  • 先天性心脏病合并气道异常的围术期处理

    摘要: 目的 提高对先天性心脏病合并气道异常的认识,总结围术期处理经验。 方法 对中南大学湘雅二医院2006年1月至2008年1月收治的5例先天性心脏病合并气道异常患者的临床资料进行回顾性分析,其中男3例,女2例;年龄5个月~9岁,平均年龄27个月。气道异常包括:左主支气管狭窄2例,右侧气管化支气管、气管轻度软化和左主支气管狭窄、长段先天性气管狭窄各1例。行先天性心脏病矫治术加左主支气管成形术1例,仅行先天性心脏病矫治术3例,放弃手术治疗1例。 结果 围术期死亡1例,为先天性心脏病合并右侧气管化支气管,术后发生右肺上叶不张、肺部感染和呼吸衰竭死亡。1例先天性心脏病合并左主支气管狭窄,同期行左主支气管成形术后顺利出院;随访2年健康。1例行先天性心脏病矫治术后撤离呼吸机困难,发现合并先天性左主支气管狭窄伴轻度气管软化,经内科处理撤离呼吸机后仍有喘憋;随访6个月患者发育好,喘憋未再发作。1例混合型室间隔缺损致心脏扩大、肺动脉扩张压迫左主支气管导致狭窄,行先天性心脏病矫治术后左主支气管压迫症状缓解;随访1年健康。1例合并长段气管狭窄,家属放弃手术。 结论 先天性心脏病合并气道异常临床上很少见。应提高对该病的认识,做到术前确诊;采用最佳的手术方法,尽量与先天性心脏病同期手术纠治;术后加强呼吸道管理,以获得最佳的疗效。

  • Efficacy of Noninvasive Positive Pressure Ventilation Plus Plateau Exhalation Valve Combined with Respiratory Stimulant on Pulmonary Encephalopathy in Patients with Acute Exacerbation of COPD

    Objective To explore the efficacy of bi-level positive airway pressure ( BiPAP)ventilation plus plateau exhalation valve ( PEV) combined with respiratory stimulant for the treatment of pulmonary encephalopathy in patients with acute exacerbation of chronic obstructive pulmonary disease( AECOPD) . Methods 70 AECOPD patients with pulmonary encephalopathy were randomly divided into a control group and a treatment group. All patients received BiPAP ventilation and conventional therapyincluding antimicrobial, bronchodilation, and expectorant treatment. In the treatment group, the BiPAP ventilator was connected to PEV additionally, and naloxone and nikethamide were administered for 3 days.Clinical symptoms, blood gas analysis, vital signs, gas leakage conditions, and adverse reactions were recorded. Results Heart rate, respiratory rate, PaCO2 , and APACHEⅡ score were more significantly lower,the time to recover consciousness was shoter, meanwhile PaO2 , SaO2 , pH, and glasgow coma scale were significantly higher in the treatment group compared with the control group( all P lt; 0. 01) . Two cases in the treatment group and 5 cases in the control group received tracheal intubation and invasive ventilation due to treatment failure. Two elderly patients in the control group died in hospital. Conclusion Noninvasive positive pressure ventilation plus PEV combined with respiratory stimulant can significantly improve symptoms, shorten the time to recover consciousness, reduce the rate of endotracheal intubation, and improve hypoxemia and hypercapnia rapidly in AECOPD patients with pulmonary encephalopathy.

  • Clinic Analysis for Nickel-titanium Alloy Stent Treated Airway Obstruction of21 Cases under Direct Observation by Electronic Bronchoscope

    目的:评价电子气管镜直视下置入镍钛合金支架治疗气道狭窄的疗效及安全性。方法:对我院3年来由各种原因引起的气管或支气管狭窄的21例患者行电子气管镜直视下经鼻置入国产镍钛合金支架术,观察置入支架前后症状、狭窄段气道直径变化、动脉血气变化情况及其并发症。结果:21例患者术后呼吸困难均明显改善,气道内径扩张及动脉血氧分压改善较术前均有统计学意义,未发生严重并发症。结论:电子气管镜直视下置入气道支架准确、迅速、安全,操作较方便,有助于延长患者的生存时间和提高生活质量,为进一步治疗创造条件。

  • 硬质气管镜下放置支架治疗大气道阻塞性疾病

    摘要: 目的 为治疗大气道内阻塞性疾病,探讨现代硬质气管镜下气管支架技术的价值。 方法 2002年9月至2008年5月,对7例大气道阻塞性疾病患者在硬质气管镜下放置气管支架,其中良性病变1例,为右主支气管外伤后瘢痕狭窄;恶性病变6例,包括食管癌术后气道狭窄4例,原发性气管肿瘤1例,气管肿瘤术后气道狭窄1例。静脉全身麻醉,不插管,仰卧位,经口置入硬质气管镜,喷射通气。首先对气道进行全面的检查和评估,将气道内病变清除或直接扩张气道,直视下利用施放器放置支架;术后不需要辅以机械通气。 结果 共放置支架7枚,每例患者1枚,包括气管支架3枚,左主支气管3枚,右主支气管1枚。手术顺利,无严重并发症及围手术期死亡。失访1例,随访6例,随访41.4±20.5个月;5例恶性疾病患者中有1例于术后1个月因心脏病猝死,4例术后平均生存14.7个月;1例良性疾病患者术后1个月因刺激性咳嗽将支架取出,随访27个月未再出现气道狭窄。 结论 硬质气管镜下放置支架治疗大气道内阻塞性疾病安全、可靠,操作简单,值得临床推广。

  • 气道廓清术在脑卒中相关性肺炎气管切开患者康复中的应用

    目的观察气道廓清技术对脑卒中相关性肺炎气管切开患者的临床疗效。方法将 60 例脑卒中相关性肺炎气管切开患者按随机数字表法分为观察组和对照组。观察组在常规治疗的基础上给予气道廓清技术排痰,对照组给予常规治疗和传统体位引流排痰。记录患者体温、外周血白细胞计数、排痰量、气体交换指数及胸部 X 线片浸润情况,比较组间临床肺部感染评分(CPIS)、气管套管拔管时间、拔管成功率及临床疗效的差异。结果干预第 2、3 周后,观察组的 CPIS 评分较对照组均明显降低(P<0.05);干预第 4 周后,观察组的 CPIS 评分较对照组显著降低(P<0.01);与干预前相比,从第 2 周开始,随着干预周数的增加,观察组 CPIS 评分降低越显著(P<0.01),而对照组至第 4 周 CPIS 评分才显著降低(P<0.01)。干预 4 周结束后,观察组显效 23 例,有效 6 例,无效 1 例;对照组显效 13 例,有效 16 例,无效 1 例,组间差异有统计学意义(P<0.05)。干预后观察组患者拔管成功率明显高于对照组(P<0.05),观察组拔管时间比对照组显著缩短(P<0.01)。结论气道廓清技术能有效减少脑卒中相关性肺炎气管切开患者痰液,改善患者的缺氧症状,有利于缩短拔管时间,减少并发症的发生,提高拔管成功率,促进患者的康复。

  • Injuries of Tracheal Mucosal Surface Structure Caused by Amikacin and Interference Effects of Ambroxol

    Objective To observe the effects of ambroxol injection on mucosal surface structure of trachea injured by intratracheal instillation of amikacin. Methods 280 Wistar rats were randomly divided into four groups( n = 70 in each group) , ie. a normal control group, a normal saline group( intratracheally instilled normal saline) , an amikacin group ( intratracheally instilled amikacin) , and an ambroxol group ( intratracheally instilled amikacin and ambroxol simultaneously) . At the time points of 2, 12, 24, 48, 72 hours six animals in each group were killed and the samples of 1/3 lower segment of trachea were collected and observed by scanning electron microscope. Endotracheal intubation were made on other 6 animals to collecte broncho-alveolar lavage fluid ( BALF) for leucocyte count. Results Compared with the normal control group, elevated leucocyte count was observed in all other groups, various grades of swelling of the cilia were revealed, followed by more or less cilia laid flat with adjacent cilia conglutinated. Then partial cell membrane on top of some cilia bulged out. In terms of injury, the normal saline group was the most mild, and the amikacin group was most serious with the highest leucocyte count. All the parameters were relieved in ambroxol group. Conclusions Intratracheal instillation of amikacin causes acute injury of the ultrastructure of mucosal surface cilia. Ambroxol can promote the recovery process and alleviate inflammation of airway.

  • 鼻塞式持续气道正压通气在婴幼儿心脏手术后的应用

    无创正压通气作为一种非侵入性辅助通气方式,越来越多地应用于婴幼儿呼吸衰竭、急性呼吸窘迫综合症(ARDS)等疾病的治疗中[1]。其中,鼻塞式持续气道正压通气(NCPAP)在新生儿、婴幼儿呼吸衰竭中的应用较多[2],但在先天性心脏病体外循环术后患儿的呼吸支持中报道较少[3]。我院重症监护病房(ICU)将NCPAP应用于40例先天性心脏病手术后患儿,在机械通气拨管后立即使用NCPAP,取得了较满意的效果,报告如下。

  • Changes of Airway Resistances before and after Bronchial Dilation Test in COPD and Asthma

    Objective To explore the profile and diagnosis value of airway resistances before and after bronchial dilation test ( BDT) in patients with COPD and asthma. Methods Airway resistances before and after BDT were measured in COPD patients and asthma patients with different severity by impulse oscillometry ( IOS) , and the characteristic changes of the two different diseases were analyzed compared with healthy subjects. Results Airway resistance indexes except X5 were higher in the COPD and the asthma patients than those in the healthy subjects before BDT ( P lt; 0. 05) . There were significant differences in airway resistance indexes except X5 and Rc between the mild asthma patients and the moderate to severe asthma patients. Significant difference in Z5, Fres, and Rp were observed in the mild COPD patientscompared with the moderate to severe COPD patients. There were statistical differences in airway resistance indexes except X5 between the two groups before and after BDT both in the COPD and the asthma patients ( P lt;0. 05) . The rates of change in Z5, Fres, R5, and Rp were higher than those of FEV1% pred, especially higher in the asthma patients than in the COPD patients ( P lt; 0. 05) . Significant negative correlations between FEV1% pred and Z5, Fres, R5, Rp were revealed in the COPD and the asthma patients ( P lt;0. 01) .The correlation between Fres and FEV1% pred was most significant in the COPD and the asthma patients ( r = - 0. 561, - 0. 761) . Conclusion Airway resistances measured by IOS is sensitive indicators in detecting the airflow obstruction in COPD and asthma, and is useful in early and differential diagnosis of COPD and asthma.

  • Efficacy of Non-invasive Bi-level Positive Airway Pressure Ventilation in the Patients with Type Ⅱ Respiratory Failure due to Acute Exacerbation of Chronic Obstructive Pulmonary Disease

    目的:评价经口鼻面罩双水平气道正压通气(BiPAP)治疗慢性阻塞性肺疾病急性加重期(AECOPD)并发Ⅱ型呼吸衰竭的临床疗效。〖HTH〗方法〖HTSS〗:对照组30例AECOPD并Ⅱ型呼吸衰竭患者给予吸氧、抗感染、化痰平喘等常规治疗,观察组38例AECOPD并Ⅱ型呼吸衰竭患者,在常规治疗的基础上同时进行BiPAP通气治疗。观察两组治疗前及治疗后4 h、24 h及72 h动脉血气变化,比较治疗前后呼吸频率、心率和白细胞计数以及血浆内皮素-1含量的变化,观察两组住院时间、气管插管率及病死率的变化。结果:观察组治疗后4 h、24 h及72 h动脉血气pH、PaO2、SaO2、PaO2/FiO2均明显高于对照组(Plt;0.05),PaCO2明显下降(Plt;0.05)。呼吸频率、心率和白细胞计数较对照组均下降(Plt;0.05)。观察组血浆内皮素-1含量恢复至正常水平快于对照组,观察组和对照组患者住院时间、气管插管率及病死率均有显著性差异(Plt;0.05)。〖HTH〗结论〖HTSS〗:BiPAP治疗慢性阻塞性肺疾病急性加重期并发Ⅱ型呼吸衰竭患者疗效确切,能减少住院时间、降低插管率和病死率。

  • Effects of Aspergillus fumigatus spores on airway inflammation and airway responsiveness on asthmatic rat model

    Objective To explore the effects of Aspergillus fumigatus(A. fumigatus) spores on airway inflammation and responsiveness in asthmatic rats.Methods Seventy male Wistar rats were randomly divided into Ⅰ and Ⅱ groups(n=35 in each group),then Group Ⅰ and Group Ⅱ were subdivided into a normal control group(n=5),an asthma group(n=10),a spores-treated control group(n=10),and a spores-treated asthma group(n=10).The rats were sensitized to ovalbumin(OVA) and challenged with aerosol OVA to establish the asthma model.The effects of A. fumigatus spores on asthmatic rats before and after OVA aerosol challenging were investigated in Group Ⅰ and Group Ⅱ,respectively.The parameters associated with bronchial epithelial damage were observed by total protein concentration in BALF measured by BCA method.Total and differential cell counts in BALF were also counted.The airway resistance and airway responsiveness were calculated by transpulmonary pressure and gas flow rate.Results In Group Ⅰ,the total protein in BALF in the asthma group treated with A. fumigatus spores before OVA challenging(Group CA) was increased remarkably compared to the asthma group(Group A1)[(1.125±0.254)μg/mL vs(0.825±0.173)μg/mL,Plt;0.01].The nonspecific airway resistances induced by different concentration of acetylcholine in Group CA [(0.997±0.196)cm H2O•mL-1•s-1,(1.123±0.142)cm H2O•mL-1•s-1,(1.130±0.197)cm H2O•mL-1•s-1]were increased significantly compared to Group A1 [(0.655±0.089)cm H2O•mL-1•s-1,(0.687±0.048)cm H2O•mL-1•s-1,(0.821±0.043)cm H2O•mL-1•s-1](all Plt;0.05).In Group Ⅱ,however,the above parameters in the asthma group treated with A. fumigatus spores after OVA challenging(Group AC) were not dramatically increased compared with the asthma group(Group A2)(all Pgt;0.05).The differences in the total and differential cell counts in BALF in Group CA were not remarkable compared to other subgroups in Group Ⅰ(all Pgt;0.05).But the BALF neutrophil count in Group AC was increased obviously compared to Group A2 [(2.488±0.420)×106 vs (0.936±0.459)×106,Plt;0.05].Conclusion These data indicate that exposure to A. fumigatus spores before challenging causes aggravated epithelial damage and increased airway resistance in an asthma rat model.
