  • Case analysis: CT manifestations of hepatoid adenocarcinoma of gallbladder with liver involvement

    The authors presented CT manifestations of a patient with hepatoid adenocarcinoma of gallbladder with liver involvement and briefly described the clinical features, imaging manifestations, and differential diagnosis of the disease in order to enhance the readers’ better awareness of the imaging manifestations, thus to reduce the misdiagnosis of the disease.

  • 10 例老年性肺炎型肺癌诊治体会

    目的 探讨老年性肺炎型肺癌( PTLC) 的临床特征及影像学特点, 提高诊断率, 减少误诊。方法 回顾性分析丽水市第二人民医院2009 年6 月至2011 年6 月经病理确诊的10 例老年性 PTLC, 分析其临床表现、实验室检查、胸部CT、支气管镜及病理资料, 总结其临床特征。结果 老年性PTLC 影像学表现多见气管充气征、蜂窝状透明影、小结节等, 缺乏特异性临床表现, 抗感染及抗结核治疗无效。结论 老年性PTLC 影像学表现与肺炎类似, 但临床特点、实验室检查及对抗感染治疗的反应性不符合感染性疾病的特征, 确诊依赖于尽早获得病理学诊断。

  • 隐源性机化性肺炎的临床特点分析

    目的 分析62例经病理确诊的隐源性机化性肺炎患者,总结隐源性机化性肺炎的临床表现、实验室检查、影像学、病理学等特点,加深临床医师对隐源性机化性肺炎的认识。方法 收集2019年3月1日—2023年5月31日于首都医科大学附属北京胸科医院住院,经病理诊断为隐源性机化性肺炎的患者的临床资料,回顾性分析其临床表现、实验室检查、影像学资料等。结果 62例患者中男53例,女9例,平均年龄(60.26±9.91)岁。症状以咳嗽、咳痰多见,部分患者伴有发热、咯血、呼吸困难。胸部CT表现为肺部实变、斑片、结节影等,病变多位于胸膜下,可伴有支气管充气征、胸腔积液、胸膜增厚,以及纵隔、肺门淋巴结肿大等。肺功能可能表现为阻塞性通气功能障碍、限制性通气功能障碍及弥散异常等。结论 隐源性机化性肺炎的临床特征、实验室检查及影像学表现缺乏特异性,常需与肺部感染、恶性肿瘤进行鉴别,确诊需病理学支持。

  • Case study—intrahepatic splenosis with CT and MR findings mimicking hepatocellular carcinoma

    This study reported a case of intrahepatic splenosis with CT and MR findings mimicking hepatocellular carcinoma. The patient had two risk factors for hepatocellular carcinoma, including elevated alpha-fetoprotein and a history of hepatitis B virus infection, and had previously splenectomy due to spleen trauma. This paper briefly described the etiology and pathogenesis of intrahepatic splenosis and reviewed the radiological findings of this disease reported in previous literature, in order to strengthen readers’ understanding of intrahepatic splenosis and reduce the misdiagnosis rate.

  • Imaging manifestations of intrahepatic ectopic splenosis: a case analysis

    Intrahepatic ectopic splenosis is rare and lacks typical clinical and imaging features. We analyzed the clinical, pathological, and imaging characteristics of a patient with intrahepatic ectopic splenosis admitted to the Guangzhou First People’s Hospital in combining with literature review. The aim of case analysis is to enhance comprehension of the disease and provide a reference for clinical doctors in diagnosis.

  • CT and MR manifestations of hepatoid adenocarcinoma of the pancreas: a case report and literature review

    Hepatoid adenocarcinoma (HAC) of the pancreas is a rare, highly malignant tumor with poor prognosis. This article presents the CT and MR features of HAC of the pancreas, while also reviewing the relevant literatures to succinctly summarize the underlying pathophysiological mechanism, imaging diagnosis, and differential diagnosis. The objective is to enhance the understanding of HAC of the pancreas among clinicians and radiologists.

    Schwannoma originating from the common bile duct is rare. We presented a patient who was diagnosed with biliary cystadenoma preoperatively and pathologically confirmed as a choledochal Schwannoma, analyzed the CT and MRI imaging manifestations, and illustrated its anatomical and pathological basis, and to improve the understanding of clinicians and radiologists for choledochal Schwannoma.

  • The Spectrum of Imaging Manifestations in Posttransplantation Lymphoproliferative Disease

    ObjectiveTo explore the imaging manifestations of posttransplantation lymphoproliferative disease (PTLD) for making a further understanding of this disease and its imaging diagnosis. MethodsBy the method of litera-ture review, the imaging manifestations of PTLD in the abdomen, thorax, head and neck, and central nervous system were summarized, respectively, and also the epidemiologic features, pathogenesis and pathologic classification were reviewed. ResultsPTLD is a spectrum of lymphoproliferative diseases following transplantation mainly caused by Epstein-Barr virus and immunosuppressive therapy with different pathologic types.Lesions at imaging examinations may be the first clue to diagnose, and the appearance of PTLD at imaging can vary depending on the region. ConclusionImproving the cognition of PTLD and the imaging features plays a pivotal role in identifying the presence of disease, guiding tissue sampling, and evaluating response to treatment.

  • Clinical analysis of pulmonary cryptococcosis with varied immune status: 32 cases review

    Objective To discuss the correlation between immune status and clinical characteristics in pulmonary cryptococcosis. Methods The clinical data of 32 non-AIDS patients with pulmonary cryptococcosis, diagnosed from August 2001 to October 2017 in Tianjin Medical University General Hospital, were retrospectively analyzed. The enrolled patients were divided into an immune-competent group with 13 cases and an immune-suppressed group with 19 cases. The clinical characteristics were compared between the two groups with different immune status. Results All 32 patients were treated for clinical symptoms. The main symptoms were cough, expectoration, fever, chest tightness, chest pain, and hemoptysis. There were no statistical differences between the two groups. The computed tomography of chest showed that there were 2 patients (6.3%) involving upper lung in the immune-competent group, and 5 patients (15.6%) in the immune-suppressed group. There were 9 patients (28.1%) involving lower lung in the immune-competent group, and 12 patients (37.5%) in the immune-suppressed group. There were 10 patients (31.3%) with nodular masses of lesions in the immune-competent group and none in the immune-suppressed group. There was 1 patient with infiltrating in the immune-competent group and 8 patients in the immune-suppressed group. There were 2 patients with mixed types of lesions in the immune-competent group and 11 patients in the immune-suppressed group. Five patients were complicated with cryptococcal meningitis, and 2 patients with eosinophilia. Conclusions The clinical characteristics of the patients with pulmonary cryptococcosis are not specific in difference immune status. The chest CT shows that the lesions of immune-competent patients are mainly nodular masses type, while lesions of immune-suppressed patients are mainly infiltrating shadow and mixed shadow. The treatment should be chose according to immune status.
