  • Clinical Observation of Splenectomy on Chronic Idiopathic Thrombocytopenic Purpura

    ObjectiveTo study the results of splenectomy in patients with idiopathic thrombocytopenic purpura. MethodsSeven patients who failed to respond to conservative management were treated with splenectomy and followed up for 6 months to 8 years (1990~1999).ResultsThe presplenectomy patients had symptoms of bleeding and their platelet count on average was 32×109/L. The 3th,7th day and 1th,2th, 6th month after splenectomy, the average platelet count was 191×109/L,354×109/L,317×109/L,200×109/L and 151×109/L respectively. Their platelet recovered to normal during a week in 7 cases (≥100×109/L); In 6 patients the platelet count was normal in the 6th month after splenectomy, the success rate was 6/7, the rate of remission was 1/6. The platelet count after splenectomy was significantly higher than that before splenectomy.ConclusionThere are no correlation between the course of disease before splenectomy and the results of splenectomy. Splenectomy is safe and effective in the treatment of idiopathic thrombocytopenic purpura.

  • Research of Changes of Platelet Count after Splenectomy in Patients with Splenic Rupture or Cirrhosis

    Objective To compare the difference of the changes of platelet counts after splenectomy between the patients with splenic rupture and patients with cirrhosis and portal hypertension, and to analyze the possible reasons and clinical significance. Methods The platelet count of 47 splenic rupture patients and 36 cirrhosis patients who had been carried out splenectomy from July 2008 to December 2009 in our hospital were counted, and the differences in platelet count and it’s change tendency of two groups were compared. Results In the splenic rupture group,the platelet count of all 47 patients increased abnormally after operation, the maxlmum value of platelet count among 300×109/L-600×109/L in 6 cases,600×109/L-900×109/L in 21 cases,and above 900×109/L in 20 cases. In the cirrhosis group,the maxlmum value of platelet count after operation was above 300×109/L in 26 cases,100×109/L-300×109/L in 8 cases,and below 100×109/L in 2 cases. The difference of maxlmum value of platelet count in the two groups had statistic significance(P=0.00). Compared with the cirrhosis group, the platelet count increased more significant and decreased more slow in splenic rupture group(P<0.05).The abnormal days and rising range of platelet count were higher in patient with Child A than Child B and C(P=0.006,P=0.002). Conclusions The change of platelet count after operation in splenic rupture group was obviously different from cirrhosis group because of the difference of the liver function and body situation of patients. To patients with splenic rupture or cirrhosis, appropriate treatment based on the platelet count and liver function could obtain good therapeutic effect.

  • CHANGES OF PLATELET ANTIBODY-IgG LEVEL IN PATIENTS WITH CONGESTIVE SPLENOMEGALY AND HYPERSPLENISM AFTER DIFFERENT SPLENIC OPERATIONS

    Objective To defect the level of platelet antibody-IgG (PA-IgG) in patients with congestive splenomegaly and hypersplenism and the change of PA-IgG level after splenectomy and subtotal splenectomy. Methods Twenty four cases of congestive splenomegaly and hypersplenism were investigated. Results The level of PA-IgG in 24 cases were higher than normal range (P<0.01), while the platelet count were lower than normal range and there was a significant negative correlation between the level of PA-IgG and platelet count (r=-0.4747, P<0.05). After subtotal splenectomy or splenectomy, the level of PA-IgG descended, the platelet count raised and the negative correlation between the level of PA-IgG and platelet count disappeared. Conclusion The results suggest that there is a immunoregulation between PA-IgG and platelet. Perhaps spleen has some relation with the immunoregulation.

  • Clinical Application of Totally Laparoscopic Splenectomy Via Anterior Approach

    ObjectiveTo investigate clinical application value of totally laparoscopic splenectomy via anterior approach. MethodsThe clinical data of 26 consecutive patients underwent totally laparoscopic splenectomy via anterior approach from March 2012 to June 2015 in this hospital were analyzed retrospectively. There were 11 males and 15 females. These patients included 2 cases of traumatic spleen ruptures, 4 cases of idiopathic thrombocytopenic purpuras, 1 case of hereditary spherocytosis, 3 cases of splenic cysts, 5 cases of autoimmunity hemolytic anemias, 2 cases of splenic hemangioma, 1 case of hematolymphangioma, 8 cases of cirrhosis portal hypertensions. The operative time, intraoperative bleeding, postoperative time to out-of-bed activity, the first flatus/bowel motion time, complications, and hospital stay were analyzed retrospectively. ResultsTwenty-six patients were successfully carried out totally laparoscopic splenectomy via anterior approach. The average operation time was 93 min (72-120 min). The average blood loss was 60 mL (10-80 mL). The postoperative time to out-of-bed activity was 24 h. The first flatus/bowel motion time was 2-3 d. The average hospital stay was 7 d (6-9 d). The postoperative pancreatic fistula (Grade A) occurred in 1 patient, who recovered well on day 6 by postoperative drainage management. After 5-32 months of following-up, all patients recovered smoothly without any long-term complications. ConclusionThe preliminary results of limited cases in this study show that totally laparoscopic splenectomy via anterior approach is feasible, safe, and minimally invasive.

  • Retaining of Spleen and Sweeping of Lymph Nodes of No.10 and No.11 During the Operation of Gastric Carcinoma

    ObjectiveTo study the indication and means in dissection lymph nodes of the No.10 and No.11 without splenectomy in radical gastrectomy for gastric cancer. MethodsAccording to the location, type of pathology, clinical and pathological classification, lymphatic drainage and spread of gastric carcinoma togather with the immunological function of spleen, selection of operative procedure without splenectomy should be considered, so the related literatures were reviewed. ResultsRetained spleen had been shown to improve 5year survival of patients with gastric cancer of stage Ⅰ,Ⅱ and Ⅲ,splenectomy had been shown to improve 5year survival of patients with gastric cancer of stage Ⅳ,whose carcinoma was infiltrating splenic and the lymph nodes of the No.10. The complications of different means of dissection of the lymph nodes made no difference.Conclusion Dissection of the lymph node without retained spleen or allogenic spleen transplantation is indicated for the patients with cancer of stage Ⅳ,whose spleen is invaded by the tumor.

  • Surgical Treatment for Primary Hepatocellular Carcinoma Associated with Hypersplenism

    【Abstract】ObjectiveTo explore the appropriate surgical management of the primary hepatocellular carcinoma with hypersplenism. MethodsOf 67 patients who has primary hepatocellular carcinoma with hypersplenism, 17 cases had hepatectomy combined with splenectomy, 7 cases had hepatectomy only, and the other 43 patients were treated with hepatic artery embolization and splenic artery embolization. ResultsThe symptoms of hypersplenism disappeared and the hemogram became normal 30 d after operation in 17 patients who had hepatectomy combined with splenectomy, but worsened in 7 patients who only had simple hepatectomy and 6 cases of those patients were treated with splenic artery embolization 3-7 months after operation. In 43 patients treated with hepatic artery embolization and splenic artery embolization, 79%(34/43)had improved hypersplenism symptoms and the hemogram became normal. ConclusionThe treatment of primary hepatocellular carcinoma with hypersplenism should be strived for hepatectomy combined with splenectomy. If the liver mass cannot be resected, hepatic artery embolization and splenic artery embolization should be chosen.

  • Effect of Splenectomy in Prognosis of Human Liver Transplantation

    【Abstract】Objective To explore the effect and indication of splenectomy in liver transplantation. Methods From January 2001 to April 2006, 260 patients underwent piggyback orthotopic liver transplantation (PBOLT), and 28 patients had undergone combined PBOLT and splenectomy (splenectomy group). These patients were compared to 56 randomly selected non-splenectomy patients from the same transplant period, meaningly two controls were selected for every non-splenectomy case. Two groups were analyzed with respect to rate of infection and survival rate, as well as biopsy-proven acute allograft rejection within 30 days after transplantation. Results Rate of infection in the splenectomy group was higher than that in the non-splenectomy patients (85.7% vs 55.4%, P<0.05). Acute rejection and survival rates in the splenectomy group were lower than those in the non-splenectomy patients (3.6% vs 14.3%, P<0.05; 46.4% vs 82.1%, P<0.05). Conclusion Concomitant splenectomy with PBOLT has a significantly higher patient mortality rate; it is mainly due to its septic complications. At present, unless there is a certain indication for splenectomy, this procedure is not recommended.

  • Effects of Hand Assistant Laparoscopic Splenectomy Plus Pericardial Devascularization on Systemic Stress Responses

    Objective To investigate the effects of hand assistant laporoscopic splenectomy plus pericardial devascularization on systemic stress responses. Methods Forty patients with cirrhotic portal hypertension were selected, 20 cases of which were underwent hand assistant laparoscopic splenectomy plus pericardial devascularization (LAP group), and the other 20 were underwent open splenectomy plus pericardial devascularization (OP group). The levels of blood glucose (BG), insulin (Ins), triiodothyronine (T3), tetraiodothyronine (T4), corticosteroid (CS) and other related clinical data were measured before operation and on day 1-3 after operation, which were compared between two groups. Results There was no statistical significance between two groups on those levels before operation. On day 1 after operation, BG and CS level in both two groups were higher than those before operation (P<0.05), but they were recovered on day 2 after operation in LAP group (Pgt;0.05), and on day 3 after operation in OP group (Pgt;0.05). BG and CS level in OP group were markedly higher than those in LAP group on day 2 after operation (P<0.05). On day 1 after operation, Ins, T3 and T4 level of two groups were lower than those before operation (P<0.05), but they were recovered on day 2 after operation in LAP group (Pgt;0.05) and on day 3 after operation in OP group (Pgt;0.05). Ins, T3 and T4 level in OP group were lower than those in LAP group on day 2 after operation (P<0.05). There was no significant difference in operation time between two groups (Pgt;0.05). But laparoscopic surgery had more advantages than conventional open surgery such as reducing bleeding quantity in operation, shortening recovery time of bowel and urinary bladder function and the length of stay. Conclusion Compared with laparotomy, the laparoscope not only imposes less impact on physical stress system, but also makes recovery after operation more quickly.

  • MANAGEMENT OF COMPLEX INTRAHEPATOLITHIASIS WITH BILIARY LIVER CIRRHOSIS AND PORTAL HYPERTENSION

    Objective To discuss the therapeutic effectiveness of surgical approach to complex intrahepatolithiasis with biliary liver cirrhosis.Methods A case of complex intrahepatolithiasis with biliary liver cirrohosis, portal hypertension was treated with splenectomy and pericardial devascularization plus left hepatectomy and portal cholangio plasty with T tube drainage. Results Follow up one year and a half after operation, no symptom of cholangitis was found, and there is no relapse up to date. Conclusion Combined operation of hepatectomy with splenectomy is an ideal and effective treatment for complex intrahepatolithiasis with biliary liver cirrhosis.

  • Intraoperative Monitor and Modulation of Portal Vein Hemodynamics During Living Donor Liver Transplantation to Prevent Small-for-Size-Syndrome 

    Objective To analyze the effect of monitoring and modulating the portal vein pressure and blood flow during living donor liver transplantation (LDLT) on preventing small-for-size-syndrome (SFSS). Methods Data of forty-four LDLT recipients between Oct.2007 and Oct.2008 were reviewed. Actual graft-to-recipient weight ratio(GRWR), portal vein flow and pressure during operation and syndrome of SFSS after operation were recorded. The patients received splenectomy or splenic artery ligation according to actual GRWR, portal vein flow and pressure and WBC. Relationships between patients’ GRWR, portal vein flow, portal vein pressure and occurrence of SFSS were analyzed. Results Six patients received splenectomy and 7 patients received splenic artery ligation to decrease the portal vein flow and pressure during the operation. The portal vein flow and pressure decreased after splenectomy (Plt;0.05). The portal vein pressure decreased (Plt;0.05) and the portal vein flow had no significant change after splenic artery ligation (P>0.05). No SFSS occurred after operation. Conclusion Modulation of portal vein flow and pressure by splenectomy or splenic artery ligation during LDLT operation can decrease the portal vein flow and pressure, and which can prevent the incidence of SFSS.
