  • Technical points of external fenestration in the treatment of aortic dissection involved visceral branch arteries after EVAR

    Objective To summarize the advantages and key points of external fenestration in the treatment of aortic dissection involved visceral branch arteries after endovascular aortic repair (EVAR), and to explore the application effect of external fenestration in aortic dissection involved visceral branch arteries. Methods A patient with abdominal aortic aneurysm resulting in abdominal aortic dissection and involving multiple visceral arteries after EVAR was treated in Center of Vascular and Interventional Surgery, Department of General Surgery, The Third People’s Hospital of Chengdu. The surgical procedure of this patient was summarized, and the current status of total lumen technique in the treatment of such diseases was discussed and analyzed. Results The operation was successful, and it took only five hours, the intraoperative blood loss was about 100 mL, the patient was kept in ICU for one day and discharged one week after surgery and no serious postoperative complications occurred (such as spinal cord ischemia, liver and kidney insufficiency, infection, lower limb ischemia, puncture pseudoaneurysm, etc.). Aortic CT angiography was reexamined in three months after surgery, and the three-dimensional reconstruction showed that the aortic stent was stable, the blood flow of visceral branch arteries was smooth, and the aortic dissection was well isolated. Conclusion Endovascular repair of aortic dissection involving branch arteries of important organs can be achieved by external fenestration technique, it is a new treatment for aortic lesions involved visceral branch arteries.

  • Progress of intraluminal treatment for isolated superior mesenteric artery dissection

    ObjectiveTo explore the current research status of endovascular treatment for isolated superior mesenteric artery dissection (ISMAD) so as to provide a reference for vascular surgeons and patients in selecting treatment methods. MethodThe recent domestic and international literature on the endovascular treatment for ISMAD was collected and summarized. ResultsThe current endovascular treatment methods for ISMAD mainly included catheter-directed thrombolysis and stent placement. For the ISMAD patients with true intraluminal thrombus formation, true luminal occlusion, and partial embolization of branch vessels, the catheter-directed thrombolysis was performed by continuously pumping urokinase locally to dissolve thrombus and improve distal blood supply. The stent placement mainly included two main types: bare stent placement and covered stent placement, both aiming to promote true lumen recanalization, false lumen thrombosis, and vascular remodeling through mechanical support and blood flow redirection. The bare stents had unique advantages such as being well-developed, offering a wide range of options, and having stable performance. However, covered stents were superior to bare stents in terms of mid-term patency rate and in-stent restenosis. ConclusionsWith the development of technology, endovascular treatment has become an indispensable method for ISMAD patients. Extensive multicenter cases reports from both domestic and international studies have supported the safety and efficacy of endovascular treatment. Catheter-directed thrombolysis and stent placement (bare stent and covered stent) offer targeted solutions for various types of ISMAD patients, providing more minimally invasive and effective treatment.

  • Mortaligy risk prediction models for acute type A aortic dissection: a systematic review

    ObjectiveTo systematically review mortality risk prediction models for acute type A aortic dissection (AAAD). MethodsPubMed, EMbase, Web of Science, CNKI, WanFang Data, VIP and CBM databases were electronically searched to collect studies of mortality risk prediction models for AAAD from inception to July 31th, 2021. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Systematic review was then performed. ResultsA total of 19 studies were included, of which 15 developed prediction models. The performance of prediction models varied substantially (AUC were 0.56 to 0.92). Only 6 studies reported calibration statistics, and all models had high risk of bias. ConclusionsCurrent prediction models for mortality and prognosis of AAAD patients are suboptimal, and the performance of the models varies significantly. It is still essential to establish novel prediction models based on more comprehensive and accurate statistical methods, and to conduct internal and a large number of external validations.

  • Surgical treatment of pregnancy complicated with aortic dissection

    Objective To summarize the characteristics and management of pregnancy complicated with aortic dissection, and to explore the reasonable diagnosis and treatment plan. Methods The clinical data of 10 patients of pregnancy complicated with aortic dissection in Wuhan Tongji Hospital from January 2011 to June 2017 were collected. Their age was 25.2 (21-29) years. Results In the 10 patients, the majority (8 patients) were primipara, and most of them were in the late stages of pregnancy (5 patients) and puerperal (4 patients). Among them, 1 patient had gestational hypertension, and the blood pressure of the left and right upper extremities was significantly abnormal (initial blood pressure: left upper limb blood pressure: 90/60 mm Hg, right upper limb blood pressure: 150/90 mm Hg). The major clinical manifestations were severe chest and back pain which happened suddenly, with D-dimmer and C-creative protein increased which may be associated with inflammatory reaction. All patients were diagnosed by thoracoabdominal aortic CTA, including 5 patients of Stanford type A dissection and 5 patients of Stanford type B dissection. In the 10 patients, 1 patient refused surgery and eventually died of aortic rupture with the death of fetus before birth. And the remaining 9 patients underwent surgical treatment, 3 patients of endovascular graft exclusion for thoracic aortic stent graft, 2 patients underwent Bentall operation, 1 patient with Bentall + total aortic arch replacement + vascular thoracic aortic stent graft, 1 patient with Bentall operation combined with endovascular graft exclusion for thoracic aortic stent graft, 1 patient with Bentall + coronary artery bypass grafting, 1 patient of thoracoabdominal aortic vascular replacement. Among them, 1 patient underwent endovascular graft exclusion for thoracic aortic stent graft died of severe postoperative infection, and the remaining 8 patients were discharged from hospital. Nine patients were single birth, among them 5 newborn patients had severe asphyxia, 4 patients had mild asphyxia. Finally, 3 neonates died of severe complications, and the remaining 6 survived. Conclusion The ratio of pregnancy with Stanford type A aortic dissection is far higher than in the general population, the possibility of fetal intrauterine asphyxia is larger, but through active and effective surgical and perioperative treatment, we can effectively save the life of mother and fetus.

  • Analysis of axillary non-sentinel lymph node metastasis and risk factors in breast cancer patients with 1–2 positive sentinel lymph nodes

    ObjectiveTo investigate the metastatic status and risk factors of axillary non-sentinel lymph node (NSLN) in breast cancer patients with 1–2 positive sentinel lymph nodes (SLN), and to provide theoretical basis for exemption of axillary lymph node dissection (ALND) in these patients. Methods A retrospective analysis was performed on 54 patients diagnosed with breast cancer who underwent sentinel lymph node biopsy (SLNB) and confirmed to have 1–2 positive sentinel lymph nodes (SLNS) and received ALND in the Department of Thyroid and Breast Surgery of Tongling People’s Hospital from January 2018 to April 2023. The patients were divided into NSLN metastatic group (17 cases) and NSLN non-metastatic group (37 cases) according to whether there was metastasis. Chi-square test was used to compare the basic information and clinicpathological features of the two groups. The independent risk factors for axillary NSLN metastasis were screened out by multivariate binary logistic regression model. The receiver operating characteristic (ROC) curve was used to evaluate the predictive value of independent risk factors combined with axillary NSLN metastasis. Results There were 54 cases with 1–2 metastasis of SLN, 17 cases with axillary NSLN metastasis (31.5%). The incidence of axillary NSLN metastasis in patients with tumor at T1 stage (maximum diameter ≤2 cm) was only 14.3% (4/28), however, the metastatic rate of axillary NSLN in patients with tumor in T2–T3 stage (maximum diameter >2 cm) was as high as 50.0% (13/26). The axillary NSLN metastasis rate was only 21.2% (7/33) with 1 SLN metastasis, while the axillary NSLN metastasis rate was 47.6% (10/21) with 2 SLN metastasis. Univariate analysis showed that T stage (tumor diameter >2 cm), 2 SLN metastases, number of SLN >5 and tumor with vascular embolus were more likely to develop axillary NSLN metastases (P<0.05). Multivariate binary logistic regression analysis showed that T stage (tumor diameter >2 cm) and 2 SLN metastases were independent risk factors for axillary NSLN metastasis in breast cancer patients, the area under ROC curve of combined prediction of axillary NSLN metastasis by the two was 0.747, 95%CI was (0.657, 0.917), sensitivity was 0.765 and specificity was 0.649. Conclusions The combination of tumor T stage and the number of SLN metastases can better predict axillary NSLN metastasis in breast cancer patients. ALND is recommended for breast cancer patients with T stage (tumor diameter >2 cm) and 2 SLN metastases to reduce the risk of residual axillary NSLN metastasis.

  • Clinical effect of in situ fenestration and chimney technique in the treatment of aortic dissection involving left common carotid artery

    ObjectiveTo investigate the clinical effect of in situ fenestration combined with chimney technique in the treatment of aortic dissection involving left common carotid artery.MethodsFrom January 2012 to June 2019, 53 patients with aortic dissection involving left common carotid artery were selected. There were 21 patients in the test group, including 14 males and 7 females, with an average age of 57.2±11.2 years; there were 32 patients in the control group, including 20 males and 12 females, with an average age of 56.7±12.1 years. In the test group, the left subclavian branch was reconstructed by in situ fenestration and the left common carotid artery was reconstructed by chimney technique. In the control group, the left common carotid artery was reconstructed by hybrid operation. The clinical data of the patients were compared.ResultsThe operation time of the test group was significantly longer than that of the control group (151.8±35.2 min vs. 101.3±29.6 min, P=0.00). The patients in the two groups were followed up for 6-20 months. There was no significant difference in the incidence of pulmonary infection, stroke, steal blood syndrome, false lumen thrombosis or internal leakage between the two groups (P>0.05). The diameters of the distal and proximal ends of the true cavity in the test group increased significantly compared with those in the control group (P<0.05).ConclusionIn situ fenestration combined with chimney technique is an effective method for the treatment of aortic dissection involving left common carotid artery, which is worthy of further clinical promotion.

  • ADVANCES IN CLINICAL APPLICATION OF LYMPH NODE DISSECTION FOR GASTRIC CANCER

    Objective To provide a current language for clinical and pathological discription of gastric cancer. Methods The literature in recent years on the distribution of lymph nodes and staging of gastric cancer were reviewed. Results The lymph nodes of gastric cancer are distributed near the blood vessel and organs of gastric milieu. To ensure radical gastrectomy rational and scientific, the anatomic structure of gastric milieu should be familiarized. Conclusion The excellent outcome of surgery will be achieved by the effective dissection and removel of lymph nodes in gastric cancer.

  • Strategies of Endovascular Repair for Complicated Stanford Type B Aortic Dissection

    ObjectiveTo investigate the therapeutic strategies of endovascular repair for complicated Stanford type B aortic dissection. MethodThe clinical data of 36 patients with complicated Stanford type B aortic dissection treated by endovascular repair were analyzed retrospectively. ResultsThirty-six patients with complicated Stanford type B aortic dissection were treated successfully by endovascular repair. Twenty-two cases were treated by endovascular repair combined with covering left subclavian artery (LSA). Ten cases were treated by endovascular repair combined with chimney technique. Two cases were treated by endovascular repair combined with vascular prosthesis bypass from left common carotid artery to LSA. Two cases were treated by endovascular repair combined with vascular prosthesis bypass from right common carotid artery to left common carotid artery, whose proximal part were ligated. The viscera artery and lower extremity artery supply were restored gradually. No complication such as endoleak occurred. ConclusionFor endovascular repair of complicated Stanford type B aortic dissection, strategies combined with covering LSA, chimney technique, and hybrid operation of small incision could extend anchor zone, expand the range of endovascular repair of aortic dissection, improve curative effect, reduce complications.

  • Surgical Treatment of Stanford Type A Aortic Dissection

    Objective To summarize treatment experience and evaluate clinical outcomes of surgical therapy for Stanford type A aortic dissection (AD). Methods Clinical data of 48 patients with Stanford type A AD who underwent surgical treatment in General Hospital of Lanzhou Military Region from October 2006 to March 2013 were retrospectively analyzed. There were 41 males and 7 females with their age of 26-72 (47.6±9.2) years. There were 43 patients with acute Stanford type A AD (interval between symptom onset and diagnosis<14 days) and 5 patients with chronic AD. There were 19 patients with moderate to severe aortic insufficiency and 6 patients with Marfan symdrome but good aortic valve function,who all received Bentall procedure,total arch replacement and stented elephant trunk implantation. There were 8 patients with AD involving the aortic root but good aortic valve function who underwent modified David procedure,total arch replacement and stented elephant trunk implantation. There were 10 patients with AD involving the ascending aorta who received ascending aorta replacement,total arch replacement and stented elephant trunk implantation. There were 5 patients with AD involving partial aortic arch who underwent ascending aorta and hemiarch replacement. Patients were followed up in the 3rd,6th and 12th month after discharge then once every year. Follow-up evaluation included general patient conditions,blood pressure control,chest pain recurrence,mobility and computerized tomography arteriography (CTA). ResultsCardiopulmonary bypass time was 121-500 (191.4±50.6) minutes,aortic cross-clamp time was 58-212 (112.3±31.7) minutes,and circulatory arrest and selective cerebral perfusion time was 26-56 (34.8±8.7) minutes. Postoperative mechanicalventilation time was 32-250 (76.2±35.6) hours,and ICU stay was 3-20 (7.1±3.4) days. Thoracic drainage within 24 hours postoperatively was 680-1 600 (1 092.5±236.3) ml. Seven patients (14.5%) died perioperatively including 2 patients with multiple organ dysfunction syndrome,2 patients with low cardiac output syndrome,1 patient with renal failure,1patient with delayed refractory hemorrhage,and 1 patient with coma. Twenty patients had other postoperative complicationsand were cured or improved after treatment. A total of 38 patients [92.7% (38/41)] were followed up for 3-48 (13.0±8.9) months,and 3 patients were lost during follow-up. During follow-up,there were 36 patients alive and 2 patients who died of other chronic diseases. There was no AD-related death during follow-up. None of the patients required reoperation for AD or false-lumen expansion. CTA at 6th month after discharge showed no anastomotic leakage,graft distortion or obstruction.Conclusion According to aortic intimal tear locations,ascending aorta diameter and AD involving scopes,appropriate surgical strategies,timing and organ protection are the key strategies to achieve optimal surgical results for Stanford type A AD. Combined axillary and femoral artery perfusion and increased lowest intraoperative temperature are good methods for satisfactory surgical outcomes of Stanford type A AD.

  • 急性Stanford A型主动脉夹层的外科治疗

    Objective To investigate the surgical experience, best timing of operative intervention, technique and clinical effects of surgical treatment for acute Stanford type A aortic dissection. Methods The clinical material, method of operation and follow-up results were retrospectively reviewed for 29 patients with acute Stanford type A aortic dissection in latest 8 years. The main principle of surgical therapy in Stanford type A aortic dissection was resecting the partial aorta of intimal tear to prevent aortic rupture, repairing the intimal tear and replacing or reconstructing the aorta by composite graft, and simultaneously dealing with the heart valve and myocardial ischemia disease. Results Emergency surgery was performed in 13 cases, and sub-emergency surgery for 16 cases. There were 2 cases (69%) early postoperative death. One patient died of severe arrhythmia after emergency surgery, and another case died of multiorgan failure after operation. After operation 6 cases needed to be re-operated for bleeding, 3 cases developed lung infection and respiratory failure, 4 cases delayed waking, 2 cases occurred acute renal failure and one case occurred gastrorrhagia, they all were treated appropriately and recovered. Twenty-four cases? (889%) were followed up for 23.6±101 months. One case died after 16 months postoperatively due to endocarditis and cerebral hemorrhage. One case was recorded of sudden death in 26 months. Currently other patients were still healthy as the normal person. Conclusion Early-time surgical treatment is the key factor for acute Stanford type A aortic dissection to reduce the mortality. Procedures chosen must depend on the location of intimal tear, involved extension, and the condition of aortic valve and aortic root. It is principle as simplifying operation and good effects of treatment.
