  • Methodological Evaluation on Domestic Clinical Trials on Traditional Chinese Medicine Nursing from 2006 to 2011

    Objective To evaluate the methodological quality of clinical trials on traditional Chinese medicine (TCM) nursing in recent six years.Methods Such databases as CNKI, VIP, WanFang Data and CBM were searched for collecting clinical trials on TCM nursing published from January 2006 to September 2011, and domestic primary nursing journals were also searched from January 2010 and September 2011. Methodological quality of included studies was assessed using quality assessment criteria of the Cochrane systematic review guideline. Results A total of 854 clinical trials were retrieved, including 706 (82.7%) randomized controlled trials (RCTs), 108 (12.6%) quasi-randomized controlled trials and 40 (4.7%) non-randomized controlled trials. In the methodological quality analysis, the comparability of baseline was mentioned in 784 trials (91.8%), a total of 498 (58.3%) reported definite diagnosis criteria. 178 (20.8%) reported exclusive criteria. 831 studies (97.3%) applied relevant statistical methods properly. However, only 55 trials (6.4%) mentioned the method of randomization sequence. 10 studies (1.2%) described the method of randomiztion assignment. Blinding was mentioned in 22 studies (2.6%). 98 trials (11.5%) did prospective follow-up. 93 trials (10.9%) had safety description. 20 trials (2.3%) reported lost and with drawl cases, but only 2 conducted intention-to-treat analysis. It was hard to determine whether there was selective reporting bias or not because all the studies did not have protocols. Only 21 studies (2.5%) mentioned the lack of outcome indicators which could be the evidence for existing of bias. By annual analysis, there were 81 trials which conformed to at least 2 low risk criteria. 10 trials (12.3%) was published in 2009, 26 trials (32.1%) published in 2010, and 27 trials published by September 2011, indicated an uptrend. Conclusions According to the Cochrane Collaboration’s tool for assessing risk of bias, the overall quality of clinical trials on TCM nursing is low with defects in different degrees, but it rises gradually over years.

  • Reporting and Methodological Quality of Systematic Reviews and Meta-Analyses in Nursing Field in China

    Objective To evaluate reporting and methodological quality of systematic reviews or meta-analyses in nursing field in China. Methods CNKI database was searched for systematic reviews or meta-analyses in nursing field from the establishment date to December 2011. Two reviewers independently identified the literature according to inclusion and exclusion criteria, and then extracted the data using Excel software. The PRISMA and AMSTAR checklists were used to assess reporting characteristics and methodological quality, respectively. Results A total of 63 systematic reviews or meta-analyses involving 21 systematic reviews and 42 meta-analyses were identified. These articles were published on 13 journals such as The Chinese Nursing Research, the Chinese Journal of Nursing, and the Chinese Journal of Evidence-Based Medicine. The deficiencies of methodological quality mainly contained literature search, heterogeneity handling, recognition and assessment of publication bias. In addition, the deficiencies of reporting characteristics were reflected on incomplete reporting of literature search, quality assessment, risk of bias and results (some studies lacked forest plot, estimated value of pooled results, 95%CI or heterogeneity). Conclusion As a whole, the included reviews and meta-analyses have more or less flaws with regard to the quality of reporting and methodology based on the PRISMA and AMSTAR checklists. Focusing on the improvement of reporting and methodological quality of systematic review or meta-analysis in nursing field in China is urgently needed in order to increase the value of these studies.

  • Clinical Efficacy of Zoledronic Acid on Postmenopausal Osteoporosis

    ObjectiveTo observe the treatment effects of zoledronic acid on postmenopausal osteoporosis. MethodsSeventy-two postmenopausal osteoporosis patients from July 2007 to December 2010 were randomly divided into observation group and control group, with 36 patients in each. Traditional drug treatment was used in the control group, while traditional drug treatment and zoledronic acid were used for patients in the observation group. The Indicator of bone mineral density (BMD) and ostocalcin were used to comapre the treatment effects between the two groups after one-year treatment. ResultsThere was a significant difference in BMD and osteocalcin in both the observation group and the control group before and after treatment (P<0.05). The treatment effect is superior in the observation group (P<0.05). ConclusionZoledronic acid is an effective treatment for postmenopausal osteoporosis; it can increase BMD and osteocalcin more effectively.

  • The quality of qualitative research reports published by high-impact journals in the field of nursing in China from 2016 to 2018

    ObjectivesTo evaluate the reporting quality of individual/focus group interviews published in nursing journals included in the Chinese science citation database (CSCD).MethodsCSCD database was electronically searched to collect qualitative studies published in nursing journals included in CSCD from January 2016 to December 2018. Two researchers independently screened literatures, extracted data, and assessed the quality of included studies by COREQ guidelines, SPSS 25.0 software was then used for statistical analysis.ResultsA total of 223 qualitative researches were included. The results of COREQ evaluation showed that full report rate of 43.8% (14/32) studies were less than 50%, 78.6% (11/14) were less than 20%, and the full report rate of item 4 was even 0%. There were no significant differences between different total cites and the availability of funding in COREQ guidelines.ConclusionThe reporting quality of the qualitative researches published in the four nursing journals included in the CSCD requires further improvement. Therefore, we suggest that domestic nursing journals should introduce COREQ guidelines in contribution and strictly implement it in editorial review and peer review.

  • Evaluation of the Treatment Effect of Off-pump Coronary Artery Bypass Surgery and Summary of the Nursing Care in the Treatment

    ObjectiveTo investigate the clinical efficacy of off-pump coronary artery bypass grafting, and summarize the experience of nursing care in the treatment. MethodsWe retrospectively analyzed the clinical data of 25 patients undergoing off-pump coronary artery bypass surgery between January 2010 and December 2012. The therapeutic effect of the surgical method and the care management measures were summarized and analyzed. ResultsAll the operations were successful without any deaths. The Mean operation time was (209.85±38.41) miniutes; Blood infusion volume averaged (1 311.23±423.70) mL; Drainage volume was (491.98±142.46) mL; The mean ICU monitoring time was (4.22±1.23) days; Hospitalization lasted for (10.10±1.89) days; And the mean number of bypass was 4.00±0.51. Two patients had high temperature and coughing after surgery, and were cured through intravenous dripping and atomization sputum suction. Two had slight injury infection and were controlled with antibiotics. Two could not endure the pain which was relieved by sauteralgyl. One patient with thoracic pain due to neural stimulation was relieved by glycerin trinitrate. ConclusionThe off-pump coronary artery bypass surgery is advantageous in having less trauma, less bleeding, shorter hospital stay and lower hospital costs. The preoperative, intraoperative and postoperative care should be prepared to ensure the smooth implementation of the surgery and improve therapeutic effect.

  • Observation and Nursing of Hypotension for Patients during Anesthesia Recovery Period

    ObjectiveTo observe the incidence of hypotension in general anesthesia patients in post anesthesia care unit (PACU), compare the blood pressure before and after treatment, and analyze hypotension causes. MethodBetween September 2012 and July 2013, data of 18 931 patients in PACU after general anesthesia were retrospectively analyzed for the incidence of hypotension. At the same time, the change of blood pressure was also detected before and after treatment. ResultsSixty-two patients had hypotension during the anesthesia recovery period after general anesthesia. The incidence was 0.33%. The proportion was the highest among patients who underwent abdominal surgery, reaching 41.9%. Systolic pressure and diastolic pressure improved significantly after treatment (P<0.05). ConclusionsIt is important to perform comprehensive evaluation of vital signs, closely observe disease conditions and vital sign changes, find out and manage low blood pressure for patients in PACU, which secure their safety.

  • An Investigation of the Status of Clinical Learning Environment in Undergraduate Nursing Students' in the late Clinical Practice Stage in Sichuan Province

    ObjectiveTo explore the status of clinical learning environment in undergraduate nursing students in the late clinical practice stage in Sichuan Province. MethodThe sampling was performed in a layered-randomized and geometric proportional way. A total of 1 027 nursing students from 6 nursing schools in Sichuan Province between May and July 2012 were included. Data were collected by the self-rating questionnaires, including demographic sheet, and the nursing clinical learning environment scale. ResultsThe general score of evaluation for clinical learning environment by the nursing undergraduates was 144.99±26.60, and the average score of the items was 3.45±0.63. The average scores were 3.41±0.74, 3.59±0.64, 3.43±0.69, 3.45±0.73, 3.35±0.77, and 3.49±0.69, representing the personality, task orientation, working atmosphere and team spirit, innovation, interpersonal relation, and student participation, respectively. ConclusionsThe assessments about clinical learning environment of nursing undergraduates in Sichuan province are medium grade and the evaluation dimensions of personality and interpersonal relationships low. It is necessary to improve clinical learning environment in the future.

  • Survey on Nursing Professional Competence and Its Influencing Factors of Fulltime System Graduated Nurses

    Objective To evaluate the level of nursing professional competence of fulltime system graduated nurses in Sichuan province and determine its influencing factors, so as to provide references for nursing high-education reform and nursing human resource management. Methods With a self-designed questionnaire and stratified cluster sample method, both graduated nurses and their direct nursing managers were investigated and asked to evaluate nursing professional competence. Results The mean scores of nursing professional competence of the nurses’ self-evaluation and their managers’ evaluation were 69.90±10.27 and 68.96±11.33, respectively. Factors influencing nursing professional competence included individual interest in nursing, graduated institution, working position, and training provided by employer. Conclusion The professional competence of the nurses graduated with full-time bachelors of clinical nursing in Sichuan province is close to the intermediate level, and it still needs to be improved on the whole, especially in the aspects of nursing research and management. Besides, nursing institutions should further enhance the construction of inner teaching environment and the professional thoughts education of nurse students, while the employer should offer more professional development opportunities, so as to improve the professional self-identity and competence, and to finally lay the talent foundation for the development of nursing disciplines.

  • The mediating effect of nursing intervention of sleep hygiene on insomnia in patients with liver cirrhosis

    Objective To investigate the mediating effect of nursing intervention of sleep hygiene on insomnia in patients with liver cirrhosis. Methods One hundred and eighteen cirrhotic patients with grade-A liver function and insomnia treated between June 2012 and January 2015 were divided into control group (n=52) and intervention group (n=66) with random number table method. The control group received conventional nursing only, while the intervention group was given nursing intervention of sleep hygiene. The sleep quality of patients in the two groups on admission and discharge were surveyed and compared based on Pittsburgh sleep quality index (PSQI). The nursing effect was also compared between the two groups on discharge. Results There was no statistically significant difference in total score and scores for each factor of PSQI between the two groups of patients on admission (P>0.05). However, the total score and scores for each factor of PSQI on discharge in both groups were significantly lower than those on admission (P<0.05). Besides, the scores in the intervention group were significantly lower than those in the control group (P<0.05). The effect of nursing in the intervention group was superior to that in the control group (P<0.001). Conclusions Nursing intervention of sleep hygiene can effectively improve sleep quality in cirrhotic patients with grade-A liver function. The role of nursing intervention in the treatment of insomnia should be taken seriously.
