  • NON-OPERATION RELATED RISK FACTORS OF WOUND COMPLICATIONS OF CALCANEAL FRACTURES USING LATERAL EXTENSIVE L-SHAPED INCISION

    Objective To analyze the non-operation related risk factors of the wound complications by using lateral extensive L-shaped incision for open reduction and internal fixation of calcaneal fractures. Methods A retrospective analysis was made on the clinical data of 58 patients with closed calcaneal fractures (63 calcaneus) treated by using lateral intensive L-shaped incision for open reduction and internal fixation between September 2006 and August 2011. There were 52 males (56 calcaneus) and 6 females (7 calcaneus), aged 18 to 64 years (mean, 35 years). The causes of injury included fall injury in 53 cases (58 calcaneus), traffic injury in 5 cases (5 calcaneus). The mean time between injury and operation was 8 days (range, 3-22 days). According to Sanders classification, 4 calcaneus were rated as type II, 31 calcaneus as type III, and 28 calcaneus as type IV. Postoperative complications were observed and graded; 58 patients were divided into complication group (≥grade 2) and control group (lt; grade 2). The univariate analysis was used to analyze 18 factors which may lead to wound complications; multi-factor unconditioned logistic regression analysis was done for the factors showing significant difference. Results According to postoperative wound complications grading, 41 patients (46 calcaneus) were included in the control group, whose incision healed primarily, and 17 patients (17 calcaneus) in the complication group. In 17 patients of the complication group, 14 had skin necrosis or dehiscence, and 3 had superficial infection; they obtained healing after symptomatic treatment. The univariate analysis showed significant differences in combined spinal fracture, diabetes mellitus, and long-term smoking between 2 groups (P lt; 0.05). The logistic regression analysis revealed that combined spinal fracture was an independent risk factor for wound complications (95% confidence interval: 0.004-0.360, P=0.004). Conclusion Combined spinal fracture is an independent risk factor for wound complications after open reduction and internal fixation of calcaneal fracture using lateral extensive L-shaped incision.

  • Introduction of international evaluation process of genetic testing technology

    ObjectivesTo provide a reference for the evaluation procedures of genetic testing technology applicable to China by combining the existing evaluation frameworks and procedures for genetic testing techniques globally, and also put forward design suggestions for the construction of evaluation procedures in China.MethodsThe literature research method was primarily used to summarize different evaluation progress, as well as put forward design suggestions.ResultsAt present, numerous developed countries have organized genetic testing technology evaluation projects. The various evaluation frameworks developed were based on the ACCE or HTA framework. The evaluation and decision-making procedures were similar in general, including topic selection, evaluation implementation, results reporting and making recommendations. However, there still remained difficulties such as limited evidence and uncertainty in decision-making.ConclusionsTo establish the procedures of genetic testing technology applicable in China, the following specific procedures are recommended: selecting target genetic testing technology topics; analyzing necessity and feasibility of target testing technology evaluation; evaluating and reviewing the evidence; applying results and decision-making transformation; developing regular review and revision mechanisms.

  • Progress on the role of nuclear factor-erythroid 2-related factor 2 in regulating cell proliferation

    Nuclear factor-erythroid 2-related factor 2 (Nrf2) is an important factor for cells to resist oxidative stress and electrophilic attack. It is involved in the formation and control of oxidative stress defense pathways. It is associated with oxidative stress-related diseases, including cancer, neurodegenerative diseases, cardiovascular diseases and aging, and is a potential pharmacological target for the treatment of chronic diseases. This article will review the important role of Nrf2 in the regulation of cell proliferation, including direct regulation of cell proliferation, regulation of reactive oxygen species, intracellular metabolism, regulation of mitochondrial function, cell lifespan and inflammatory response. The aim is to provide a theoretical basis for further research on how to use Nrf2 to regulate cell proliferation.

  • Research progress on the application of novel sensing technologies for sleep-related breathing disorder monitoring at home

    Sleep-related breathing disorder (SRBD) is a sleep disease with high incidence and many complications. However, patients are often unaware of their sickness. Therefore, SRBD harms health seriously. At present, home SRBD monitoring equipment is a popular research topic to help people get aware of their health conditions. This article fully compares recent state-of-art research results about home SRBD monitors to clarify the advantages and limitations of various sensing techniques. Furthermore, the direction of future research and commercialization is pointed out. According to the system design, novel home SRBD monitors can be divided into two types: wearable and unconstrained. The two types of monitors have their own advantages and disadvantages. The wearable devices are simple and portable, but they are not comfortable and durable enough. Meanwhile, the unconstrained devices are more unobtrusive and comfortable, but the supporting algorithms are complex to develop. At present, researches are mainly focused on system design and performance evaluation, while high performance algorithm and large-scale clinical trial need further research. This article can help researchers understand state-of-art research progresses on SRBD monitoring quickly and comprehensively and inspire their research and innovation ideas. Additionally, this article also summarizes the existing commercial sleep respiratory monitors, so as to promote the commercialization of novel home SRBD monitors that are still under research.

  • Association between bisphenol A levels during pregnancy and spontaneous abortion: a meta-analysis

    Objective To systematically review the association between exposure to bisphenol A during pregnancy and spontaneous abortion. Methods The PubMed, Web of Science, EMbase, CNKI, WanFang Data and VIP databases were electronically searched to identify cohort studies and case-control studies related to bisphenol A exposure and spontaneous abortion from inception to April 1st, 2022. Two reviewers independently screened the literature, extracted data, and assessed the risk of bias of the included studies. Meta-analysis was then performed using Stata 16.0 software. Results A total of 7 case-control studies and 1 cohort study were included, with a total of 1 179 subjects. The results of meta-analysis showed that there was a statistically significant difference in bisphenol A concentrations between the spontaneous abortion group and the control group regardless of whether the sample source was serum or urine (SMD serum=1.05, 95%CI 0.34 to 1.77, P=0.004; SMD urine=0.20, 95%CI 0.02 to 0.38, P=0.027). Conclusion The current evidence shows that exposure to bisphenol A during pregnancy may lead to unexplained recurrent spontaneous abortion. Due to the limited quantity and quality of the included studies, more high-quality studies are needed to verify the above conclusion.

  • Diagnostic value of fine-needle biopsies for benign and malignant breast nodules: a meta-analysis

    ObjectiveTo systematically review the value of the fine-needle biopsies in the diagnosis of benign and malignant breast nodules. MethodsA computer based online search was conducted in PubMed, The Cochrane Library (Issue 7, 2016), EMbase, CBM, CNKI, VIP, WanFang Data databases up to August, 2016 to collect the relevant diagnostic studies of the fine-needle biopsies for benign and malignant breast nodules. Two reviewers independently screened literature, extracted data and assessed the methodological quality of included studies. Then, meta-analysis was performed using Stata 12.0 and Meta-Disc 1.4 software.ResultsA total of 22 studies involving 24 496 patients were finally included. The results of meta-analysis showed that the pooled Sen, Spe, +LR, –LR, and DOR were 0.95 (95%CI 0.95 to 0.96), 0.96 (95%CI 0.96 to 0.96), 38.27 (95%CI 23.17 to 63.19), 0.05 (95%CI 0.03 to 0.07), 1 031.11 (95%CI 514.19 to 2 067.68), respectively. The AUC of SROC was 0.99 (95%CI 0.98 to 1.00).ConclusionThe current evidence indicates that the FNAC has high diagnostic value in the differential diagnosis of benign and malignant breast nodules.

  • Application value of electronic rectosigmoidoscope in screening of anorectal diseases in elderly population

    ObjectiveTo understand the prevalence of anorectal diseases in the elderly in the community, and to evaluate the role of electronic rectosigmoidoscope in the screening of anorectal diseases.MethodThe resident population aged 60–74 in the Xichang’an Street Area of Xicheng District of Beijing were collected to complete the initial screening by filling in the risk factor assessment questionnaire, and who were classified as the high-risk with positive initial screening were admitted to the diagnostic re-screening by electronic rectosigmoidoscope.ResultsThere were 4 782 screening subjects, while 4 347 people actually participated in the initial screening, and 2 997 people were positive (positive rate was 68.9%). Among them, 2 312 participated in the electronic rectosigmoidoscope screening, and the compliance rate was 77.1%. A total of 2 275 patients with various anorectal diseases were detected by electronic rectosigmoidoscope, 1 311 (56.7%) of whom were hemorrhoids, 458 (19.8%) were rectal polyps, 154 (6.7%) were anal papillary hypertrophy, 147 (6.4%) were proctitis, 103 (4.4%) were anal fissure, 52 (2.2%) were anal fistula , 33 (1.4%) were rectal melanoma , and 17 (0.7%) were suspected rectal cancer. The detection rates of rectal polyps, proctitis, and anal fistula in the men were higher than those in the women (P<0.05), while the detection rates of hemorrhoids, anal papilla hypertrophy, anal fissure, and rectal melanosis in the women were higher than those in the men (P<0.05).ConclusionsAs a screening tool, electronic rectosigmoidoscope has the characteristics of simple operation, good compliance of the subject, and high compliance rate for re-screening. It is suitable for large-scale screening of anorectal diseases for elderly residents in the community.

  • Research of Establishing Hyperlipidemia Model in Rats with Fat Emulsion

    Objective To compare two ways of establishing hyperlipidemia model in rats with fat emulsion.Methods Thirty male SD rats were randomly divided into three groups, which fed with normal diet (normal control group), low concentration fat emulsion (low concentration fat emulsion group), and high concentration fat emulsion (high concentration fat emulsion group), respectively. All the rats were sacrificed and tested for serum total cholesterol (TC), triglyceride (TG), high density lipoprotein-cholesterol (HDL-C), and low density lipoprotein-cholesterol (LDL-C)in two weeks after establishment. Results The levels of TC, TG, HDL-C, and LDL-C in the high concentration fat emulsion group were significantly higher than those in the normal control group and the low concentration fat emulsion group (P<0.05);the levels of TC, TG, and HDL-C in the low concentration fat emulsion group didn’t significantly differ from normal control group (P>0.05). Conclusions High concentration fat emulsion can be used to establish hyperlipidemia model in SD rats, low concentration fat emulsion is not suitable for establishing hyperlipidemia model in SD rats.

  • Study on Expression of ORP150 in Pancreas of Rats with Severe Acute Pancreatitis

    ObjectiveTo explore the change of expression of oxygen-regulated protein 150 (ORP150) in pancreatic injury of rats with severe acute pancreatitis. MethodsForty male Wistar rats were randomly allocated into two groups: sham operation group (SO group, n=10) and severe acute pancreatitis model group (SAP group, 3 h, 6 h, and 12 h after modeling, each time n=10). SO group rats were only turned over the pancreas, and the SAP group rats were induced by retrogradely infusing 5% sodium taurocholate into the biliopancreatic duct. SO group rats were killed at 12 h after sham operation, and the SAP group rats were killed at 3 h, 6 h, and 12 h after modeling. Blood samples were obtained for detecting the amylase (AMY) and alanine transarninase (ALT) levels. The quantity of ascites were collected and measured. Pancreatic tissue samples were stained with hematoxylin and eosin for histopathological evaluation. Pancreatic tissue was collected to detect the expressive quantity of ORP150 mRNA by RT-PCR. ResultsThe quantity of ascites, AMY and ALT levels, and histopathological evaluation were significantly higher in SAP group than those in SO group (Plt;0.05). AMY and ALT levels, histopathological detection, and expression of ORP150 mRNA in pancreatic rats among 3 h, 6 h, and 12 h after modeling were significantly different from each other (Plt;0.05), except for ascites. The ascites were not significantly different between 3 h and 6 h after modeling (Pgt;0.05), while 12 h were significantly higher than those at 3 h and 6 h (Plt;0.05). The expression of ORP150 mRNA was low in SO group, and were rise in subgroup SAP 12 h, 6 h, and 3 h gradually. Subgroup was statistical difference (Plt;0.05). ConclusionThe expressive quantity of ORP150 mRNA is high in pancreatic tissues with SAP rats, prompting that ORP150 may play a role in pancreatic injury with SAP.

  • Effect of Inducible Nitric Oxide Synthase and Endothelial Nitric Oxide Synthase mRNA Expressions on Lung Injury of Rats with Acute Necrotizing Pancreatitis 

    Objective Observing the expressions of inducible nitric oxide synthase (iNOS) and endothelial nitric oxide synthase (eNOS) mRNA in lung tissues of rats with acute necrotizing pancreatitis (ANP) to explore the role of NOS in ANP associated-lung injury. Methods Forty Wistar rats were assigned into ANP group (n=30) and sham-operation group (SO group, n=10). ANP model was induced by retrograde injection of 5% sodium taurocholate into the bili-pancreatic duct. Pathological changes of the lung tissue were observed under light microscope at 3 h, 6 h and 12 h after the ANP-model operation, and the expressions of iNOS mRNA and eNOS mRNA in lung tissue were assayed by RT-PCR. Results Different degrees of pathological changes of the lung tissue, such as hyperemia, edema, inflammatory cells infiltration, hemorrhage and necrosis, were found in the ANP group. The pathologic injury scores of lung tissue in ANP group were higher than that in SO group (Plt;0.05), and gradually increased with the duration extension of ANP (Plt;0.05). Compared with the SO group, the expressions of iNOS and eNOS mRNA in ANP group were all higher at 3 h, 6 h, and 12 h (Plt;0.05). Conclusions The overexpressions of iNOS and eNOS mRNA may play important roles in lung injury of ANP. This provides us a theory basis that lung injury of ANP could be relieved by inhibiting the expressions of iNOS and eNOS mRNA.
