,应用腰硬联合麻醉复合1.00 mg/kg负荷量的丙泊酚继而以3.75 mg/(kg•h)剂量持续泵注,可取得良好的镇静效果,不良反应小。【Abstract】 Objective To investigate the feasibility, ideal dose, intra-operative awareness as well as the quality and effectiveness of constant infusion of propofol under combined spinal-epidural anesthesia (CSEA) for conscious sedation. Methods A total of 480 patients at ASA grade Ⅰ-Ⅲ to be operated in the lower abdomen, perineum and lower limbs under CSEA from March to December 2009 were randomly divided into four groups: midazolam group (M group), propofol group Ⅰ (PA group), propofol group Ⅱ (PB group), and propofol group Ⅲ (PC group), with 120 patients in each group. All four groups of patients underwent CSEA at L2-3 or L3-4 and accepted pinal injection of 12-15 mg of 0.2% hypobaric bupivacaine. After the anesthetic plane was confirmed, patients in M group accepted 0.04-0.06 mg/kg of midazolam; patients in PA group accepted propofol at a loading dose of 0.50 mg/kg followed by continuous infusion at a dose of 2.00 mg/(kg•h); patients in PB group accepted propofol at a loading dose of 0.75 mg/kg followed by continuous infusion at a dose of 3.00 mg/(kg•h); patients in PC group accepted propofol at a loading dose of 1.00 mg/kg followed by continuous infusion at a dose of 3.75 mg/(kg•h). The change of hemodynamics including the mean arterial pressure (MAP) and the heart rate (HR), bispectral index (BIS) values, sedation scores, memory of pictures seen during operation and adverse effects before drug administration (T0), at minute 1 (T1), 3 (T2), 5 (T3), 10 (T4), 30 (T5) and 60 (T6) after drug administration were observed. Results MAP and HR decreased in all the four groups after administration of sedatives, but the changes of measured values were within normal ranges. BIS value and the Observer’s Assessment of Alertness and Sedation (OAA/S) scale decreased in all groups at T3, compared with those at T0 (Plt;0.05). Compared with the other 3 groups, BIS valueand OAA/S scale were significantly lower in PC group at T4, T5 and T6 (Plt;0.05), and the satisfaction rate of sedation and amnesia was much higher. No serious glossocoma, apnea and SpO2 below 90% was observed in all the four groups. Conclusion During the surgery of lower abdomen and lower limbs, application of CSEA combined with propofol at a loading dose of 1.00 mg/kg followed by continuous infusion at a dose of 3.75 mg/(kg•h) can achieve a good sedative effect, with little side effect.

  • Clinical Observation of Penehyclidine Hydrozhloride Reducing Excessive Oral Secretion in Children with Cerebral Palsy after Free and Excision of Sympathetic Plexus of Common Carotis Artery under General Anesthesia

    目的 研究需要行颈动脉外膜剥脱术脑瘫患儿全身麻醉(全麻)诱导时给予盐酸戊乙奎醚预防术后口腔分泌物过多的效果。 方法 2009年12月-2011年12月选择60例美国麻醉医师协会分级Ⅰ~Ⅱ级的需要在全麻下行颈动脉外膜剥脱术的脑瘫患儿,随机分为两组,每组30例。A组于麻醉诱导时静脉注射阿托品10 μg/kg,B组于麻醉诱导时静脉注射戊乙奎醚10 μg/kg。分别记录两组的麻醉持续时间、入室心率、气管插管后10 min的心率差、停药后的拔管时间及拔管时口腔分泌物评分[采用视觉模拟评分法(VAS)]。 结果 两组相比,患儿麻醉持续时间、入室心率和停药后拔管时间差异均无统计学意义(P>0.05),而拔管时口腔分泌物的VAS评分差异有统计学意义(P<0.05),其中口腔分泌物过多(VAS>2分),A组为46.67%,B组为16.67%,B组比A组口腔分泌物更少,气管插管后10 min心率A组为(28.30 ± 9.73)次/min,B组为(9.93 ± 10.25)次/min,两组差异有统计学意义(P<0.05)。 结论 盐酸戊乙奎醚能够有效减少需要行颈动脉外膜剥脱术的脑瘫患儿术后口腔分泌物过多的情况,且比常规应用阿托品的效果更好。

  • Application of Combined Spinal Epidural Anesthesia Plus Propofol in Cesarean Section

    目的:观察丙泊酚静脉泵注复合腰硬联合麻醉于剖宫产术中的可行性及安全性。方法:50例ASA I~II级行择期剖宫产术产妇,于L2-3行腰硬联合麻醉,确定麻醉平面为T4-6,取出胎儿后静脉缓推丙泊酚1mg/kg,然后2~4mg·kg-1·h-1静脉泵入,连续监测平均动脉压,心率,血氧饱和度,呼吸频率。结果:腰硬联合麻醉及静脉推注负荷剂量丙泊酚后平均动脉压降低,但无临床意义。余心率,氧饱和度,呼吸频率各时点无统计学差异。结论:丙泊酚复合腰硬联合麻醉用于剖宫产术患者生命体征平稳,镇静效果良好。

  • 早泄的药物治疗进展

    早泄是最常见的男性性功能障碍,药物一直是早泄治疗的焦点。选择性5-羟色胺再摄取抑制剂(SSRIs)是目前临床证据最多、应用最广泛的药物,其中达泊西汀是唯一被批准用于治疗早泄的短效SSRI;局部麻醉药物疗效确切,可按需使用,且不良反应少,是治疗早泄的理想药物;α1-肾上腺素能受体阻滞剂、5型磷酸二酯酶抑制剂、曲马多的有效性和安全性尚缺乏大量确切临床证据,今后有望成为治疗早泄的新方法。随着对生理性射精过程和相关神经递质的深入研究,将会为早泄的药物治疗提供新的思路和选择。

    【摘要】 目的 评价脑电双频指数(BIS)监测全身麻醉深度的准确性及实用性。 方法 2007年10月-2009年10月择期行腹腔镜胆囊切除手术的40例ASAⅠ或Ⅱ级患者随机平均分为A、B组,两组均采用丙泊酚、瑞芬太尼诱导和维持。A组以BIS值判断麻醉深度并指导调整用药,B组根据经验调整用药,使BIS值维持在50±5、MAP和HR维持在基础值±20%范围内。常规监测收缩压(SBP)、舒张压(DBP)、心率(HR)和BIS,计算用药总量,记录苏醒时间;诱导期进行改良警觉/镇静(OAA/S)评分,评价BIS对全身麻醉手术期间麻醉深度的监测和指导意义。 结果 麻醉期间,B组SBP、DBP、HR和BIS波动明显大于A组(Plt;0.05);A组的苏醒时间(7.5±2.5) min明显短于B组(9.8±3.9) min(Plt;0.05);拔管后,A组躁动、嗜睡、恶心、呕吐患者少于B组;A组无1例发生手术中知晓,B组1例发生手术中知晓。 结论 BIS可动态反映大脑生理功能的变化,有助于临床判断全身麻醉深度,指导麻醉用药。【Abstract】 Objective To evaluate the accuracy and practicality of bispectral index (BIS) used in patients undergoing general anesthesia. Methods Forty patients of ASA class Ⅰ or Ⅱ were randomly divided into group A and B,20 patients in each group. Anesthetic depth was judged and anesthetics was administered by BIS monitoring (being kept in 50±5) in group A or based on the experience of anesthetist in group B. After anesthesia induction, target propofol and remifentanil concentrations were adjusted to maintain the MAP, HR within the range of ±20% of preinduction values. Systolic pressure (SBP), distolic pressure (DBP), HR, SpO2 and BIS were monitored. Awake time and anesthetic consumption were recorded. Results The changes of SBP, DBP, HR and BIS were less in group A than those in group B (Plt;0.05).Awake time was shorter in group A than that in group B and restlessness drowsiness, nausea and vomitting after extubation were less in group A than those in group B. No awareness patient during operation was seen in group A, but one patient suffered from awareness in group B. Conclusion BIS monitoring can dynamicly reflect cerebral physilological function during general anesthesia and is helpful in judging anesthesia depth and directing the administration of anesthetics.

  • Anesthesia management in hepatic echinococcosis surgery

    We reviewed the research progress of anesthesia management in hepatic echinococcosis surgery in recent years, including the key technologies, practical experience, and research progress of anesthesia management in hepatic echinococcosis surgery, so as to guide clinical practice. Firstly, in the selection of anesthesia, the general anesthesia combined with epidural block or regional nerve block is recommended to improve surgical safety and patients’ comfort. At the same time, the importance of intraoperative continuous monitoring, including key indicators such as hemodynamics and respiratory function, is emphasized, and transesophageal echocardiography and brain function monitoring techniques are introduced to optimize anesthesia management. Finally, the concept of enhanced recovery after surgery is promoted, and measures such as preoperative optimization, intraoperative heat preservation, refined fluid management, and postoperative analgesic management are implemented to promote the rapid recovery of patients. At the same time, some challenges and unsolved problems in the current research also are pointed out, such as complex case evaluation, complications prevention, and teamwork, etc., which need to be further studied in the future to optimize the anesthesia strategy.
