  • REPLANTATION OF COMPLEX THUMB MUTILATION WITH LOCAL ISLAND FLAP

    Objective To investigate the cl inical effect and operative method of local island flap for complex thumb mutilation with soft tissue and blood vessel defect. Methods From May 2003 to March 2006, 6 cases of complex thumb mutilation with soft tissue and blood vessel defect were treated with local island flap. There were 4 males and 2 females aged 14-48 years, with an average of 23.5 years, among whom 2 cases were caused by triangular bandage twist, 3 cases by machinesavulsion and 1 case by explosion. Five cases suffered thumb mutilation of soft and blood vessel defect only, and 1 case was combined with middle and ring finger injures. The defect was located in pulp soft tissue in 4 cases and in dorsal soft tissue in 2 cases, ranging 2.0 cm × 1.2 cm-2.5 cm × 1.8 cm in size. The time from injury to operation varied from 30 minutes to 6 hours. Two cases were replanted with bridging index finger radial is digital artery island, 2 cases were repaired by ring finger radial is digital artery island and 2 cases by index finger near dorsi-flap. The flap was 2.0 cm × 1.4 cm-2.5 cm × 1.8 cm in size. Free-skin graft from forearm was conducted. Results All flaps free skin and replanted thumbs in 6 cases survived completely, following up for 6-24 months after operation. The flaps and thumb had good texture and color match, two-point discrimination was 10-12 mm on thumb pulp and 8-10 mm on flap. All replanted thumb recovered satisfied function, there were no donor site dysfunction. According to the criteria for function assessment of amputated finger issued by the Branch of Hand Surgery of Chinese Medicine Association:4 cases were regarded as excellent and 2 as good. Conclusion Local island flap is capable of repairing complex thumb mutilation with soft tissue and blood vessel defect, maximizing the recovery of thumb appearance and function.

  • 拇指软组织缺损的修复

    目的 探讨不同类型拇指软组织缺损的修复方法。方法 2003年1月~2005年1月,对23例外伤性拇指软组织缺损患者采用单纯或联合食指背侧岛状皮瓣、拇指桡侧指动脉逆行岛状皮瓣、指动脉侧方岛状皮瓣、趾腹皮瓣及足母甲皮瓣移植术治疗。 结果 术后皮瓣全部成活,均获随访6~24个月。皮瓣血运、外观、质地均良好,拇指活动、对掌功能及皮肤感觉均恢复良好。 结论 不同皮瓣对于拇指软组织缺损修复有其适应证。手术时皮瓣选取适宜、设计合理,可以最小的创伤获得最佳的拇指修复效果。

  • 指背动脉筋膜瓣结合皮肤原位回植治疗拇指末节指腹撕脱伤

    目的探讨以指背动脉筋膜瓣结合皮肤原位回植治疗拇指末节指腹撕脱伤的疗效。 方法2014年3月-2015年1月,收治9例(9指)因机器挤压导致的拇指末节指腹撕脱伤患者。男6例,女3例;年龄13~58岁,平均33岁。均为拇指指间关节平面以远指掌侧皮肤软组织撕脱缺损,伴骨、肌腱外露,无再植条件。创面范围为1.4 cm×1.2 cm~1.6 cm×1.4 cm。受伤至手术时间3~10 h,平均6 h。以拇指指背动脉筋膜瓣覆盖外露肌腱、指骨,将撕脱皮肤修薄成全厚皮片回植覆盖筋膜瓣。 结果术后回植皮片顺利成活,创面Ⅰ期愈合。患者均获随访,随访时间6~12个月,平均8个月。筋膜蒂部无臃肿,回植皮片质地柔软、外观满意、颜色与周围皮肤接近、皮纹恢复。术后6个月按照总主动活动度法评定手功能,获优7指,良2指。 结论采用指背动脉筋膜瓣结合皮肤原位回植治疗拇指末节指腹皮肤撕脱伤不损伤指动脉和指神经,可获得较好疗效。

  • 第二掌骨背侧岛状皮瓣修复拇指掌侧及虎口区皮肤软组织缺损

    目的 总结采用第2 掌骨背侧岛状皮瓣修复拇指掌侧及虎口区皮肤软组织缺损的方法及疗效。 方法 2006 年5 月- 2008 年5 月,采用第2 掌骨背侧岛状皮瓣修复拇指掌侧及虎口区皮肤软组织缺损19 例。男12 例,女7 例;年龄15 ~ 42 岁,平均27 岁。机器压砸伤18 例,虎口区组织挛缩1 例。压砸伤患者中,拇指末节缺损11 例,拇指近节缺损5 例;虎口区皮肤缺损2 例;皮肤软组织缺损范围为2.2 cm 1.0 cm ~ 3.8 cm 2.4 cm。压砸伤患者均为受伤后3 h 内手术,虎口区组织挛缩患者为伤后6 个月手术。术中皮瓣切取范围为2.8 cm 1.2 cm ~ 4.0 cm 2.6 cm,供区采用中厚皮片游离植皮修复。 结果 术后皮瓣及供区皮片均顺利成活,创面Ⅰ期愈合。术后患者均获随访,随访时间6 ~ 24 个月。拇指外形饱满,感觉恢复至S2+;屈伸、外展、对掌、对指功能良好。虎口张开度为80 ~ 85°。 结论 第2 掌骨背侧岛状皮瓣修复拇指掌侧及虎口区皮肤软组织缺损具有手术操作简便、皮瓣质量好、术后皮瓣成活率高等优点,可获得较好疗效。

  • REPAIR OF IRREGULAR WOUNDS ON THUMBS AND INDEX FINGERS WITH FIRST DORSAL METACARPAL ARTERY-BASED PEDICLE DICTYO-PATTERN SUBLOBE FLAPS

    Objective To explore the effect of the first dorsal metacarpal artery-based pedicle dictyo-pattern sublobe flaps in repairing irregular wounds on thumbs and index fingers. Methods From December 2006 to March 2009, 15 patients with irregular wounds on thumbs and index fingers were treated, including 11 males and 4 femals, with an average age of 31 years (range, 18-48 years). Of them, 6 cases of hyperplastic scar of postburn had a 2-25 years course of disease (7.5 years onaverage) and 9 cases of machine injury had a 14-30 days course of disease (20 days on average). In 8 thumb wounds on palmaris, there were 3 cases of rhomboid wounds, 1 case of C-shape wound, and 4 cases of irregular wounds; the area of wounds ranged from 4.5 cm × 3.0 cm to 5.5 cm × 4.5 cm and wounds were treated by the first dorsal metacarpal artery-based proximate pedicle dictyo-pattern sublobe flaps (5 cm × 3 cm to 6 cm × 5 cm). In 7 index fingers wounds on dorsi-fingers, there were 3 cases of 2-wounds, 4 cases of irregular wounds; the area of wounds ranged from 1.0 cm × 0.5 cm to 2.2 cm × 2.0 cm and wounds were treated by the first dorsal metacarpal artery-based distal pedicle dictyo-pattern sublobe flaps (1.2 cm × 0.5 cm to 3.0 cm × 2.2 cm). The donor sites were covered with skin grafts or sutured directly. Results All of the flaps survived completely, the wound of recipient site healed at stage I. The free skin graft on donor site survived completely, the wound of donor site healed at stage I. All cases were followed up for 6-12 months (9 months on average). There was good appearance of flaps. The two point discrimination was 5-7 mm for the proximate pedicle flaps and 9-10 mm for the distal pedicle flaps. The thumbs had digital opposition, opposition function, the index fingers had no dysfunction. According to Hand Surgery Association Society of Chinese Medical Association Society standard for the part function evaluation trial-use of upper l imb, the results of the total active movement were excellent in 14 cases, and good in 1 case. The l ine-scar was existed at donor site of the case of direct suture, the function of thumb web had no dysfunction. Conclusion The first dorsal metacarpal artery-based pedicle dictyo-pattern sublobe flaps can repair the irregular wounds on thumbs and index fingers. It has rel iable blood supply and simple operation.

  • FREE MULTIPLE FLAPS OF LOWER EXTREMITY FOR SEVERLY BURNED HAND RECONSTRUCTION

    Objective To introduce the free multiple flaps of lowerextremity based on the anterior tibital vascular pedicle for primary repair of the complex burned hand deformities.Methods From September 2000 to February 2003, the lateral leg flap, dosalis pedis flap and trimmed first toe based on the anterior tibial vascular pedicle were utilized to reconstruct the thumb and repair the first web, thenar, wrist or palmar scar contracture simultaneously in 6 patients. The flap size of lateral leg and dosalis pedis ranged from 4 cm×10 cm to 7 cm×10 cm and from 5 cm×10 cm to 9 cm×12 cm, respectively.Resutls Six cases were treated and followed up for 6 weeks to 1 year. The transplanted flaps survived with satisfactory recovery in function and appearance of theburned hand. The function of donor lower extremity was not damaged. Conclusion The procedure of the free multiple flaps of lower extremity based on the anterior tibial vascular pedicle is reliable and effective for primaryrepair of burned hand.

  • CLINICAL APPLICATION OF ISLAND FLAP PEDICLED WITH DORSAL CUTANEOUS BRANCHES OF THUMB RADIAL DIGITAL ARTERY

    Objective To investigate the effectiveness of the island flap pedicled with the dorsal cutaneous branches of thumb radial digital artery from the same finger for repairing pulp defect. Methods Between June 2009 and March 2010, 10 patients (10 fingers) with pulp defect of thumb were treated. There were 6 males and 4 females, aged 13-68 years with an average of 38 years. Defect was caused by machine crush in 4 cases, by saw machine in 3 cases, by chronic infection in 2 cases, and by burn in 1 case. The disease duration was 3 hours to 4 months. In 4 cases of distal pulp defect (1.0 cm × 0.8 cm to 2.0 cm × 1.4 cm) with exposure of bone or tendon, defect was repaired with island flap pedicled with the interphalangeal joint cutaneous branches of thumb radial digital artery (1.0 cm × 0.8 cm to 2.2 cm× 1.5 cm). In 6 cases of proximal pulp defect (1.0 cm × 0.8 cm to 2.5 cm × 2.0 cm) with exposure of bone or tendon, defect was repaired with island flap pedicled with the metacarpophalangeal joint cutaneous branches of thumb radial digital artery (1.0 cm × 0.8 cm to 2.6 cm × 2.2cm). The donor sites were repaired with skin grafts. Results All flaps and skin grafts survived, and wounds healed by first intention. Ten cases were followed up 6-12months (mean, 8 months). The colour, texture, and contour of the flaps were good. The two-point discrimination was 7-10mm on the island flap at last follow-up. According to total active motion (TAM) standard, the thumb function was assessed as excellent in 8 cases, good in 1 case, and fair in 1 case, and the excellent and good rate was 90%. Conclusion The main digital artery and nerve of thumb will not be sacrified when the island flap pedicled with the dorsal cutaneous branches of thumb radial digital artery is used. The operative procedure is simple, so it is a good method for repairing pulp defect of thumb.

  • 皮神经营养血管皮瓣的临床运用

    目的 探讨吻合神经的手背皮神经营养血管皮瓣的临床应用疗效。 方法 2003年1月~2006年3月,采用拇指桡侧、尺侧、虎口背侧支皮神经营养逆行岛状皮瓣修复拇指末节创面39例。男28例,女11例;年龄16~53岁。冲压伤11例,电锯伤23例,切割伤5例。皮瓣切取范围3.3 cm×2.6 cm~5.6 cm×3.5 cm。术后予石膏制动、抗凝、解痉、预防感染等处理,2周后拆石膏行功能锻炼。 结果 术后患者获随访6~12个月,平均9个月。根据赵书强手功能评定标准改进标准进行临床疗效评定,优31例(79.5%),皮瓣成活,两点辨别觉4~7 mm,对掌、对指功能恢复正常;良7例(17.9%),皮瓣远端皮肤边缘性坏死,两点辨别觉5~9 mm,对掌、对指功能恢复接近正常;差1例(2.6%),皮瓣坏死,改行腹股沟皮瓣修复。 结论 该皮瓣手术切取成活率较高,术后外观及功能恢复优良,是修复拇指末节创面的一种有效方法。

  • REPAIR OF PULP DEFECT OF THUMB BY FREE PALMARIS BREVIS MUSCULOCUTANEOUS FLAP

    It is difficult to repair the pulp defect of finger with good function. Here reported two cases of pulp defect of the thumb which were repaired with free palmris brevis musculo-cutaneous flap. The flap was designed as the following: taken the line crossing the pisiform and metacarpo-phalangeal joint of the little finger as the longitudinal axis of the flap. The proximal end of the flap was at the level of pisiform and the distal end was the distal transverse palmar crease. The radial border was the radial side of the palmris brevis and ulnar border was the ulnar edge of the palm. The flap should not be larger than 6.5 cm x 2.5 cm. The flap was dissected with proper ulnar vessels and the accompanying arterial branch and superficial branch of the ulnar nerve. The flap was transferred to repair the pulp defect of the thumb. The vessels were anastomosed with the radial artery and cephalic vein at the snuffbox. The nerves were sutured to the digital nerve. Three months after operation, the defect was healed and the sensation of pulp was recovered. The donor area was painless and without sensation disturbance. The motion of the little finger was normal. The function of the thumb was restored. It was concluded that the palmaris brevis musculocutaneous flap is one of the best donor region to repair pulp defect of thumb.
