  • Arthroscopic medial patellofemoral ligament reconstruction combined with tibial tuberosity transfer for recurrent patellar dislocation

    ObjectiveTo investigate the effectiveness of arthroscopic medial patellofemoral ligament (MPFL) reconstruction combined with tibial tuberosity transfer for recurrent patellar dislocation.MethodsBetween February 2012 and December 2013, 24 patients (24 knees) with recurrent patellar dislocation were treated with arthroscopic MPFL reconstruction combined with tibial tuberosity transfer. There were 7 males and 17 females, with a mean age of 23.2 years (range, 18-37 years). One patient had recurrence dislocation after operation in the other hospital, and the others were the first operation. The disease duration ranged from 6 months to 20 years (mean, 5.6 years). The patellar apprehension tests were positive. The preoperative Lysholm score was 49.79±11.67 and the Kujala score was 49.63±6.28. X-ray films showed that 13 patients had dysplasia of the patella and femoral trochlea; 8 patients had high tibia (Caton-Deschamps index>1.2); the congruence angle was (23.96±5.54)°. CT examination showed that the tibial tuberosity-trochlear groove distance (TT-TG) value was (23.71±2.35) mm.ResultsAll incisions healed by first intention. Twenty-two patients were followed up 59-81 months, with an average of 66.8 months. No dislocation occurred during the follow-up period. The patellar apprehension tests were negative. At 1 week after operation, the results of X-ray films and CT showed that the congruence angle angle was (–1.96±4.65)°, and the TT-TG value was (13.75±1.89) mm, which were significantly lower than those before operation (P<0.05). At 6 months, 1 year, and last follow-up, Lysholm scores were 81.13±17.76, 91.35±3.60, and 92.23±2.71, respectively; and Kujala scores were 84.04±3.98, 91.48±3.64, and 91.45±3.29, respectively. The Lysholm and Kujala scores were significantly increased after operation when compared with the preoperative scores (P<0.05). At last follow-up, the effectiveness was excellent in 11 cases, good in 8 cases, and fair in 3 cases, with an excellent and good rate of 86%.ConclusionArthroscopic MPFL reconstruction combined with tibial tuberosity transfer for recurrent patellar dislocation can effectively improve the patellofemoral joint matching relationship and has a satisfactory short- and medium-term effectiveness.

  • Emergency cosmetic suturing of 210 children with maxillofacial skin lacerations

    Objective To analyze the case data of children with maxillofacial skin lacerations and provide a reference for the diagnosis and treatment of maxillofacial skin lacerations in children. Methods The medical records of children with maxillofacial skin lacerations who underwent emergency cosmetic suturing at the Lingang Branch of Yibin Second People’s Hospital between March 2023 and March 2024 were retrospectively collected. The clinical characteristics, diagnosis, treatment, and follow-up of the children were analyzed and summarized. Results A total of 210 children with maxillofacial skin lacerations were included, of which 142 were boys, accounting for 67.62%. The age of the children was most common at 3 years old, accounting for 18.57% (39 cases). A total of 5 children had two lacerations, 3 cases of which were in the same location and 2 cases were not in the same location. The most common site of skin laceration in children was the forehead, accounting for 46.23% (98 cases). The most common cause of maxillofacial skin lacerations was spontaneous falls, accounting for 51.43% (108 cases). The maxillofacial skin lacerations of the children were mainly minor injuries (168 cases, accounting for 80.00%). All children underwent modified vertical mattress suture, and the postoperative satisfaction of the children families was relatively high. Finally, 37 children were successfully followed up, and the follow-up results showed that the wound healing of the maxillofacial skin lacerations were good, the skin scars were not obvious, and the appearance of the skins were basically normal. Conclusion For children with maxillofacial skin lacerations, using the modified vertical mattress suture for emergency cosmetic suturing is beneficial for their injury recovery and facial aesthetics.

  • Effectiveness of medial open wedge high tibial osteotomy combined with posterior slope angle of tibial plateau correction in treatment of osteoarthritis of limited flexion knee with varus deformity

    ObjectiveTo explore the short-term effectiveness of medial open wedge high tibial osteotomy (OWHTO) combined with posterior slope angle of tibial plateau correction to treat the osteoarthritis of limited flexion knee with varus deformity.MethodsThe data of 18 cases (18 knee) with osteoarthritis of limited flexion knee with varus deformity between January 2014 and July 2016 were analyzed retrospectively. There were 6 males and 12 females with an average age of 54.9 years (range, 48-64 years). There were 8 cases of left knee and 10 cases of right knee. The varus of knee ranged from 7.45 to 15.52° (mean, 10.63°). According to Kellgren-Lawrence grading standard, there were 4 cases of grade Ⅱ and 14 of grade Ⅲ. OWHTO was used to adjust the varus deformity, and the posterior slope angle of tibial plateau was adjusted to solve the limited flexion.ResultsThe thickness of osteotomy was 10-19 mm (mean, 14.91 mm). The operation time was 1.2-2.0 hours (mean, 1.4 hours). All incisions healed by first intension. All patients were followed up 1.0-2.5 years, with an average of 1.5 years. At last follow-up, the range of knee flexion and Lysholm score, Hospital for Special Surgery (HSS) score, and International Knee Documentation Committee (IKDC) score were significantly higher than preoperative ones, showing significant differences (P<0.05). X-ray films showed that the osteotomy healed at 3- 7 months (mean, 3.6 months) after operation. At last follow-up, the limb alignment by the relative position of tibial plateau and the posterior slope angle of tibial plateau were significantly improved, showing significant differences when compared with preoperative ones (P<0.05).ConclusionThe OWHTO combined with posterior slope angle of tibial plateau correction can significantly improve the range of flexion and functional score in short-term.
