  • Law-Regulated Practice and Evidence-Based Self-Protection

    In recent two decades, the incidence and severity of medical disputes have been dramatically increasing in China which has a negative influence for patients, doctors and hospitals. It must be seriously regarded that 80% of the medical disputes are caused by the bad attitude of health care professionals, ethical problems, and poor communication skill with patients. Chinese health care professionals should be aware of how to establish a good doctor-patient relationship. The development of evidence-based medicine (EBM) will help us bridge a gap between medical science and clinical practice, revise our opinions, update our knowledge and improve our service. Cooperation with the Chinese Evidence-Based Medicine Center (CEBMC), and the Chinese Medical Doctor Association (CMDA) will help doctors practice legally and in an evidence-based manner so as to protect both rights of patients and doctors.

  • Comparison and Analysis on Four Evidence-Based Medicine Databases

    Objective To study and analyze both merits and demerits of 4 famous foreign evidence-based medicine databases, so as to provide references for the development of Chinese evidence-based medicine databases. Methods By means of document analysis and web search, the databases including UpToDate, MD Consult, Clinical Evidence and DynaMed were comprehensively analyzed from the following aspects: management ideas, editing process, personalized services and so on. Results a) Time of foundation: UpToDate founded in 1992 is the earliest-established evidence-based medicine database; b) Management ideas: All 4 databases aim to integrate all the high quality evidences about some clinical topics and help doctors to make the most reasonable decisions at present; c) Editing process: The inclusive criteria of Clinical Evidence is more strict than other databases, for the evidence needs to go through 18 steps before it is included; and d) Update rate: DynaMed updates every day as the fastest than other databases. Conclusion A mature evidence-based medicine database needs a powerful methodology team, b financial support and a large number of literature services. Besides learning good foreign experiences, it is also very important to assemble a methodology team, and particularly to integrate domestic characteristics for the establishment of domestic evidence-based medicine database.

  • Investigation into the Practice of Ensuring a Steady Blood Supply for Medical Rescue during the Wenchuan Earthquake△

    In the rescue that follows a major natural disaster, blood donation is a unique, necessary method to assist the injured. To achieve effective assurance of the blood supply for medical rescue in an orderly fashion, the current procedure must be adjusted by using scientific prediction, analysis, and adaptation. After the process of ensuring the blood supply for medical rescue during the Wenchuan Earthquake, the Ministry of Health of China, with great efforts, continues to investigate and reflect upon the application of the above principals during actual practice. Objectively, these efforts will lead to better results and establish a standard supplying blood during a disaster rescue.

  • The methodological assessment of cross-sectional surveys about Chinese medicine syndrome in a population at potential risk of cerebrovascular diseases

    ObjectiveTo evaluate the methodological quality of cross-sectional surveys about Chinese medicine syndrome in a population at potential risk of cerebrovascular diseases. Methods The CNKI, WanFang Data, CBM and PubMed databases were electronically searched to collect cross-sectional surveys about Chinese medicine syndromes in a population at potential risk of cerebrovascular diseases from inception to December, 2022. The methodological quality was assessed using the JBI scale. Results A total of 105 studies were included. The average reporting rate of JBI was 52.06%, and the items with the highest scores included "sufficient coverage of the identified sample in data analysis" (100%), "description of study subjects and setting" (92.38%), and "using valid methods for the identification of the condition" (86.67%). Items with the lowest scores included "adequate sample size" (13.33%), "adequate response rate or low response rate managed appropriately" (14.29%), and "study participants recruited in an appropriate way" (20.95%). Subgroup analysis suggested that type of publication and number of implementation centers were potential factors influencing methodology quality (P<0.05). Conclusion The methods essential to a cross-sectional survey such as sampling, sample size calculation and handling with the response rate, and the syndrome diagnosis scales specific to Chinese medicine require further improvement.

  • Value Orientation of Medical Information Activity Reflected in Systematic Review

    As one of the most sophisticated research methods of evidence-based medicine, systematic review is an activity of collecting, arranging and analyzing medical information. The methodological characteristics of systematic review reflect the value orientation of this activity. By analyzing and summarizing these characteristics, this paper points that the process of systematic review reflects the value orientation of attaching importance to obtaining universal information sources, treating individual information in an add-weight way, discretion in interpreting results and updating of time-limited information in medical information activities.

  • Evaluation on the Effect of Improved Oven for Defluorination in China

    Objectives To analyze the effect of improved oven for defluorination in coal-burning endemic fluorosis areas in China, and to provide evidence for the prevention and control of fluorosis. Methods Electronic databases including CNKI, CBM, VIP and CDMD-D (1989 to 2005), were searched. We also checked the reference lists of relevant articles. We selected relevant articles according to the predefined inclusion and exclusion criteria. The methodological quality was assessed . Data on room heat preservation and the effect of improved oven for defluorination were collected in the surveillance spots of Three Gorges Reservoir. Correlation analyses were conducted between the improved oven and its effect parameters. Results Twelve articles of low quality met the selection criteria, of which 9 were graded C and 3 were graded D in terms of the methodological quality. A negative correlation was found between the decreasing rate of normal oven use and the decreasing rate of dental fluorosis as well as of urine fluorine (Pearson correlation coefficient r = – 0.87, – 0.63, Plt;0.01, lt;0.05, respectively). Analysis also revealed a positive correlation between room heat preservation and the decreasing rate of dental fluorosis as well as of normal oven use (the two Spearman correlation coefficients and P values were the same: r = –1.00, Plt;0.01). Conclusion High-quality studies on the effect of improved oven for defluorination in China are not available. Based on the current evidence, the improved oven for defluorination and the correct use, maintenance and house rebuilding for heat preservation may help to prevent fluorosis.

  • Get to know about pain again: new interpretations, new classifications and new treatments

    The scientific understanding of pain has stayed on a traditional classic “pain pathway” for many years. However, with the continuous progress of clinical diagnosis, this pathway obviously cannot explain all pain phenomena, and the understanding and interpretation of pain needs to be updated. Based on the influence of comprehensive medicine based on philosophy and the continuous summary and research of clinical cases of somatic symptoms, this paper takes the biological defense mechanism as the basic starting point, analyzes the position of pain in the biological defense mechanism, and reinterprets and defines pain from the new point of view of comprehensive medicine with the new perspective of biological defense mechanism. In addition, this paper also presents the new treatment principles of pain according to the occurrence mechanism through the perspective of biological defense and psychosomatic mechanism, and puts forward new principles of treatment and the view that “defense” is the fundamental purpose of all biology, aiming to provide a reference for the reinterpretation of more diseases and the research on the nature of biological behavior.

  • RESEARCH AND APPLICATION PROGRESS OF ADIPOSE-DERIVED STEM CELLS

    Objective To review the biochemical characteristics, appl ication progress, and prospects of the adiposederived stem cells (ADSCs). Methods The recent original experimental and cl inical l iterature about ADSCs was extensively reviewed and analyzed. Results ADSCs can be readily harvested in large numbers from adipose tissue with properties of stable prol iferation and potential differentiation in vitro. Significant progress of ADSCs is made in the animal experimentand the cl inical appl ication. It has been widely used in the cl inical treatment of cardiovascular disease, metabol ic disease, encephalopathy, and tissue engineering repair. Conclusion ADSCs have gradually replaced bone marrow mesenchymal stem cells and become the focused hot spot of regenerative medicine and stem cells.

  • Application of evidence-based medicine combined with problem-based learning method in clinical teaching: a meta-analysis

    ObjectivesTo systematically review the application of evidence-based medicine (EBM) combined with problem-based learning (PBL) method in clinical teaching.MethodsDatabases including WanFang Data, CNKI, VIP, CBM, PubMed and EMbase were searched to identify eligible randomized controlled trials that compared EBM plus PBL with traditional lecture based learning (LBL) method in clinical teaching from inception to March 28th, 2020. Two reviewers independently screened literature, extracted data and assessed risk bias of included studies. Statistical analysis was performed using Stata 14.0 software.ResultsA total of 33 randomized controlled trials with 2 855 students were included. The results showed that students in EBM combined with PBL group obtained better scores of specialized theory (WMD=6.87, 95%CI 5.08 to 8.66, P<0.001), skills examination (WMD=10.57, 95%CI 8.98 to 12.16, P<0.001) and case analysis (WMD=9.79, 95%CI 4.71 to 14.88, P<0.001), comparing with those in LBL group. Besides, EBM combined with PBL method improved students’ overall ability including independent learning capability, clinical thinking ability, problem-solving capability, literature exploring capacity, communication and presentation skills and team-work ability.ConclusionsApplication of EBM combined with PBL method can improve clinical teaching quality.
