ObjectiveTo summarize the research progression of Doppler-guided hemorrhoidal artery ligation in the treatment of hemorrhoids. MethodsThe related literatures in recent years were reviewed, and to investigate the operation principle, operation process, the indications and effects, and existing problems of Doppler-guided hemorrhoidal artery ligation. ResultsThe surgical principle of Doppler-guided hemorrhoidal artery ligation make use of Doppler ultrasound looking for hemorrhoidal artery, and ligation it directly. It's best indication is Ⅱ or Ⅲ degree hemorrhoids or mixed hemorrhoid with grade Ⅰ or Ⅲ mainly, especially for bleeding hemorrhoids disease curative effect is better. This surgical method has lots of advantages, such as less invasive, quick recovery, and low incidence of complications. But it is no significant treatment effect for the external hemorrhoids, therefore, the procedure does not apply to external hemo-rrhoids and mixed hemorrhoids dominated by external hemorrhoids. ConclusionsDoppler-guided hemorrhoidal artery ligation has the advantage of minimally invasive, it is a safe and effective treatment for Ⅱ and Ⅲ degree internal hemorr-hoids or Ⅱ and Ⅲ degree mixed hemorrhoids dominated by internal hemorrhoids.
【Abstract】 Objective To introduce the recent studies about the lymph node-targeted chemotherapy for gastric carcinoma. Methods The literatures on the lymph node-targeted chemotherapy for gastric carcinoma in recent years were collected and reviewed. Results The lymph node-targeted chemotherapy for gastric carcinoma was effective because it couled improve the drug concentrations in regional lymph node. Conclusion As a part of multiple treatments for gastric carcinoma, lymph node-targeted chemotherapy will be further developed.

Objective To analyze the risk factors of prethrombotic state of obstructive sleep apnea and hyponea syndrome (OSAHS), providing basis and reference for the prevention of prethrombotic state of OSAHS. Methods Two hundred and thirty-eight patients excluding the presence of possible effects of coagulation factors from June 2014 to July 2016 were diagnosed as OSAHS by polysomnography (PSG) and underwent coagulation, thrombosis, fibrinolysis, and inflammatory factors testing. Fifty-six patients met the standard of prethrombotic state (prethrombotic state group) and 59 patients randomly selected from the remaining 182 patients did not meet the standard (non-prethrombotic state group). The age, sex, body mass index (BMI), sleep apnea and hypopnea index (AHI), interleukin-6 (IL-6), complicating chronic obstructive pulmonary disease (COPD) and hypertension were compared between two groups. Results Non conditional Logistic regression analysis showed that the risk factors of prethrombotic state of OSAHS were age (OR=1.202, 95%CI: 1.107 to 1.305), IL-6 (OR=1.127, 95%CI: 1.014 to 1.252), AHI (OR=1.151, 95%CI: 1.055 to 1.256), and complicating COPD (OR=4.749, 95%CI: 1.046 to 21.555). Conclusion Age, AHI, IL-6, and complicating COPD may be the risk factors of prethrombotic state of OSAHS, among which complicating COPD may be the most important risk factor.
Objective To evaluate the effect of laparoscopy-assisted surgery for lymph node dissection in patients with carcinoma of gastric cardia. Methods The clinical data of patients with carcinoma of gastric cardia who underwent either laparoscopy-assisted or open gastrectomy between January 2004 and September 2009 in the Department of General Surgery, the Nanchong Central Hospital were analyzed retrospectively. The number of lymph node dissection was compared. Results Thirty-nine patients underwent laparoscopy-assisted gastrectomy (laparoscopy group) and 63 patients underwent open gastrectomy (open group). There was no significant difference in preoperative complications, type of pathology or pTNM stage between two groups (Pgt;0.05). The number of lymph node dissection was 16.44±6.25 in the laparoscopy group, of which the number of first station lymph node was 10.56±3.78 (metastasis rate was 74.4%), the second station was 3.82±1.82 (metastasis rate was 46.2%), the third station was 2.00±1.36 (metastasis rate was 5.1%); in the open group, the numbers of corresponding lymph node were 16.38±5.83, 10.94±3.91 (metastasis rate was 71.4%), 3.71±1.55 (metastasis rate was 42.9%), and 1.75±1.06 (metastasis rate was 3.2%), respectively. There was no significant difference between two groups (Pgt;0.05). Conclusion The effectiveness of lymph node dissection is satisfactory by laparoscopy-assisted surgery for patients with carcinoma of gastric cardia, but prospective efficacy is still being followed up.
