  • Effectiveness and Safety of Olanzapine Combined with Fluoxetine for Refractory Depression: A Systematic Review

    Objective  To evaluate the effectiveness and safety of both olanzapine combined with fluoxetine (combination therapy) and fluoxetine (monotherapy) for refractory depression. Methods According to the computer retrieval from PubMed (1966 to September 2009), Cochrane Library (Issue 3, 2009), EMbase (1974 to September 2009), SCI (1974 to September 2009), CNKI (1994 to September 2009), CBM (1978 to September 2009), CSJD (1989 to September 2009) and Wanfang Database (1997 to September 2009), and the manual retrieval from related journals and conference proceedings were conducted, to include randomized controlled trials of comparison in between olanzapine combined with fluoxetine and fluoxetine in treating refractory depression. We collected the valid data after assessing the methodology quality of included studies on the basis of Jadad scoring standard, and conducted meta-analysis with RevMan 5.0 software. Results A total of 7 studies with 1 230 patients were included. The meta-analysis showed that, there was no significant difference between two groups about the scores of HAMA (Hamilton Anxiety Scale) at the end of the 1st week, but the olanzapine combined with fluoxetine in trial group was much better for relieving anxiety situation compared to fluoxetine in control group at the end of the 2nd, 4th, 8th and 12th week. In accordance with the scores of CGI (Clinical Global Impression Scale), there was no significant difference at the end of 2nd and 4th week after treatment, but there was a significant difference at the end of 8th and 12th week. As to the changes of MADRS (Montgomery and Asberg Depression Rating Scale), the trial group was much distinct than control group at the end of the 1st, 2nd, 4th and 8th week. In summary, the clinical effect of trial group was superior to that of control group, and there was no significant difference in adverse reactions between two groups (RR=1.10, 95%CI 0.99 to 1.23). Conclusion Current evidence shows that, the clinical effect and safety of olanzapine combined with fluoxetine in treating refractory depression is obviously superior to that of fluoxetine.

  • Expression of MicroRNA-1, 21 in Ischemic Preconditioning, Ischemic Postconditioning and Remote Ischemic Preconditioning in an Isolated Rat Heart Model

    Abstract: Objective To observe the expression changes of microRNA 1 (miRNA-1) and microRNA 21(miRNA-21) after ischemic preconditioning (IPC), ischemic postconditioning (IPO) and remote ischemic preconditioning (RIPC)in an ischemia-reperfusion rat heart model in vitro, as well as the expression of their target protein heat shock protein 70 (HSP70) and programmed cell death 4 (PDCD4), and evaluate whether miRNA are involved in endogenous cardio-protective mechanism. Methods The Langendorff-perfused Sprague-Dawley rat hearts were randomly assigned into one of the four groups, control group (CON group, n=12), ischemia preconditioning group (IPC group, n=12) , ischemia postconditioning group (IPO group, n=12) and remote ischemia preconditioning group (RIPC group,n=12). Cardiac function was digitalized and analyzed. The expression of HSP70, PDCD4, B-cell lymphoma/leukemia-2 (Bcl-2) and Bax was detected by Western blotting. The expression of miRNA-1 and miRNA-21 was detected by real-time reverse transcriotion-polymerase chain reaction (RT-PCR). Assessment of cardiac infarct size and myocardial apoptosis was determined using triphenyltetrazolium chloride (TTC) assay and terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick end labeling assay (TUNEL) assay respectively. Results The expressions of miRNA-1 and miRNA-21 were up-regulated in IPC group, but the expression of miRNA-1 was down-regulated in RIPC group and IPO group (P<0.05). The expressionsof PDCD4, HSP70 and Bax were down-regulated in ‘conditioning’ groups compared with CON group (P<0.05). The expression of Bcl-2 was not statistically different among the four groups. The infarct size and the myocardial apoptosis in ‘conditioning’ hearts were significantly decreased compared with CON group (P<0.05). Conclusion The expressions of the miRNA-1 and miRNA-21 are different in IPC, RIPC and IPO groups, and their target proteins are not inversely correlated with the miRNAs in all the ‘conditioning’ groups.

  • The value of biphasic CT in the diagnosis of chronic obstructive pulmonary disease

    Objective To investigate the feasibility of diagnosis of potential chronic obstructive pulmonary disease (COPD) patients who cannot finish the pulmonary function test via biphasic CT scan. Methods Sixty-seven male individuals aged 43 to 74 (57.0±5.9) years were divided into a COPD group (n=26) and a control group (n=41). All individuals underwent biphasic quantitative CT scan for calculating the proportion of emphysema, functional small airway disease, and normal component of the whole lung and each lobe. Results Based on principle component analysis, two principal components “imaging feature function 1 and imaging feature function 2” were calculated and analyzed by logistic regression, which found that imaging feature function 1 was an independent risk factor of COPD (odds ratio=8.749, P<0.001), and imaging features function 1 could be used to assist the diagnosis of COPD (area under receiver operating characteristic curve=0.843, P<0.001). Conclusion Imaging features function 1 is an independent risk factor for COPD and can assist the diagnosis of COPD.
